    Background: With an aging global population, establishing integrated systems for long-term care is challenging in several countries. Adequate and quality service for older adults in nursing homes can improve their quality of life. The career self-reliance of nurses working in nursing homes may affect the quality of life of older adults; this suggests a need for educational support for career self-reliance behavior. Objective: Our study aimed to identify characteristics associated with the career self-reliance behaviors of nurses working in nursing homes. Methods: Data were collected from nurses working in 789 nursing homes in Japan using questionnaires in 2018. The questionnaire consisted of the Scale of Career Self-Reliance Attitude, Career Self-Reliance Behavior to measure the degree of nurses’ career self-reliance, questions about personal attributes, work environment, and work commitment; and the Clinical Nursing Competence Self-Assessment Scale. A multiple regression analysis was conducted using career self-reliance as the dependent variable and characteristics as the independent variables. Results: Consequently, a total of 336 questionnaires were collected, of which 289 without missing data were analyzed. Of the 289 participants, 79 (27.3%) had certifications or licenses other than those for nursing or had completed graduate school. As a result of multiple regression analysis, a higher frequency of nursing practice (β = 0.38, p <0.001) was associated with career self-reliance attitudes. Having certifications or licenses other than those for nursing or completing graduate school (β = 0.11, p = 0.03), job satisfaction as a nurse (β = 0.12, p = 0.03), higher frequency of nursing practice (β = 0.28, p <0.001), and a higher degree of career self-reliance attitude (β = 0.45, p <0.001) were associated with career self-reliance behavior. Relationships with supervisors or colleagues were not associated with career self-reliance. Conclusion: It is important for nurses working in nursing homes to enrich their nursing practice, continue learning, and be satisfied with their jobs to improve their career self-reliance. Nurses and administrators working in nursing homes should make efforts to obtain these characteristics for nurses’ career self-reliance behaviors. Particularly, maintaining nursing practice frequency is essential for improving the quality of life of older adults through nurses’ career self-reliance.

    Background: To provide quality patient care, nurses are required to actively learn and improve their nursing competence. Aim: This study examined the impact of nurses’ learning factors related to in-service education on nursing competence, targeting nurses with up to five years of clinical experience working in hospitals. Methods: Questionnaires were mailed to qualified nurses working in two areas of Japan. Data from 177 responses with no missing values for the dependent and independent variables were analysed. The survey items included the Clinical Nursing Competence Self-Assessment Scale (CNCSS), Learning Scale on In-service Education for Nurses, demographic factors such as age and career self-reliance, and institutional environmental factors such as number of beds in the hospital where they work. Findings: Among the three concepts in the CNCSS, “fundamental in nursing practice” had the highest average score, and “personal and professional development” had the lowest average score. Of the nurses’ learning factors, “acquisition of basic nursing” and “self-growth as a person” had the highest and lowest mean scores, respectively. Nurses’ total learning scores were significantly associated with nursing competence — the higher the learning score, the higher the CNCSS score. Moreover, the scores for “acquisition of autonomy as a nursing professional” and “self-growth as a person” in nurses’ in-service education learning showed a significant positive relationship with nursing competence. Conclusion: Hospitals need to devise educational programs and train educators to improve the autonomy of nurses and encourage their growth as a person.

    Aims: This study aimed to investigate the relationship between family caregivers' health-related quality of life and health outcomes in care recipients. Methods: A total of 160 female caregivers in Japan were initially assessed using self-reported baseline questionnaires to determine physical and mental aspects of their health-related quality of life. Based on these scores, they were divided into three groups: lower, middle, and higher quality of life. We followed up with the 133 participants (after excluding those that did not respond) 6 years later to assess the health of their care recipients; 36 caregivers continued to provide home care, while 97 reported that their care recipient had died or been admitted into institutionalized care. Results: Statistically, a higher risk for care recipients' health deterioration was strongly associated with the mental component score in the lower caregivers' quality of life group of than for the middle group, after adjusting for the care recipients' age, health status, and caregivers' age at baseline. There was no significant association between deterioration of health of care recipients and physical component scores of the caregivers. Conclusion: Caregivers' poor health-related quality of life, particularly poor mental health, may be linked to the deterioration of care recipients' health.

    本研究は、国外でも検討が始まっている認知症発症に対する懸念や恐怖といった感情 “Dementia Worry” (以下、DWとする)をもつ中高年者を支援するシステムを構築することを目的とする。具体的には、中高年者の認知症発症についての感情とその背景を詳細なインタビューを通じて明らかにする。次に、明らかになった感情(DW)の有訴率とそれに関連する要因について、郵送法による質問紙調査から明らかにする。そして、その解析結果をもとに、科学的根拠に基づいた認知症発症の脅威をもつ中高年者を支援するシステムを構築する。
    本研究は、認知症発症に対する懸念や恐怖といった感情 “Dementia Worry” (以下、DWとする)をもつ中高年者を支援するシステムを構築することを目的とする。具体的には、中高年者の認知症発症についての感情とその背景をインタビューを通じて明らかにするとともに、明らかになった感情(DW)の有訴率とそれに関連する要因について、質問紙調査から明らかにすることを予定している。
    本年度は、中高年者の認知症発症についての感情(懸念、不安、恐怖など)、なぜそのように感じるのか、感情による健康や行動への影響などを明らかにするための面接調査の準備を行った。面接調査は40歳以上の中高年者を対象とし、研究内容を理解し、文書による同意を得た者を研究協力者とする。面接調査の内容は、「あなたは認知症発症についてどのような感情を持っていますか。あなたは、なぜ、そのように感じるのですか。」、「あなたは、そのような認知症発症についての感情のために、今の健康状態(心配となる症状)や行動に影響がありますか。」等とし、対象に事前に同意を得て録音およびメモを取ることを予定している。現在までに、所属機関の倫理審査を受け、研究実施の承認を得ている。また、調査の指針となる「面接調査実施マニュアル(インタビューガイドを含む)」の作成等の準備を行った。しかし、COVID-19の感染拡大予防のため、対象者への調査は実施していない。
    新型コロナウィルス感染症の感染拡大予防のため、地域で暮らす中高年者を対象とした面接調査は実施できない。
    新型コロナウィルス感染症の状況が落ち着き次第、面接調査を実施する予定である。面接調査実施後は、分析を行い、中高年者の認知症発症についての感情などを明らかにする。また、面接調査の結果に基づき第2段階として予定している質問紙調査の準備を行う。

    The purpose of this study is to clarify the factor structure of learning that nurses working at a hospital acquire from clinical education in order to develop the scale of leaning from clinical education of nurses. Also, we elucidate the relationship between what nurses working in hospitals have learned so far through clinical education and clinical nursing competence.
    The scale were extracted: [acquisition of basic nursing], [expanded view toward work], [self-growth as a person], [acquisition of competency to accomplish work at one’s own department], and [acquisition of autonomy as a nurse professional].
    The results showed that all 13 clinical nursing competencies and 5 factors of learning were significantly associated within the coefficient range of 0.13-0.33. The results of the multiple regression analysis showed that factors of learning associated with all 13 clinical nursing competences were “acquisition of autonomy as a nurse professional” and “self-growth as a person.”
