<p><b>目的</b>　日本では多くの女性が月経時，下着に装着する生理用ナプキンを使用しているが，環境意識の高まりやライフスタイルの多様化，女性の健康問題に対する理解の高まりによって近年，生理用品は大きく多様化している。とくに膣内に挿入して使用する生理用タンポンについては日本国内でも使用者が増えているのではないかと推察されるが，日本人女性における生理用タンポンの正確な使用実態は明らかでない。本研究では日本人女性における生理用タンポンの使用実態とそれに関連する心理的・社会的要因，トキシックショック症候群（TSS）などの有害事象の認知度を明らかにすることを目的として調査を行った。</p><p><b>方法</b>　2024年3月に18–54歳の女性14,000人を対象にインターネット調査を実施した。生理用タンポンの使用状況について「使用している/以前使用していた/使用したことがない」のいずれかを選択することで評価し，その理由について調査した（一次調査）。さらに使用状況ごとにそれぞれ1,000人を年齢階級別に無作為に選び出し，月経症状や経済状況について調査を行い比較した。生理用タンポン使用者については生理用タンポン使用方法，有害事象の一つであるTSSの知識の有無も調べた（二次調査）。</p><p><b>結果</b>　不適切回答者を除いた13,983人が一次調査の解析対象とされた。解析対象者のうち「使用している」は13.0%（95%CI：12.4–13.5），「以前使用していた」は28.8%（27.7–29.2），「使用したことがない」は58.2%（57.8–59.5）であった。「使用している」群の使用理由は下着が汚れることがほとんどない（51.7%）の割合が最も高かった。また二次調査の結果，「使用している」群において25.4%が使用前の手指衛生を実施しておらず，TSS認知度は16.3%だった。また，タンポン使用と月経症状，経済的事情との横断的関連について，「使用している」群の月経症状の申告数（平均）はほかの2つの群よりも有意に多く（3.58個，3.26個，2.96個；<i>P</i>＜0.001），「経済的困難により生理用品が購入できなかった経験があった」と回答した割合とも関連が認められた（16.9%，6.8%，3.6%；<i>P</i>＜0.001）。</p><p><b>結論</b>　欧米と比較して日本の生理用タンポン使用者の割合は13.0%と低いが，安全な使用方法を啓発する上でこれらの使用理由や社会的要因を考慮することが重要であると考えられる。</p>

Both underweight and obesity have been linked to increased risk of disability, yet evidence on the relationship between body mass index (BMI) and activities of daily living (ADL) impairment remains inconsistent. To examine these associations in adults, we conducted this systematic review and meta-analysis following the standard methods and reported according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 guidelines. Six databases were searched from inception to August 12, 2024. The protocol is registered in the PROSPERO (CRD42022357046). Abstracts and full-text articles describing associations between BMI and ADL impairment were screened using Rayyan software. Study quality was assessed using the RoBANS tool. A random-effects model meta-analysis was performed using metaanalysisonline, a web-based meta-analytic platform, with BMI categories of <18.5 (underweight), 25–29.9 (overweight), and ≥30 (obese) as exposures, and 18.5–24.9 (normal) as the reference. Of the initial 25,212 articles identified, 132 studies met the inclusion criteria. The meta-analysis showed positive association in both longitudinal studies [odds ratio (OR) 1.77, 95% confidence interval (CI): 1.54–2.03] and cross-sectional studies (OR: 1.56, 95% CI: 1.38–1.76). Overweight individuals exhibited a moderate increase in ADL risk in longitudinal studies (OR: 1.19, 95% CI: 1.09–1.30), but not in cross-sectional studies (OR: 1.06, 95% CI: 0.95–1.18). Both longitudinal and cross-sectional studies revealed a positive association between underweight participants and ADL impairment (OR: 1.20, 95% CI: 1.02–1.42; OR: 1.46, 95% CI: 1.24–1.72, respectively). Individuals with higher BMI, as well as those who are underweight, are at increased risk of ADL limitations.

