BACKGROUND: EN-DEAVOR was a multi-institutional retrospective study evaluating the effectiveness and safety of trastuzumab deruxtecan (T-DXd) in gastric cancer patients. This secondary analysis investigated prognostic factors for real-world progression-free survival (rwPFS) and objective response rate (ORR) for T-DXd as third- or later-line treatment. PATIENTS AND METHODS: Patients aged ≥20 years with histopathologically confirmed human epidermal growth factor receptor 2 (HER2)-positive unresectable advanced or recurrent gastric or gastroesophageal junction adenocarcinoma, who had worsened after chemotherapy, were included. Patients received T-DXd as third- or later-line therapy between September 2020 and September 2021. Univariate and multivariate analyses identified prognostic factors for rwPFS and ORR. RESULTS: Of the 307 patients, 75.6% were male and 69.1% were aged ≥65 years. The median duration of prior trastuzumab treatment was 6.5 months (range 0-81.5 months). Multivariate analysis showed HER2 immunohistochemistry (IHC) 3+ [hazard ratio (HR) 0.65, 95% confidence interval (CI) 0.49-0.86], intestinal type lesions (HR 0.59, 95% CI 0.43-0.79), modified Glasgow Prognostic Score (mGPS) 0 and 1 (HR 0.71, 95% CI 0.53-0.95), and longer duration of prior trastuzumab treatment (≥ median) (HR 0.75, 95% CI 0.58-0.97) as positive prognostic factors for rwPFS. Longer prior trastuzumab treatment was also a positive prognostic factor for ORR (odds ratio 2.02, 95% CI 1.13-3.63). CONCLUSIONS: Patients with clinical benefits from prolonged trastuzumab treatment are likely to benefit from T-DXd. Similarly, HER2 IHC 3+, intestinal type lesions, and better mGPS (0 or 1) are associated with longer rwPFS. However, T-DXd should not be withheld even in patients without these factors, as a median PFS of 3.42 months was observed in those with the shortest prior trastuzumab exposure.

BACKGROUND: Trastuzumab-deruxtecan (T-DXd) was approved for the treatment of HER2-positive patients with advanced gastric cancer in Japan based on the results of the DESTINY-Gastric01 trial. This study aimed to collect real-world data and evaluate the effectiveness and safety of T-DXd. METHODS: Patients aged ≥ 20 years at the start of T-DXd administration with a histopathologically confirmed diagnosis of HER2-positive unresectable advanced or recurrent gastric or gastroesophageal junction (GEJ) adenocarcinoma that had worsened after chemotherapy were enrolled in this retrospective cohort study. Key outcomes included T-DXd treatment status, overall survival (OS), real-world progression-free survival (rwPFS), time to treatment failure (TTF), objective response rate and frequency of grade ≥ 3 adverse events (AEs). RESULTS: Of the 312 patients included in the analysis, 75.3% were male, the median (range) age was 70.0 (27.0-89.0) years, 12.2% had an ECOG PS ≥ 2, 43.3% had ascites and the initial T-DXd dose was > 5.4- ≤ 6.4 mg/kg in 78.2% of patients. The median (95% confidence interval) OS, rwPFS and TTF (months) was 8.9 (8.0-11.0), 4.6 (4.0-5.1) and 3.9 (3.4-4.2), respectively. The response rate was 42.9% in patients with a target lesion. In total, 48.4% of patients experienced a grade ≥ 3 AE, 2.6% experienced grade 5 AEs and 60.9% experienced AEs leading to T-DXd dose adjustments (reduction: 36.9%, interruption: 34.0% or discontinuation: 23.7%). No new safety signals were detected. CONCLUSIONS: T-DXd was effective and had a manageable safety profile as a third- or later-line treatment for patients with HER2-positive gastric or GEJ cancer in Japanese clinical practice. Oncological similarities between large type 3 and type 4 tumors in patients with resectable gastric cancer: a propensity score-matched analysis of a multi-institutional dataset

