    これまで、当研究グループでは、体性幹細胞の1つである「骨髄由来間葉系幹細胞」の免疫抑制機能・抗炎症機能を期待し、その細胞シートを大動脈瘤モデルマウス(ApoE KOマウスにアンギオテンシン(Ang)II持注)の腹腔内に留置することで、大動脈瘤の進展を抑制し、各種炎症性サイトカインやマトリクスメタロプロテアーゼ(MMP)などの蛋白分解分子の抑制および、細胞外マトリクスの産生促進に寄与する分子の増加を示し、その有効性を示唆してきた。本研究では、骨髄由来間葉系幹細胞を局所留置ではなく、全身的に静脈内投与することで、同様に大動脈瘤の進展を抑制することができるかを検証した。
    高齢(24週齢)のアポリポプロテインE欠損(apoE-/-)マウスに(Ang)IIを4週間持続注射し、10^6個の骨髄由来間葉系幹細胞を尾静脈から静脈注入し(MSC群,n=14)、2週および8週後に犠牲死させ評価を行った。コントロール群として生理食塩水を静脈注射した(C群,n=11)。大動脈瘤径は、2週後の横隔膜下レベルで有意にMSC群がC群より小さかったが、他の部位はMSC群が小さい傾向があるものの有意差はなかった。エラスチカ・ファン・ギーソン染色では、C群のエラスチックラミナの崩壊が激しい印象であったが、エラスチン量定量では両群間で有意差はなかった。ザイモグラフィーによるマトリクスメタロプロテアーゼ(MMP)の発現は、MSC群でpro MMP-9が有意に減少したが、MMP-2(pro, activeともに)やactive MMP-9では有意差がなかった。
    上述のように、MSC投与群の方が良好な傾向はみせるものの十分な効果が得られなかった。この結果から、投与量(細胞数)の最適化や複数回投与の必要性が示唆された。細胞が大動脈瘤に到達しているかの検証や細胞のどのような作用が影響を及ぼすのか、メカニズムの解明も今後必要である