Purpose: This study aimed to (1) identify distinct multidimensional fatigue profiles among employed cancer survivors, (2) examine associations between workplace factors and these profiles, and (3) assess how these profiles are associated with occupational outcomes. Methods: Seventy-two employed cancer survivors aged 27 to 64 years (median 9.4 years since diagnosis) from a local government in Japan completed questionnaires in 2023 assessing cancer-related fatigue (Cancer Fatigue Scale), workplace factors (job demands, job control, workplace support), and occupational outcomes (work engagement, presenteeism, cognitive function at work, and quality of life). Fatigue profiles were identified using hierarchical cluster analysis. Associations between workplace factors and profiles were examined using multinomial logistic regression. Occupational outcome differences among profiles were examined using linear regression. Results: Three profiles were identified: Low Global Fatigue (n = 12), Dominant Mental Fatigue (n = 36), and High Global Fatigue (n = 24). Workplace support—particularly supervisor understanding, colleague understanding, and schedule flexibility—was associated with approximately 75% lower odds of belonging to the High Global Fatigue group. Job demands and job control showed no associations. The High Global Fatigue group reported lower work engagement, higher presenteeism, reduced cognitive function at work, and poorer quality of life compared to the other groups. Conclusions: Three multidimensional fatigue profiles were identified among employed cancer survivors. Workplace support factors were more strongly associated with fatigue profiles than job characteristics. These profiles were associated with occupational outcomes. Implication for Cancer Survivors: These distinct fatigue profiles can guide personalized workplace interventions to optimize support for employed cancer survivors.

This study investigated trends in psychological distress and mental health service use in Japan before and after the COVID-19 pandemic using nationally representative data from the Comprehensive Survey of Living Conditions (2013–2022). Psychological distress was assessed using the Japanese version of the Kessler 6 Scale, with scores of 5–12 indicating moderate distress and scores of ≥13 indicating high distress. Mental health service use was defined as regular visits to hospitals or clinics for depression or other psychological disorders. Analysis of data from 1,765,102 participants revealed that while the prevalence of moderate distress decreased from 24.9 % in 2019 to 21.8 % in 2022, the prevalence of high distress increased from 4.7 % to 5.0 % over the same period. Notably, increases in high distress were most prominent among aged 35–49 population. Cross-sectionally, women aged 26–34 showed highest prevalence of high distress (7.6 %). Additionally, mental health service use rose markedly, especially among young adults with high distress, with women consistently reporting higher distress and service utilization than men. These findings suggest a polarization in psychological distress in Japan, with a decline in moderate symptoms concurrent with a rise in severe distress, highlighting emerging gaps in mental health care. The results underscore the need for targeted interventions and the expansion of accessible, diverse mental health services to address unmet needs, particularly among vulnerable subgroups including young women and middle-aged population.

Aims/hypothesis: We aimed to investigate the association between reversion to normoglycaemia among individuals with prediabetes (fasting plasma glucose 5.6–6.9 mmol/l in the absence of other criteria for type 2 diabetes) and the subsequent risk of type 2 diabetes, and to examine whether concurrent favourable cardiometabolic risk factor profiles modify this association. Methods: We used individual-level data from prospective cohorts in the USA, Australia and Asia. Participants with prediabetes at baseline, with at least two follow-ups (n=8191) at median intervals of 2.9 years (IQR 2.3–9.0) and 3.1 years (IQR 2.6–3.6), were classified into restoration of normoglycaemia and persistent prediabetes groups based on the glucose status at the first follow-up (normoglycaemia or prediabetes). Type 2 diabetes occurrence was assessed at subsequent follow-ups. Hierarchical mixed-effects proportional hazards Weibull models estimated type 2 diabetes risk, adjusting for age, sex and cardiometabolic risk factors. A subgroup analysis evaluated the combined association of normoglycaemia restoration and normal cardiometabolic risk factor levels on subsequent type 2 diabetes risk. Results: In individuals with prediabetes, normoglycaemia restoration compared with persistent prediabetes was associated with a 51% lower risk of developing type 2 diabetes. Even lower risks were observed among individuals with concurrent favourable cardiometabolic profiles, including non-smokers (HR 0.20, 95% CI 0.10, 0.31), and those with normal BMI (0.16, 95% CI 0.06, 0.27), waist circumference (0.22, 95% CI 0.12, 0.33), waist-to-height ratio (0.15, 95% CI 0.03, 0.26), WHR (0.17, 95% CI 0.05, 0.28), and systolic (0.20, 95% CI 0.11, 0.30) and diastolic (0.25, 95% CI 0.12, 0.38) blood pressure, triacylglycerol (0.24, 95% CI 0.13, 0.35) and HDL-cholesterol levels (0.21, 95% CI 0.13, 0.29). Weight loss combined with normoglycaemia restoration was also associated with lower type 2 diabetes risk (0.18, 95% CI 0.07, 0.30) compared with weight gain and persistent prediabetes. Conclusions/interpretation: Our results suggest that prioritising normoglycaemia restoration during the prediabetes stage in clinical guidelines may contribute to reducing the risk of type 2 diabetes, particularly when accompanied by favourable cardiometabolic health profiles.