Background: Large type 3 (diameter ≥ 8 cm) and type 4 gastric cancers have been arbitrarily combined in Japan as a single entity. However, whether these two types are oncologically similar remain unclear. This study aimed to clarify this issue. Methods: In this retrospective study, we analyzed a database of 3,575 patients from nine institutions who underwent gastrectomy between 2010 and 2014. Using propensity scores to balance significant variables, we compared prognoses and tumor recurrences. Results: Of patients with clinical T3/T4 who underwent R0 resection, 75 and 73 had large type 3 and 4 tumors, respectively. Patients with type 4 tumors had significantly lower overall survival rates than those of patients with large type 3 tumors (hazard ratio [HR] 1.77; 95% confidence interval [CI] 1.14–2.74). However, among the large type 3 tumors, a remarkable difference in prognosis was observed between the differentiated and undifferentiated histological types. A comparison was made between large type 3 with undifferentiated phenotype and type 4, each with 39 patients after propensity score matching. Outcomes in both groups were similar in terms of overall survival (HR 1.28; 95% CI 0.73–2.25) and relapse-free survival (HR 1.34; 95% CI 0.80–2.27). No statistically significant differences were observed in the incidence of peritoneal recurrence (35.9% vs. 46.1%, P = 0.36) and lymph node recurrence (25.6% vs. 12.8%, P = 0.15). Conclusions: Large type 3 tumors with undifferentiated phenotype and type 4 tumors were oncologically similar. This subgroup could be considered as a new entity for future clinical trials.

Background: Despite numerous studies of peripancreatic inflammatory fluid collection (PIFC) that report on the relevance of the drain amylase concentration (D-AMY), early prediction using this assay is problematic. This study aimed to investigate the clinical significance of measuring the D-AMY at 3 h after gastrectomy (POD0) for gastric cancer. Methods: This retrospective analysis included consecutive patients who underwent gastrectomy combined with peripancreatic lymph node dissection. The predictive value of D-AMY on POD0 and postoperative day 1 (POD1) for clinically relevant PIFC was evaluated together or individually. Results: Analyses were performed in 204 patients. Twenty (9.8%) patients experienced PIFC. D-AMY cutoffs of 721 IU/L on POD0 and 1695 IU/L on POD1 were determined using the receiver operating characteristic curve analysis for predicting PIFC. The D-AMY on POD0 had higher sensitivity (80%) but lower specificity (66.3%) for prediction of PIFC, compared with those of D-AMY on POD1 (65%, 89.1%, respectively). When combination marker analysis was performed, the highest risk group (D-AMY ≥ the cutoff values of POD0 and POD1) were associated with an elevated rate of occurrence (44%) and a high positive likelihood ratio (7.36) compared with those of the single cutoff group. The lowest risk group (D-AMY < the cutoff values on POD0 and POD1) was associated with a low rate of occurrence (2.5%) and low negative likelihood ratio (0.24) compared with those of the single cutoff group. Conclusions: Combined measurements of D-AMYs on POD0 and POD1 enhanced early prediction of PIFC after gastrectomy.

Background: Few well-controlled studies have compared postoperative complications between Billroth I (B-I) and Roux-en-Y (R-Y). The aim of the present study was to compare the incidence of overall and severe postoperative complications by reconstruction method after distal gastrectomy. Methods: We performed a multi-institutional dataset study of patients who underwent distal gastrectomy with B-I or R-Y reconstruction from 2010 to 2014. Using propensity scores to strictly balance the significant variables, we compared postoperative complications between the techniques. Results: After matching, we enrolled 1014 patients (n = 507 in each group). The incidence of postoperative complications in the R-Y group was significantly higher vs the B-I group (29% vs 17%, P < 0.0001). The incidence of intra-abdominal abscess (4.3% vs 1.8%, P = 0.0177), bowel obstruction (2.6% vs 0.6%, P = 0.0203), and delayed gastric emptying (5.3% vs 1.0%, P < 0.0001) in the R-Y group was significantly higher vs the B-I group, respectively; we saw no significant difference in leakage (3.4% vs 4.1%, P = 0.5084). The incidence of grade ≥ III severe postoperative complications in the R-Y group was significantly higher vs the B-I group (13% vs 7.1%, P = 0.0013). Multivariable analysis showed that R-Y reconstruction was a strong independent risk factor for overall postoperative complications (odds ratio 1.58, P = 0.0044) and grade ≥ III severe postoperative complications (odds ratio 1.75, P = 0.0127). A forest plot revealed that R-Y reconstruction was associated with a greater risk of both overall and grade ≥ III severe postoperative complications in any subgroups. Conclusions: R-Y reconstruction was associated with increasing overall postoperative complications, as well as severe postoperative complications.

