    Background: To provide quality patient care, nurses are required to actively learn and improve their nursing competence. Aim: This study examined the impact of nurses’ learning factors related to in-service education on nursing competence, targeting nurses with up to five years of clinical experience working in hospitals. Methods: Questionnaires were mailed to qualified nurses working in two areas of Japan. Data from 177 responses with no missing values for the dependent and independent variables were analysed. The survey items included the Clinical Nursing Competence Self-Assessment Scale (CNCSS), Learning Scale on In-service Education for Nurses, demographic factors such as age and career self-reliance, and institutional environmental factors such as number of beds in the hospital where they work. Findings: Among the three concepts in the CNCSS, “fundamental in nursing practice” had the highest average score, and “personal and professional development” had the lowest average score. Of the nurses’ learning factors, “acquisition of basic nursing” and “self-growth as a person” had the highest and lowest mean scores, respectively. Nurses’ total learning scores were significantly associated with nursing competence — the higher the learning score, the higher the CNCSS score. Moreover, the scores for “acquisition of autonomy as a nursing professional” and “self-growth as a person” in nurses’ in-service education learning showed a significant positive relationship with nursing competence. Conclusion: Hospitals need to devise educational programs and train educators to improve the autonomy of nurses and encourage their growth as a person.

    Aims: This study aimed to investigate the relationship between family caregivers' health-related quality of life and health outcomes in care recipients. Methods: A total of 160 female caregivers in Japan were initially assessed using self-reported baseline questionnaires to determine physical and mental aspects of their health-related quality of life. Based on these scores, they were divided into three groups: lower, middle, and higher quality of life. We followed up with the 133 participants (after excluding those that did not respond) 6 years later to assess the health of their care recipients; 36 caregivers continued to provide home care, while 97 reported that their care recipient had died or been admitted into institutionalized care. Results: Statistically, a higher risk for care recipients' health deterioration was strongly associated with the mental component score in the lower caregivers' quality of life group of than for the middle group, after adjusting for the care recipients' age, health status, and caregivers' age at baseline. There was no significant association between deterioration of health of care recipients and physical component scores of the caregivers. Conclusion: Caregivers' poor health-related quality of life, particularly poor mental health, may be linked to the deterioration of care recipients' health.

    This cross-sectional study aims to clarify the relationship between the number of behavioral and psychological symptoms in long-term care recipients and family caregivers’ depressive states. Participants were 38 family caregivers who had provided care for their relatives for 6 years or more; they were recruited from in-home care settings in Aichi, Gifu, and Shiga Prefectures, Japan. Participants answered a self-administered questionnaire assessing their depressive state using the Japanese version of the Center for Epidemiologic Studies for Depression Scale (CES-D). They also answered questions inquiring about behavioral and psychological symptoms of care recipients, including resistance to care, irritability, and feelings of persecution. Using CES-D scores, 11 participants were categorized as depressed and 27 as non-depressed. Depressive symptoms were significantly greater in those with more behavioral and psychological symptoms, following adjustment for confounding factors. The odds ratio of being in a depressive state was 9.43 higher for those with more than 4 behavioral and psychological symptoms compared to those with none, showing a distinct threshold for the influence of behavioral and psychological symptoms on depressive state. Knowing the number of behavioral and psychological symptoms of care recipients may help quickly identify depressed caregivers and alleviate depressive symptoms.

    <p>The purpose of this study was to determine the extent of learning through in-hospital education of nurses as well as the influences of the organizational environment and individual factors on this learning. The study participants were 1,185 nurses working at hospitals in two adjacent prefectures. To recruit the participants, 2,777 questionnaires were delivered to nurses and a total of 1,243 nurses responded. A self-administered questionnaire survey was sent in the mail. In addition to the basic attributes of participants, the survey items included organizational environmental factors such as the number of beds in the facility where they were currently working, the presence or absence of mentors or models, and the presence or absence of leadership role experience. Additional survey items were related to career self-reliance and learning through in-hospital education among nurses. A hierarchical multiple regression analysis, where learning through in-hospital education among nurses was set as the dependent variable, was used. Of the five subfactors of "learning scale on in-hospital education for nurses," "acquisition of basic nursing" exhibited the highest average value, and "self-growth as a person" exhibited the lowest average value. Results of the hierarchical multiple regression analysis revealed that the factors related to learning through in-hospital education among nurses included nurse age, clinical experience, number of facility beds, presence of mentors or models, leadership role experience and career self-reliance. These results suggest that an organizational environment that is conducive to learning and a strong willingness to learn may promote the extent that nurses learn through in-hospital education.</p>

    <p><b>Purpose:</b> This study examined the emotions associated with the development of dementia among the middle-aged or older adults living in community.</p><p><b>Methods:</b> A qualitative descriptive research method was adopted in this study. Data were collected through semi-structured interviews from 13 middle-aged or older adults who belonged to several clubs.</p><p><b>Results:</b> The participants were six men and seven women aged 60–91 years. Twelve participants (92.3%) had contact with people with dementia. Consequently, 40 codes, 24 sub-categories, and 13 categories were formed, and the following six core categories representing the emotions associated with developing dementia among the middle-aged or older adults living in community were identified: (a) fear, (b) apprehension, (c) threat, (d) denial, (e) despair, and (f) relief.</p><p><b>Conclusions:</b> The middle-aged or older adults worry about dementia due to unfavorable knowledge. Thus, there is a need to promote sufficient knowledge about dementia among the middle-aged or older adults and provide good care via professionals for people with dementia.</p>