Background: Insulin resistance in adipocytes, manifested as high basal circulating free fatty acid (FFA) is thought to contribute to the development of type 2 diabetes mellitus (T2DM). However, the association between adipocyte insulin resistance (Adipo-IR) index and T2DM has rarely been explored in prospective studies. We examined this association in a middle-aged Japanese workers’ cohort. Since the association may differ according to the degree of overall adiposity, the analysis was stratified by the presence of overweight/obesity defined with body mass index (BMI). Methods: A total of 3,257 subjects (men 2501, women 756) aged 35–66 years were followed-up for up to 17 years. T2DM incidence was defined as fasting blood glucose level ≥126 mg/dL, glycated hemoglobin level ≥6.5%, or self-reported initiation of glucose-lowering medications. Adipo-IR was calculated as the product of FFA (mmol/L) and insulin (pmol/L) obtained from baseline fasting blood samples and divided into sex- and BMI category (<25 or ≥25 kg/m²)-specific tertiles. Cox-proportional hazards model was used to estimate hazard ratios (HRs) and 95% confidence intervals (CIs) adjusted for age, BMI, smoking status, physical activity, drinking habit, and family history of diabetes. Results: During a median of 14.6 years of follow-up, 365 developed T2DM. Compared with the lowest tertile, T2DM risk was significantly increased among the highest tertile category in overweight/obese men (HR 2.94; 95% CI, 1.76–4.90) and women (HR 4.24; 95% CI, 1.08–16.61). Conclusion: Adipo-IR was positively associated with T2DM risk in overweight/obese men and women.

Chronotype, which reflects an individual’s preference for activity and sleep timing, has been associated with various chronic conditions, particularly the evening chronotype. Metabolic syndrome (MetS) is a cluster of metabolic abnormalities that collectively increase the risk of developing type 2 diabetes mellitus and cardiovascular diseases. Previous studies on chronotype and MetS have been inconsistent, and none has explored the longitudinal association between MetS and its components. Therefore, this study investigated these associations in middle-aged Japanese civil servants. We analyzed 2231 non-rotating, non-night shift workers (aged 29–69, 71.6% male) from the Aichi Workers’ Cohort Study who participated in surveys in 2018 (baseline) and 2022 and were free of MetS at baseline. Participants were classified into morning, evening, or intermediate chronotypes using the shortened Morningness–Eveningness Questionnaire. MetS was defined according to the harmonized criteria for Japanese, based on waist circumference (≥85 cm for men and ≥90 cm for women), triglycerides, HDL cholesterol, blood pressure, and blood sugar levels. Over 4 y, 232 participants (10.4%) developed MetS, with a cumulative incidence rate of 9.9% in morning, 17.6% in evening, and 9.5% in intermediate types. Logistic regression analysis adjusted for sex, age, educational background, occupation, leisure-time physical activity, alcohol intake, sleep duration, smoking status, snoring, night-time awakening, sleep medication use, working hours, breakfast eating habit, and medication use for diabetes, hypertension, and dislipdemia, revealed that the evening type individuals had a significantly higher risk of MetS (OR: 2.30, 95% CI: 1.48–3.57) compared to intermediate types. In conclusion, evening chronotype was independently associated with a higher risk of developing MetS in middle-aged Japanese civil servants.