The aim of this study was to explore whether a combination of S-1 and paclitaxel offers any benefit over paclitaxel alone to patients pretreated by S-1.
Gastric cancer patients who developed progression during S-1-based first-line chemotherapy or had recurrence during postoperative adjuvant chemotherapy by S-1 were randomly assigned to receive second-line treatment either by weekly administration of paclitaxel at 80 mg/m(2) three times every 4 weeks or daily oral S-1 (80 mg/m(2)) for 2 weeks plus paclitaxel (50 mg/m(2)) given on days 1 and 8, every 3 weeks (S-1 plus paclitaxel). The primary endpoint was progression-free survival (PFS) at 4 months after the initiation of treatment.
A total of 78 patients were eligible for efficacy analyses-40 were assigned to the paclitaxel group and 38 to the S-1 plus paclitaxel group. PFS at 4 months was similar between the groups (50 % for paclitaxel vs 55 % for S-1 plus paclitaxel, P = 0.641). There were no differences between the groups either in progression-free survival (4.6 vs 4.6 months, respectively, P = 0.526), overall survival (10.0 vs 10.0 months, respectively, P = 0.464), or overall response rate (27 vs 22 %, respectively, P = 0.767). The incidences of grade 3 or 4 hematological and non-hematological toxicities were also equivalent between the two groups (25 vs 26 % and 24 vs 26 %, respectively).
No benefit of S-1 administration beyond progression was shown when paclitaxel was selected as the key drug for second-line chemotherapy.

BACKGROUND: Human epidermal growth factor receptor 2 (HER2)-positive advanced gastric cancer (AGC) presents significant therapeutic challenges due to its molecular heterogeneity. Previous studies suggest that immune checkpoint inhibitors (ICIs) may enhance the efficacy of subsequent HER2-targeted therapy. However, evidence suggesting an optimal sequence for nivolumab and trastuzumab deruxtecan (T-DXd) treatment is limited. This exploratory analysis of EN-DEAVOR evaluated the effectiveness and safety of administering T-DXd relative to the timing of prior ICI administration. METHODS: This study assessed real-world outcomes of T-DXd in patients with HER2-positive AGC stratified by prior ICI exposure: within 2 months of nivolumab (Group A), >2 months (Group B), and no prior nivolumab (Group C). The primary effectiveness endpoints included real-world progression-free survival (rwPFS) and objective response rate (ORR). Safety endpoints included grade ≥ 3 adverse events (AEs). RESULTS: Among 311 eligible patients, Group A showed the longest median rwPFS (n = 63; 6.9 months) compared with Group B (n = 63; 4.6 months) and Group C (n = 185; 4.2 months). The risk of progression was significantly lower in Group A compared with Group B (hazard ratio [95% confidence interval]: 0.6 [0.4-0.9]; P = .0074). ORR was numerically highest in Group A (54.9%) versus Group B (30.8%) and Group C (43.4%). More patients in Group B (58.7%) experienced grade ≥ 3 AEs than in Group A (50.8%) and Group C (43.8%). No new safety signals were observed. CONCLUSIONS: Initiating T-DXd within 2 months post-ICI may enhance therapeutic efficacy in HER2-positive AGC without affecting safety, supporting a potential sequencing option after ICI therapy.

OBJECTIVE: This study aimed to clarify the current status of cytological methodologies in high-volume gastric cancer centers and explore the relationship between methodology and positive rates. BACKGROUND: International standards for peritoneal lavage cytological methods for collection, handling, and cytopreparation in gastric cancer have not been established yet. METHODS: A questionnaire survey on cytological methodology was conducted in 61 institutions within the Japan Clinical Oncology Group in 2024. Aggregated data from patients with clinical T3 to T4 gastric cancer with cytology from 2017 to 2022 were collected to calculate positivity rates in each institution for the comparison of methodologies between institutions with high- and low-positivity rates. RESULTS: Thirty-three institutions (64%) collected samples from 2 sites, primarily from the Douglas pouch and left subphrenic area. Fifty-eight institutions (95%) used ≤100 mL of normal saline for injection, and 51 (87%) performed intraoperative rapid cytology. Twenty-five institutions (41%) used additives in samples. Scraping glass slides and centrifugal direct smears were predominant cytopreparation methods in 31 (51%) and 22 (36%) institutions, respectively, and ethanol fixation was employed in 53 (87%). In 61 institutions (11,367 patients), the median cytological positivity rate for clinical T3 to T4 gastric cancer was 8.5% (2.1%-28.3%). Institutions with higher positivity rates more often employed ethanol fixation (97% vs. 77%, P = 0.026) and used ≤50 mL of normal saline for injection (61% vs. 37%, P = 0.074). CONCLUSIONS: Even among Japanese high-volume centers, cytological methodologies for gastric cancer lack uniformity, thereby leading to substantial variability in the proportion of positive cytology.

