    Background: A child's death affects not only family members but also the health-care professionals involved in patient care. The education system for bereavement care in Japan, however, is not set up in a systematic way, and the care provided is based on the individual experience of the health-care professional. The aim of this study was to investigate pediatrician awareness of and actual circumstances involved in bereavement care in Japan. Methods: A qualitative descriptive study was conducted at four facilities in Japan. Data collected using semi-structured interviews of 11 pediatricians were assessed using inductive qualitative analysis. Results: Pediatrician recognition of the elements of bereavement care was categorized as follows: (i) developing relationships with families before a child's death is important in bereavement care; (ii) after the child dies, family involvement is left to the doctor's discretion; (iii) coping with a child's death myself through past experience is essential; (iv) doctors involved in a child's death also experience mental burden; and (v) a system for the family's bereavement care must be established. Two categories were established according to actual circumstances involved in bereavement care: (i) attention must be given to the emotions of the families who lost a child; and (ii) doctor involvement with bereaved families depends on doctor awareness and expertise. Conclusion: Japanese pediatricians provided bereavement care to families who lost their children in a non-systematic manner. This is necessitates improvement of the self-care of health-care professionals with regard to grief by improving bereavement care-related education. Additionally, health-care professionals must be trained, and a national-level provision system must be established to provide high-quality bereavement care to families who lose a child.

    子どもを亡くした遺族への支援は、我が国ではまだ体系化されていない。そこで本研究では、体系化の第一歩として、遺族へのビリーブメントプログラムの開発を目的とした。
    本研究では、先行研究およびアクセスしづらかった対象の実態把握とそれらを含むプログラムを開発する。また、開発したプログラムは、病院の枠だけにとどまらず、第3の場であるNPO法人福岡子どもホスピスプロジェクトとも連携し、病院・地域と連携したビリーブメントプログラムを開発・実装する。さらに、子どもの生前から関わる医療者へのビリーブメントに対する教育・支援についてもプログラムに含め、遺族・医療者双方の支援を目指す。
    本年は、ビリーブメントプログラム開発の初期段階として、専門家や関係職種とのディスカッション、研修会への参加、文献検討および医療者および子どもを亡くした親の基礎調査を行った。
    文献検討は、子どもを亡くした親・きょうだい児、医療者の視点から実施している。文献検討の結果、特にレビューの少なかった「きょうだい児」については、きょうだい児を亡くした子どものグリーフ体験についてシステマティックレビューに着手し、継続中である。
    基礎調査では、死別による子どものグリーフの研修会参加者(医療・福祉関係者)に対して、研修会の内容や実行可能性、参加者のグリーフに対する認識について質問紙調査を行った。結果について、現在集計・分析中である。子どもを亡くした親を対象とした調査は、研究参加者がまだ限られているが、得られたデータについては、現在分析中である。研究参加者がまだ十分に得られていないため、今後もデータ収集を継続する予定である。

    The aim of this study was to develop a model of nursing assistance and problem solving to improve the quality of life of children and parents from school age to adolescence after liver transplantation. Child and family monitoring indicators were developed based on the results of a survey of children and parents, interviews, and a review of the literature. The survey results and literature review led to a conceptual framework, nursing assistance model, and pattern-specific nursing assistance guidelines for improving the quality of life of children and parents from late school age to adolescence after living donor liver transplantation. The developed nursing assistance model was practiced, evaluated, and revised, and the content was refined to construct a nursing assistance model for improving the quality of life of children and parents from late school age to adolescence after liver transplantation.

    The aim of this study was to clarify nursing intervention approach to improve the quality of life (QOL) of children liver transplant recipients and their parents. Nine parent-child dyads were enrolled and all children were donor liver transplant recipients. Members of the parent-child dyads completed the questionnaires and participated in semi-structured interviews. The results showed that the post-transplantation daily life of the parent-child dyads had four distinct patterns. In addition, perspectives for nursing intervention approach to improve the QOL were elucidated.

    The main objectives of this project were to promote the effect of peer support by giving concrete form to this disease, and to further raise awareness of the current status of this rare disease, by listening directly to the voices of female patients with cloacal abnormalities, which are defecation disorders and reproductive organ dysfunction. Through a questionnaire survey, we clarified the needs from the patient's perspective, the nature of support, the expectations of peer support for patients and their families, and its effects. We conducted interviews with adult patients, analyzed the results qualitatively and inductively, and submitted them to an English journal. The public lecture was held in a hybrid format as an opportunity for three adult patients to share their experiences and thoughts. It left a strong message for the viewers, the citizens and those involved with the disease, and also received cooperation from the media.