Objectives: This study aimed to investigate the association between occupational stress responses indicated by fatigue and nonspecific physical complaints and the incidence of subsequent cardiovascular disease (CVD) in middle-aged Japanese workers. Methods: A cohort of 4,820 Japanese workers (3876 men and 944 women) aged 35–65 years was followed up from 2007 to March 2022 for CVD incidence. Fatigue and nonspecific physical complaints were evaluated using the 57-item Brief Job Stress Questionnaire, and each was categorized into two groups based on the presence or absence. Hazard ratios (HRs) and the corresponding 95% confidence intervals (CIs) for stress responses to CVD were estimated using a multivariable Cox proportional hazard model adjusted for age, sex, smoking status, alcohol consumption, regular exercise, sleep duration, working hours, body mass index, systolic blood pressure, total cholesterol, and histories of hypertension, hyperlipidemia, and diabetes, night shift and job type. Results: During the median follow-up of 13.7 years, 116 patients with CVD were identified. The presence of fatigue was independently associated with CVD incidence (HR: 2.28, 95% CI: 1.05–4.94). Similarly, the presence of nonspecific physical complaints was significantly associated with CVD incidence (HR: 2.54, 95% CI: 1.43–4.52). The sex-stratified analysis showed consistent results in men but not in women. Conclusions: These findings suggest that fatigue and nonspecific physical complaints may serve as early indicators of CVD risk among middle-aged Japanese male workers. Incorporating routine assessment of these stress responses into workplace health screening could facilitate targeted prevention strategies.

Association between eveningness and low back pain among public servants in Japan: a cross-sectional analysis of the Aichi Workers' Cohort Study 査読有り Open Access

Purpose: Low back pain (LBP) is a common occupational health problem. Chronotype, an individual’s preferred timing of activity and sleep, has been linked to pain perception. Previous studies have found a potential association between chronotype and LBP, although the findings were limited by small sample sizes and restricted population types. We investigated the association between evening chronotype (eveningness) and LBP in a cohort comprising public servants of various ages. Methods: We conducted a cross-sectional analysis of 4728 full-time public servants in Japan. LBP was assessed using a self-reported question. Individuals’ chronotypes were determined using the reduced Morningness–Eveningness Questionnaire. The association between chronotype and LBP was analyzed using the chi-squared test and multiple logistic regression models adjusted for age, sex, occupation, overtime work, use of the internet/email, body mass index, smoking status, exercise habits, sedentary behavior, sleep duration, sleep medication use, and depressive symptoms. Results: In total, 30% of the subjects reported LBP. The most common chronotype was intermediate type (51%), followed by morningness (38%) and eveningness (11%). A significant association between eveningness and LBP was observed. After adjusting for potential confounders, individuals with eveningness had a higher prevalence of LBP than those with morningness (36.2% vs. 28.7%; odds ratio: 1.46; 95% confidence interval: 1.16–1.83). Conclusion: Eveningness was statistically associated with LBP in a cohort of public servants. The present findings would strengthen the association’s generalizability to more varied populations.

Objective: This study analyzed the changes in health-related quality of life (HRQOL) and employment status of patients undergoing lung cancer surgery in Japan. Methods: This was a single-center, prospective study on patients who underwent lung anatomical resection. The eligible patients completed self-reported HRQOL and employment surveys at baseline and 6 and 12 months postoperatively. HRQOL was assessed using questionnaires including the European Organization for Research and Treatment of Cancer Core Quality of Life questionnaire (EORTC QLQ-C30) and EORTC QLQ and Lung Cancer module and additional social engagement and work-related stress evaluation tools. Results: In total, 93 patients completed the baseline survey, and 80 provided survey data at 6 months postoperatively. The HRQOL scores of several factors significantly declined immediately after the surgery and then gradually improved. The EORTC global health score, which represents overall health status, returned to baseline levels at 12 months postoperatively. However, symptoms such as fatigue, dyspnea, and coughing did not return to baseline levels at 12 months postoperatively. Approximately 68% of the patients who were employed preoperatively continued to work at 12 months postoperatively. Conclusions: Lung cancer surgery significantly affected the HRQOL and employment status of the patients within the first 6 months after surgery. For patients who decide to return to work before full recovery of QOL, we consider the need for enhanced support to assist them as they can reintegrate into work and activities of daily living.