BACKGROUND: Intraperitoneal chemotherapy is promising for gastric cancer with peritoneal metastasis. Although a phase III study failed to show a statistically significant superiority of intraperitoneal paclitaxel combined with S-1 and intravenous paclitaxel, the sensitivity analysis suggested clinical efficacy. Thus, attempts to combine intraperitoneal paclitaxel with other systemic therapies with higher efficacy have been warranted. We sought to explore the efficacy of intraperitoneal paclitaxel with S-1 and cisplatin. PATIENTS AND METHODS: Gastric cancer patients with peritoneal metastasis were enrolled in the phase II trial. In addition to the established S-1 and cisplatin regimen every 5 weeks, intraperitoneal paclitaxel was administered on days 1, 8, and 22 at a dose of 20 mg/m2. The primary endpoint was overall survival rate at 1 year after treatment initiation. Secondary endpoints were progression-free survival and toxicity. RESULTS: Fifty-three patients were enrolled and fully evaluated for efficacy and toxicity. The 1-year overall survival rate was 73.6% (95% confidence interval 59.5-83.4%), and the primary endpoint was met. The median survival time was 19.4 months (95% confidence interval, 16.1-24.6 months). The 1-year progression-free survival rate was 49.6% (95% confidence interval, 34.6-62.9%). The incidences of grade 3/4 hematological and non-hematological toxicities were 43% and 47%, respectively. The frequent grade 3/4 toxicities included neutropenia (25%), anemia (30%), diarrhea (13%), and anorexia (17%). Intraperitoneal catheter and implanted port-related complications were observed in four patients. There was one treatment-related death. CONCLUSIONS: Intraperitoneal paclitaxel combined with S-1 and cisplatin is well tolerated and active in gastric cancer patients with peritoneal metastasis.

Background: The pretreatment albumin–globulin ratio (AGR) is a frequently used inflammation-associated factor that has been reported to have associations with the survival outcomes of various malignancies. Methods: We retrospectively analyzed 162 patients with pancreatic cancer who underwent preoperative treatment followed by curative surgery at Nagoya University Hospital between April 2010 and December 2020. Representative nutritional status indicators of neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR), prognostic nutritional index (PNI), and albumin–globulin ratio (AGR) were calculated for each case. Results: Among pretreatment blood examination parameters, only AGR (cutoff: 1.33) showed a significant difference in overall survival time (OS) and progression-free survival time (PFS) from the beginning of the preoperative treatment. Median PFS was 22.3 mo, in high AGR cases and 17.1 mo, in low AGR cases (P = 0.019). Median OS was 48.7 mo, in high AGR cases and 32.9 mo, in low AGR cases (P = 0.043). Conclusion: High pretreatment AGR may be a favorable prognostic factor for pancreatic cancer patients who received preoperative multimodal therapy followed by curative cancer resection. It may imply that nutritional status and inflammation control before the multimodal treatment affect the survival outcomes of pancreatic cancer cases and needs to be optimized.

Pancreatic fistula after gastrectomy with lymph node dissection is associated with prolonged hospital stay and critical complications such as intra-abdominal bleeding and sepsis. Polyglycolic acid (PGA) sheets are absorbable suture reinforcement materials. A randomized Phase II trial has been planned to evaluate the effect of PGA sheets on preventing postoperative pancreatic fistula. A total of 320 patients will be recruited from thirteen institutions. Patients who are scheduled to undergo distal or total gastrectomy will be randomly allocated into the PGA group or control group, and the dissected area around the pancreas will be covered by the PGA sheet in the PGA group. The primary endpoint will be the maximum value of drain amylase concentration up to 5 days after surgery. The secondary endpoints will be as follows: transition of value of amylases of drain discharge, incidence of pancreatic fistula, incidence of intra-abdominal abscess, white blood cell count, value of C-reactive protein, incidence of postoperative complication, duration of antibiotic agents administration, duration of abdominal drainage, usage of octreotide, duration of hospital stay, incidence of bleeding in abdominal cavity, mortality, and incidence of reoperation.