    <p>The purpose of this study is to clarify the factor structure of learning that nurses working at a hospital acquire from clinical education in order to develop and test the reliability and validity of the scale of leaning from clinical education of nurses. A questionnaire comprising 53 items was prepared through examination by nurse practitioners. The subjects of a questionnaire survey were nurses who work at hospitals in Prefecture A. Exploratory factor analysis was conducted with the principal factor method and promax rotation, and reliability and validity were confirmed with the Cronbach's α coefficient and confirmatory factor analysis, respectively. The number of responses was 720 (valid response rate=43.6%), of whom 88.6% were female and 11.4% were male. The mean working year and standard deviation was 16.3±9.1 years. Five factors for 20 items comprising the scale were extracted: [acquisition of basic nursing], [expanded view toward work], [self-growth as a person], [acquisition of competency to accomplish work at one's own department], and [acquisition of autonomy as a nurse professional]. Cronbach's alpha coefficient for reliability was 0.960, which cinfirmed internal consistency. The degree of compatibility of the model with confirmatory factor analysis was GFI=0.911, AGFI=0.883, CFI=0.956, and RMSEA=0.066. It was suggested that this scale is reliable and validity, and can be used to evaluate learning acquired by nurses from clinical education.</p>

    <p><b>Objective:</b> In our previous study, we developed a scale of nurses' learning from clinical education. This study aimed to test the cross-validation of the Learning Scale on In-service Education for Nurses.</p><p><b>Methods:</b> A self-report questionnaire was administered to nurses working at hospitals. There were 435 participants (response rate: 38.6%). Surveys from 82 participants were excluded from the analysis because they did not answer all the questions. Finally, 353 questionnaires were used for the analysis. We examined the Learning Scale on In-service Education for Nurses, the Scale of Career Self-reliance Behavior, and social support in the workplace. Confirmatory factor analysis was performed to test the fitness of the model obtained from our previous study.</p><p><b>Results:</b> The Learning Scale on In-service Education for Nurses demonstrated acceptable construct validity and internal reliability of its subscales. A path analysis showed that social support from a superior at work (path coefficient=0.23, p=0.01) directly predicted learning from in-service education. Career self-reliance behavior was explained by learning from the in-service education (path coefficient = 0.41, p < 0.01).</p><p><b>Conclusion:</b> Using a different sample from our previous study, this study suggested that the scale is reliable and valid and can be used to evaluate learning acquired by nurses from clinical education. Social support from a superior at work may promote learning from in-service education. Moreover, learning may facilitate self-reliance behavior for career development.</p>

    本研究は、国外でも検討が始まっている認知症発症に対する懸念や恐怖といった感情 “Dementia Worry” （以下、DWとする）をもつ中高年者を支援するシステムを構築することを目的とする。具体的には、中高年者の認知症発症についての感情とその背景を詳細なインタビューを通じて明らかにする。次に、明らかになった感情（DW）の有訴率とそれに関連する要因について、郵送法による質問紙調査から明らかにする。そして、その解析結果をもとに、科学的根拠に基づいた認知症発症の脅威をもつ中高年者を支援するシステムを構築する。
    本研究は、認知症発症に対する懸念や恐怖といった感情 “Dementia Worry” （以下、DWとする）をもつ中高年者を支援するシステムを構築することを目的とする。具体的には、中高年者の認知症発症についての感情とその背景をインタビューを通じて明らかにするとともに、明らかになった感情（DW）の有訴率とそれに関連する要因について、質問紙調査から明らかにすることを予定している。
    本年度は、中高年者の認知症発症についての感情（懸念、不安、恐怖など）、なぜそのように感じるのか、感情による健康や行動への影響などを明らかにするための面接調査の準備を行った。面接調査は40歳以上の中高年者を対象とし、研究内容を理解し、文書による同意を得た者を研究協力者とする。面接調査の内容は、「あなたは認知症発症についてどのような感情を持っていますか。あなたは、なぜ、そのように感じるのですか。」、「あなたは、そのような認知症発症についての感情のために、今の健康状態（心配となる症状）や行動に影響がありますか。」等とし、対象に事前に同意を得て録音およびメモを取ることを予定している。現在までに、所属機関の倫理審査を受け、研究実施の承認を得ている。また、調査の指針となる「面接調査実施マニュアル（インタビューガイドを含む）」の作成等の準備を行った。しかし、COVID-19の感染拡大予防のため、対象者への調査は実施していない。
    新型コロナウィルス感染症の感染拡大予防のため、地域で暮らす中高年者を対象とした面接調査は実施できない。
    新型コロナウィルス感染症の状況が落ち着き次第、面接調査を実施する予定である。面接調査実施後は、分析を行い、中高年者の認知症発症についての感情などを明らかにする。また、面接調査の結果に基づき第2段階として予定している質問紙調査の準備を行う。

    The purpose of this study is to clarify the factor structure of learning that nurses working at a hospital acquire from clinical education in order to develop the scale of leaning from clinical education of nurses. Also, we elucidate the relationship between what nurses working in hospitals have learned so far through clinical education and clinical nursing competence.
    The scale were extracted: [acquisition of basic nursing], [expanded view toward work], [self-growth as a person], [acquisition of competency to accomplish work at one’s own department], and [acquisition of autonomy as a nurse professional].
    The results showed that all 13 clinical nursing competencies and 5 factors of learning were significantly associated within the coefficient range of 0.13-0.33. The results of the multiple regression analysis showed that factors of learning associated with all 13 clinical nursing competences were “acquisition of autonomy as a nurse professional” and “self-growth as a person.”