Relationship between abdominal visceral adipose tissue and cardiovascular events in patients with acute coronary syndrome 査読有り Open Access

Abdominal visceral adipose tissue (AVAT) is associated with the incidence of cardiovascular events (CVEs). We retrospectively evaluated the association between AVAT and the incidence of CVEs in 602 patients with acute coronary syndrome (ACS) who underwent percutaneous coronary intervention (PCI). Patients were divided into four groups according to the quartiles of AVAT areas using computed tomography. The incidence of CVEs (cardiovascular death, ACS recurrence and stroke) during the follow-up period (median 49.5 months) was evaluated. Cox analysis adjusting for cardiovascular risk factors revealed that the AVAT quartile classification exhibited a significant association with the incidence of CVEs. The risk in quartile 3 (moderate AVAT areas, ≥ 106.0 to < 142.6 cm²) was significantly lower than in quartiles 1 (low AVAT areas, < 71.0cm²; P < 0.01; hazard ratio [HR], 5.06), 2 (mild AVAT areas, ≥ 71.0 to < 106.0 cm²; P < 0.01; HR, 4.25) and 4 (severe AVAT areas, ≥ 142.6 cm²; P < 0.01; HR, 4.52). Polynomial analyses revealed that quadratic model was the most appropriate to illustrate the relationship between AVAT area and the hazard ratios for CVEs (corrected Akaike's information criterion, 49.2; R², 0.47). The AVAT area and the incidence of CVEs exhibited a U-shaped relationship in patients with ACS undergoing PCI independent of conventional cardiovascular risk factors. The risk of CVEs was the lowest in patients with moderate AVAT areas. Evaluating AVAT may provide additional information for the assessment of long-term prognosis in patients with ACS.

Age-related changes in anthropometric measurements, body composition, and physical function among middle-aged and older Japanese community-dwellers: A longitudinal study 査読有り Open Access

Objective: To quantify annual changes and develop population-normalized aging curves for height, body weight, and grip strength according to specific birth cohorts. Methods: This longitudinal study analyzed data from the National Institute for Longevity Sciences – Longitudinal Study of Aging, following 1901 community-dwelling adults aged 40−79 years for up to 12 years. Annual changes in height, body weight, body mass index (BMI), body fat percentage, skeletal muscle mass, and grip strength were assessed by sex using linear mixed-effects models. Birth cohort-specific reference values according to sex were developed using participants who participated in at least one wave between 1997 and 2022 (n = 1990) with generalized additive mixed models. Results: Significant negative follow-up years × age interactions were observed for height, body weight, BMI, skeletal muscle mass, and grip strength in men (all p-values < 0.001) and for height, body weight, BMI, body fat percentage, and skeletal muscle mass in women (all p-values < 0.01). Grip strength began declining preceding any significant change in other indicators. For height and grip strength, birth cohort-specific reference values for age-related changes showed a gradual decline with age in both sexes, with more recent birth cohorts being consistently taller but having weaker grip strength than earlier cohorts. Men experienced slight weight increases until ages 60−70 years, followed by a decline. Women maintained a stable weight until age 60 years before declining. Conclusions: These reference values provide evidence-based tools for detecting age-related changes in body composition and function among relatively healthy participants and guiding preventive interventions in clinical and community settings.

Prevalence, severity, and risk factors of cancer-related fatigue among working cancer survivors: a systematic review and meta-analysis 査読有り

Purpose: The aim of this study is to evaluate the prevalence, severity, and risk factors of cancer-related fatigue among currently working cancer survivors. Methods: We searched the PubMed, Embase, Scopus, CINAHL, Cochrane Library, and ICHUSHI databases. The risk of bias was evaluated independently using the Risk of Bias Assessment Tool for Non-randomized Studies (RoBANS). A meta-analysis was conducted to determine the prevalence, severity, and related factors associated with cancer-related fatigue among currently working cancer survivors. Results: Our meta-analysis included 18 studies and revealed that 42.2% of currently working cancer survivors experience cancer-related fatigue. The fatigue severity in this group was significantly higher than that in workers without cancer (absolute standardized mean difference (SMD) = 0.67), but lower than that in cancer survivors who had previously worked and were not currently working (absolute SMD = 0.72). Distress was identified as a potential risk factor for cancer-related fatigue in working cancer survivors (partial correlation coefficient = 0.38). Conclusions: The high prevalence of cancer-related fatigue among employed cancer survivors underscores the need for targeted workplace interventions and fatigue management strategies. While the severity of fatigue is less than that seen in non-working survivors, the comparison with the general working population highlights a significant health disparity. The association between distress and fatigue suggests the necessity for a holistic approach to fatigue management that considers both physical and mental factors in working cancer survivors. Implications for Cancer Survivors: Our findings highlight the critical need for healthcare professionals and employers to monitor fatigue levels among working cancer survivors and offer appropriate support.