Cisplatin should be administered with diuretics and Magnesium supplementation under adequate hydration to avoid renal impairment. Patients should be evaluated for eGFR (estimated glomerular filtration rate) during the treatment with pemetrexed, as kidney injury has been reported. Pemetrexed should be administered with caution in patients with a CCr (creatinine clearance) < 45 mL/min. Mesna is used to prevent hemorrhagic cystitis in patients receiving ifosfamide. Febuxostat is effective in avoiding hyperuricemia induced by TLS (tumor lysis syndrome). Preventative rasburicase is recommended in high-risk cases of TLS. Thrombotic microangiopathy could be triggered by anticancer drugs and there is no evidence of efficacy of plasma exchange therapy. When proteinuria occurs during treatment with anti-angiogenic agents or multi-kinase inhibitors, dose reductions or interruptions based on grading should be considered. Grade 3 proteinuria and renal dysfunction require urgent intervention, including drug interruption or withdrawal, and referral to a nephrologist should be considered. The first-line drugs used for blood pressure elevation due to anti-angiogenic agents are ACE (angiotensin-converting enzyme) inhibitors and ARBs (angiotensin receptor blockers). The protein binding of drugs and their pharmacokinetics are considerably altered in patients with hypoalbuminemia. The clearance of rituximab is increased in patients with proteinuria, and the correlation with urinary IgG suggests similar pharmacokinetic changes when using other antibody drugs. AIN (acute interstitial nephritis) is the most common cause of ICI (immune checkpoint inhibitor)-related kidney injury that is often treated with steroids. The need for renal biopsy in patients with kidney injury that occurs during treatment with ICI remains controversial.

Background: The presence of chronic inflammation and nutritional status in cancer patients affects its prognosis. There is a clinical need for a prognostic predictor that is objective and accurate, and that can be easily evaluated by preoperative screening. We evaluated the importance and usefulness of the preoperative modified systemic inflammation score (mSIS) to predict the long-term outcome of patients undergoing curative resection for gastric cancer (GC). Methods: Of the 3571 patients who underwent curative resection for GC in nine institutions between January 2010 and December 2014, 1764 patients who met the inclusion criteria were included. The mSIS was formulated according to the serum albumin level (ALB) and lymphocyte-to-monocyte ratio (LMR) as follows: mSIS 0 (ALB ≥ 4.0 g/dL and LMR ≥ 3.4), mSIS 1 (ALB < 4.0 g/dL or LMR < 3.4), and mSIS 2 (ALB < 4.0 g/dL and LMR < 3.4). Results: Patients were categorized into preoperative mSIS 0 (n = 955), mSIS 1 (n = 584), and mSIS 2 (n = 225) groups. The overall survival times and the disease-free survival times of patients in preoperative mSIS 0,1 and 2 sequentially shortened (P < 0.0001), and mSIS 1 and 2 were identified as an independent prognostic factor (hazard ratio [HR] 1.35, 95% confidence interval [CI] 1.06–1.272, P = 0.0125 and HR 1.63, 95% CI 1.21–2.19, P = 0.0012). A stepwise increase in the prevalence of hematogenous recurrences was directly proportional to the mSIS. A forest plot revealed that mSIS 0,1 was associated with a greater risk of overall survival in most subgroups. Conclusion: Preoperative mSIS can be easily calculated, and it is suggested that it is useful as a prognostic predictor of patients with different disease stages, for stratifying and evaluating clinical outcomes.

Background: Intra-abdominal desmoid tumors are usually slow growing and solitary, but multifocal desmoid tumors develop on rare occasions. Diagnosing desmoid tumors before histological examination of a surgical biopsy is often difficult. In particular, if a patient has a prior history of malignancy, it may be difficult to differentiate between these lesions and disease recurrence or metastasis.
Case presentation: We present a rare case of multiple rapidly growing intra-abdominal desmoid tumors after surgical trauma, without familial adenomatous polyposis. A 51-year-old male underwent abdominal perineal resection with lateral lymph node dissection after neoadjuvant chemotherapy for lower rectal cancer. Follow- up computed tomography (CT), performed 6 months after primary surgery, showed a 20-mm solitary mass in the pelvic mesentery. Another CT scan, performed 3 months later, revealed that the mass had grown to 35 mm in size and that two new masses had formed. Based on imaging studies and his medical history, it was difficult to distinguish the desmoid tumors from recurrence of rectal cancer. Curative resection was chosen for therapeutic diagnosis. The pathological diagnosis was multiple mesenteric desmoid tumors.
Conclusions: Desmoid tumors should not be excluded as a differential diagnosis for intra-abdominal masses after intra-abdominal surgery, even in cases of rapidly growing multiple masses.