    To explore the experiences accompanying the grief of families who have lost a child, a visual narrative method (drawing interviews) was used to investigate the past, present, and future relationships with the child, as well as the grief, targeting parents who have lost a child. A total of 56 drawings were collected from 16 participants, and pattern analysis and case studies were conducted based on their narratives.The changes in the relationship with the child were characterized by a reversal of roles between subject and object, such as transitioning from "embracing" and "watching over" in the past and present, to being "embraced" and "watched over."
    Furthermore, to create a space for sharing grief and explore the meaning of that space, ten sharing sessions were held during the period. The meanings generated by these spaces included "feeling that one is not alone," "a space to express sadness," and "a space to talk about the child."

    This study was Action Research for exploration the possibility of Children's Hospice in Japan. The purpose of this research was to clarify the experiences of children with life threatening illness and their parents, and their palliative care needs. Palliative care needs of them are needs for their development. Especially, QOL in infancy and after graduate high school were decreasing. We have concerned with three cases of children in terminal stage and found not only medical care but also the necessity of children's and family developmental support. Next, we made some citizen forum and meetings as enlighten programs for pediatric palliative care (children's hospice), and some programs to support the children's and the family's QOL. Finally, we made a network for children's hospice through cooperating with not only heath care providers but also ordinary people and people in multiple ranges. Through the process of action research, a direction for children's hospice in Japan were showed.

    This research aimed to uncover the state of families with children in outpatientchemotherapy to realize nursing care supports their health promotion. The parents ofchildren with acute lymphoblastic leukemia who have faced outpatient chemotherapywere interviewed and their daily life experience was studied by questionnaire method. Our survey revealed that most parents suffered from difficulties in managing theirchild' s physical condition during the outpatient therapy. Nursing supports finelytailored to each family are strongly required.

    目的:子ども達の健全育成のための生活習慣の改善と睡眠衛生からみた介入法を構築するために、遅寝の規定要因を明らかにすることと、特に両親の子ども睡眠に対する認識の特徴を知り、夜型生活への改善について、質的・量的視点から乳幼児期の睡眠の安定性が精神に及ぼす影響を評価する。
    方法:平成22年度第3ステップ;睡眠日誌、シール帳による睡眠・覚醒行動を継続観測(睡眠教材「早寝、早起き元気さんシール帳」を用いた家庭実践)による睡眠行動の定着と、5名の親子への睡眠の質評価(Actigrapと携帯脳波による)および、睡眠・生活行動改善に向けた親子取り組みのインタビューにより、睡眠健康習慣の介入法を検討した。
    結果:(1)1-2年目に実施してきた睡眠健康教室に参加した親子インタビューから抽出されたサブカテゴリーを基に、睡眠環境の工夫、子どもの睡眠に関する親の認識など「子どもの睡眠を整えるための親の困難さ」について検討した。父親の帰宅時刻、子どもとの過ごす時間など親子の関係性から遅寝改善の方向性を探るための演繹的なアプローチは今後の課題である。
    (2)実験・計測的アプローチによる事例検討:親子で早寝生活習慣に取り組んだ約100名ChiSCoP(Child Sleep Cohort Project)会員から、同意の得られた5名について、携帯脳波およびアクチグラフ(Actigraph)と睡眠日誌を同時に継続測定し、夜型生活の睡眠リズムの特徴と睡眠の質評価を行なった。5日間の携帯脳波による睡眠の質評価では、深いノンレム睡眠は1日36.5-63.4%前後を認め,睡眠効率79.3-96.5%であった。年2回10日間の24時間睡眠日誌を記載することで、経年的に生活リズムがついてきたこと、シール帳チェックリストによって、「太陽さんにごあいさつ」「トイレ習慣」などの項目に子ども自身が早起き生活への前向きな取り組みの行動促進化を図っていた。
    (3)教育方法の充実化;「お父さん、お母さんも早起きさん」の達成度が低かったが、子どもの発達年齢に合わせた認知行動的接近を目指した、早寝得点満点時の王冠マーク獲得は、親子の睡眠習慣改善の意識づけになっていた。「眠育」スローガンに睡眠健康教育やシンポジウム、地域でのシール帳の取り組みをDVD教材やWeb指導教材につなげることができた。