Psychiatric and psychological adverse effects associated with dulaglutide, semaglutide, and liraglutide: A vigibase study 査読有り 国際共著 Open Access

Background: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are crucial in managing type 2 diabetes mellitus (T2DM) and obesity. Agents like dulaglutide, semaglutide, and liraglutide mimic endogenous GLP-1, enhancing insulin secretion and promoting satiety. Beyond glycemic control, they offer cardioprotective and neuroprotective benefits. However, concerns about psychiatric adverse effects have emerged, necessitating a systematic assessment of their safety profile. Material & Methods: This multinational pharmacovigilance study utilized VigiBase® data up to December 1, 2024, focusing on adverse drug reaction (ADR) reports for GLP-1 RAs. Psychiatric ADRs were classified using MedDRA terminology. Disproportionality analyses were conducted via logistic regression to calculate reporting adjusted Reporting Odds Ratios (aRORs) within reports involving antidiabetic or anti-obesity medications. A causal forest model assessed the individual treatment effect (ITE) of semaglutide on depression and suicidality reporting. Results: Among 2,061,901 reports, 21,414 involved psychiatric ADRs for GLP-1 RAs. Significant signals were observed for anxiety (aROR: 1.26, 95%CI: 1.18–1.35), depressed mood disorders (aROR: 1.70, 95%CI: 1.57–1.84) and suicidality (aROR 1.45, 95%CI: 1.29–1.63) with semaglutide, and eating disorders with all three GLP-1 RAs (aRORs between 4.17 and 6.80). The causal forest model estimated an average treatment effect of 0.0046 for semaglutide on depression and suicidality reporting, with significant heterogeneity across regions and demographic groups. Discussion: The study found no significant increase in overall psychiatric ADR reporting for GLP-1 RAs, except for eating disorders and depression/suicidality in semaglutide-treated patients. Sensitivity analyses before June 4, 2021, found no signals, aligning with RCT data. Post-marketing reports linked semaglutide to depression and suicidal ideation, prompting regulatory investigations. The causal forest model revealed significant heterogeneity in the psychiatric safety profile of semaglutide, with regional factors influencing ADR reporting. Conclusion: While GLP-1 RAs do not show a significant increase in overall psychiatric ADR reporting, findings suggest that clinicians should remain vigilant for potential depressive symptoms and suicidality, particularly in individuals with obesity treated with semaglutide since its approval for weight management.

Aim: The best modality for evaluating lateral pelvic lymph node (LPLN) metastases from rectal cancer remains unclear. This study compared the diagnostic ability of 1-mm computed tomography (CT) and 3-mm magnetic resonance imaging (MRI) in identifying LPLN metastases based on size. Methods: This observational study analyzed not individual patients but 191 sides from 100 rectal cancer patients without preoperative treatment for whom preoperative CT and MRI and corresponding pathological results for LPLNs were available. A swollen LPLN was defined as an LN with a short-axis size of ≥5 mm on 1-mm CT. Results: LPLNs were detected significantly more frequently with 1-mm CT than with 3-mm MRI (p < 0.001). Among the 117 sides without swollen LPLNs, metastasis was observed in 1.7% of patients. In contrast, LPLN metastasis was confirmed in 28.4% of 74 sides with swollen LPLNs. In the evaluation of swollen LPLNs, 3-mm MRI yielded a 34% improvement in the diagnostic performance of LPLN metastasis over 1-mm CT (categorical net reclassification improvement: 0.341, p = 0.045). Conclusions: In conclusion, 1-mm CT should be performed preoperatively to evaluate LPLNs. Further evaluations can be omitted in the absence of swollen LPLNs. In patients with swollen LPLNs, a careful assessment of LPLN metastasis should be conducted via additional MRI, even in early T-stage tumors.