ポリスチレンスルホン酸カルシウム(アーガメイトゼリー)は急性および慢性腎不全に伴う高カリウム血症の治療に用いられる陽イオン交換樹脂の一つである。副作用としては便秘をはじめとする消化器症状がよく知られている。また重大な副作用として腸管穿孔の報告があり,添付文書でも注意勧告がなされている。今回,同剤内服中に大腸穿孔を起こした3症例を経験した。本症例と文献的な検討を行った結果,穿孔部はS状結腸,直腸に集中していること,ほとんどの症例で便秘を認めることが特徴であった。本剤内服中に便秘になり,便塊が停滞し機械的な損傷から大腸穿孔をきたすものと考えられた。本剤を投薬する際には,浸透圧性下剤,刺激性下剤をうまく使いながら適切に排便コントロールを行うことが重要であり,関係者に広く啓発していく必要があると思われる。(著者抄録)

外来化学療法への薬剤師業務が推進されているが、地方中規模一般病院での費用対効果は明らかではない。そこで、中津川市民病院(以下、当院)の外来化学療法室において2018年8月1日〜2019年7月31日の1年間に薬剤師が主治医に薬物療法に関する提案あるいは情報提供(薬学的介入)を行った内容を後向きに調査した。期間中に行った薬剤管理指導は1,067件であり、薬学的介入が294件、受諾率は88.1%であった。そのうち抗がん剤に関する提案は192件、その他の併用薬に関する提案が69件であった。田坂らによる推算を用いると、薬学的介入による医療経済効果は年間11,032,000円と考えられる。一方で、がん患者指導管理料3による収益は684,000円であった。外来化学療法の薬剤管理指導は、現在はその収益で補うことはできていないが、安全性・有効性の確保と医療訴訟の回避により病院を損失から守り、かつ医療経済効果は高いと考える。(著者抄録)

症例は35歳女性。妊娠29週に腹痛,嘔吐を主訴に近医を受診し,イレウスの疑いで当院へ搬送となった。1回の経妊経産歴があり開腹歴はなかった。腹部単純CTで臍左側背側の小腸に閉塞機転を疑う所見を認め,小腸イレウスの診断で入院となった。第3病日になってもイレウス所見の改善がないため,緊急手術となった。まず帝王切開を施行した後に,腹腔内を観察するとS状結腸間膜左葉に2cm大の小孔を認め,Treitz靱帯から290cmの小腸が嵌頓しており,S状結腸間膜内ヘルニアによるイレウスと診断した。用手的に整復することができ,Richter型の嵌頓で腸管壊死は認めなかったため,イレウス解除術で終了し,術後6日目に軽快退院となった。児は生後5ヵ月明らかな障害はなく生存中である。妊娠時イレウスは母児ともに死亡率の高い疾患であり,積極的な検査と手術のタイミングを逸しないことが母児の救命に直結すると考えられる。(著者抄録)

食道癌転移の分子病態解明にもとづいたデュアル修飾型アンチセンス核酸医薬創出研究

食道癌は解剖学的・生物学的特徴から早期に転移をきたす予後不良な疾患である。なかでも難治な食道癌血行性転移の治療成績を改善するには、癌進展の精緻な病態解明に基づく新規分子標的治療薬の開発が望まれる。研究代表者らはTranscriptome解析において血行性遠隔転移を有する食道癌組織で有意な発現増加を認める分子群から標的分子としてAKR1B15とGDPD2を厳選した。本研究ではこれらの食道癌転移分子機構における機能を明らかにするとともに、医薬基盤・健康・栄養研究所創薬デザイン研究センターとの共同研究で革新的な効果を有する食道癌治療アンチセンス核酸医薬、およびその治療奏効度を予測するコンパニオン診断技術の開発を目指す。
食道癌は解剖学的ならびに生物学的特徴から早期に転移をきたす予後不良な疾患である。なかでも難治な食道癌血行性転移の治療成績を改善するには、癌進展のさらに精緻な病態解明に基づく新規分子標的治療薬の開発が望まれる。研究代表者らはTranscriptome解析において血行性遠隔転移を有する食道癌組織で有意な発現増加を認める分子群から標的分子としてAKR1B15とGDPD2を厳選した。本研究ではこれらの食道癌転移分子機構における機能を明らかにするとともに、医薬基盤・健康・栄養研究所創薬デザイン研究センターとの共同研究で革新的な効果を有する食道癌治療アンチセンス核酸医薬、およびその治療奏効度を予測するコンパニオン診断技術の開発を目的としている。
令和6年度の実績を以下に記す。
アクセス効率を最大化するmRNA二次構造予測システムを用いてAKR1B15/GDPD2それぞれに対して候補配列をデザインした。アンチセンス核酸をCEM法によりトランスフェクションし、濃度依存性のノックダウン効率を評価した。一次候補配列は一定のノックダウン活性を示したが、濃度依存性の活性について再現性の高い結果が得られなかったため、対象食道扁平上皮癌細胞株が本実験系に適しているかをpositive controlとしてhMALTAに対するアンチセンス核酸のノックダウン活性を指標に評価し、問題がないことを確認した。ついで、二次候補配列をそれぞれ設計、合成した。これらからノックダウン活性をもとにAKR1B15 4配列、GDPD2 5配列の有望配列を選抜した。これらは全て、再現性をもって濃度依存性のノックダウン活性をを示した。現在、in vitroから機能解析実験へと進めている。
一次候補配列は一定のノックダウン活性を示したが、濃度依存性の活性について再現性の高い結果が得られなかったため、以下の工程を要した。
・対象食道扁平上皮癌細胞株が本実験系に適しているかをpositive controlとしてhMALTAに対するアンチセンス核酸のノックダウン活性を指標に評価し、問題がないことを確認した。
・二次候補配列をそれぞれ設計、合成した。
令和7年度は、以下を順次実施していく。
腫瘍学的機能解析：AKR1B15とGDPD2がどのような分子生物学的機序によって食道癌転移に関与しているのかを明らかにする。ヒト食道扁平上皮癌細胞株を対象にして、AKR1B15/GDPD2ノックダウン前後での細胞増殖能、細胞接着能、浸潤能を評価する。また、Flow cytometryを用いてApoptosis細胞比率の変化および細胞周期の変化も調べる。細胞形態とスフェロイド形成能の変化、癌幹細胞性（ALDH発現細胞比率）の変化、食道癌のKey drugである5-FU、シスプラチン、パクリタキセル感受性の変化を調べる。レスキュー実験として無発現癌細胞株に強制発現を行い、細胞増殖能および浸潤能を評価する。
シグナル解析：AKR1B15とGDPD2の細胞内pathwayへの干渉を知ることは未知な部分の多いAKR1B15/GDPD2の癌細胞での役割を紐解くこと、ひいては薬効メカニズムを明確にすることにつながる。AKR1B15/GDPD2ノックダウン前後の蛋白溶解液を対象に細胞内シグナル伝達経路のリン酸化部位を網羅的に比較解析する。
臨床検体での発現解析：300症例の食道癌切除組織検体と非癌部食道組織を対象にAKR1B15/GDPD2 mRNA発現レベルを定量的PCR法で調べ、予後や転移形式との相関性を解析する。
転移先臓器嗜好性のメカニズム解明：in vitroでは血行性転移の主たる転移先である肝臓と肺への嗜好性を評価するため、肝類洞内皮細胞（HSE細胞）とヒト肺微小血管内皮細胞（HPME細胞）への接着細胞比率のAKR1B15/GDPD2発現調節による変化を調べる。
・in vivoでは腹部食道への同所移植モデル（リンパ節転移を惹起）、尾静注による肺転移モデルでAKR1B15/GDPD2安定的ノックダウンの影響を比較評価する。

本研究では胃癌腹腔洗浄液中のSYT13/CEA mRNA発現解析の臨床応用の可能性を追求するために、以下のことを行う。
●洗浄細胞診陰性例の胃切除時腹腔洗浄液検体中のSYT13/CEA mRNAを定量し腹膜再発予測マーカーとしての精度を検証する。
●腹膜播種に対する化学療法施行症例（腹腔内化学療法、Conversion手術）で、繰り返し得られた腹腔洗浄液検体中のSYT13/CEA mRNA発現量の推移と治療効果との比較解析をおこなう。
SYT13は胃癌腹膜播種に特徴的に高発現する分子であり、腹腔洗浄液中のSYT13 mRNA発現は治療効果のモニタリング指標として有用である可能性が示された。治療奏効例ではSYT13発現量が治療経過とともに低下する傾向が認められたが、非奏効例ではSYT13が低下しない、もしくは上昇する症例を多く認めた。一方、SYT13発現と腹膜播種の量的指標（PCIスコア）との間には有意な相関は認められず、SYT13は播種量ではなく腫瘍の生物学的特性を反映する質的マーカーである可能性が示唆された。SYT13/CEA mRNA発現の経時的解析は、個別化治療やバイオマーカー指向型治療における評価法として期待される。
腹腔洗浄液中mRNA発現解析は、胃癌腹膜播種における治療効果モニタリングや再発予測の精度向上に寄与する可能性がある。具体的には、治療効果を早期に判定できれば、不必要な治療を回避し、患者の負担を軽減するとともに、適切な治療選択を迅速に行うことで、治療成績の向上が期待される。また、SYT13を標的とした分子標的治療が実現すれば、治療選択の幅が広がり、胃癌腹膜播種患者のQOL向上に大きく貢献する可能性がある。本研究は、個別化医療やバイオマーカー指導型治療の新たな評価法を確立し、治療戦略の最適化に貢献する意義を持つ。

これまでの研究で、少ない症例数における解析ではあるが、術後に自立度が低下する群が存在する可能性が示唆されていた。このため本研究では、消化器癌における様々な術式の自立度への影響と複数の診療科を含む多職種横断的な介入の効果を包括的に把握し、高齢者に対する癌治療のエビデンス構築の一助となるデータを構築することを目的としている。
本年度は、術前、退院時、術後6ヶ月に自立度の調査を行い、症例の集積を進めた。胃癌患者に対する周術期高齢者機能評価は、約65例の症例を集積し、解析が可能な症例数を得た。このため次年度は、自立度に影響を及ぼすリスク因子を抽出することで、周術期自立度の低下に関わる、より詳細な情報を得る予定である。さらに、本年度は75歳以上の胃癌を有する高齢者に対して胃切除術を行った患者を対象として、老年科専門医による評価および術後早期の理学療法士によるリハビリテーションの導入を開始している。次年度以降も症例の集積を進める。以上により、介入による有効性を評価することを計画しているが、介入の有効性、あるいは抽出したリスク症例に対する介入の有効性に関しては、現時点で介入効果が不明であるため、研究分担者である統計家と密に相談しながら、評価を行う。
また、自立度が低下した症例群と維持された症例群の2群において、自立度のバイオマーカーの探索を開始している。現在、自立度が維持された症例と低下した両群の症例の術前血清を用いて、網羅的なたんぱく質の同定および比較定量を行っている。この結果を元に、次年度はより詳細な検討を予定している。

胃癌肝転移はきわめて予後不良な転移形式であるが、有効な治療戦略の構築が進んでいない。本研究では、次世代シーケンサーを用いた網羅的な解析を肝転移に的を絞って応用することで、肝転移に特化した鋭敏な診断マーカーおよび分子標的治療薬の開発につながる新規標的分子に関する知見をさらに深めることを目的として進めている。独自の転移形式特異的transcriptome解析から胃癌肝転移関連分子候補として同定したkiller cell lectin like receptor G2 (KLRG2)について、胃癌細胞における機能、肝転移形成能への関与、発現度の臨床的意義を調べている。令和3年度は、主として胃癌細胞株を用いたin vitro解析を実施した。安定的にKLRG2を喪失させた胃癌細胞株では、親株と比較して細胞増殖能、細胞遊走能、細胞接着能が低下していた。さらに、細胞増殖能が低下する要因として、アポトーシス細胞比率が増加していることを発見した。KLRG2の悪性腫瘍における役割には未知の部分が大きく、どのような腫瘍関連分子やsignaling pathwayとの干渉を有するのかを知ることは、その特性を理解するために重要である。PCR array解析を実施し、KLRG2発現に強く相関する癌関連遺伝子としてDSC2およびMMP9を同定した。300例の胃癌手術検体を対象とし、癌部および非癌部組織中のKLRG2 mRNA発現度を定量的PCR法で測定した。臨床病理学的因子との相関解析により、癌部KLRG2発現レベルが進行度のみならず累積血行性転移発生頻度に有意に相関することが示された。
