    There are approximately 2.4 million international residents in Japan in 2016. Seventy percent of these residents do not speak English. However, medical and nursing schools emphasize education in Medical English, rather than taking a broader approach that encompasses diverse cultures. This is why current healthcare professionals struggle in dealing with international patients, especially those from non-English speaking countries. Therefore, shifting the viewpoint from how we can technically support international patients to how we can improve the intercultural competence of healthcare professionals is critical. the aim of this literature review is to examine the similarities and differences between the intercultural competence in pedagogy and healthcare in Japan and the US by comparing literatures from both fields, and by clarifying how intercultural competence has been conceptualized as well as its significance. Results show that when comparing the concept of intercultural competence in pedagogy and healthcare, while the components are similar, the historical background of the concept and the social contexts in use are different (between the two disciplines). In order to understand the concept of intercultural competence, it is critically important to define Japan’s own interdisciplinary concept of intercultural competence in both healthcare and pedagogy.

    The purpose of this study was to reveal the problems recognized by Indonesian nurses after returning to work in Indonesian health and medical facilities after work experience in nursing or nursing care work in Japan. The Root Cause Analysis (RCA) with the theme of “Issues of my own workplace” was conducted on three people including two Indonesian nurses and one Japanese nurse as a facilitator. The issue tree as the result of RCA was an analysis subject. As a result, a series of eight events emerged in the issue tree. Four root causes were extracted. Problem consciousness emerged when subjects became culturally aware of the difference in standards between their home country and Japan which was then identified as cultural awareness.

    Competence in culturally sensitive care in healthcare has been included into the Japanese medical school curriculum since 2001 and will be integrated into the Japanese nursing school curriculum in 2018. Since the concept of competence in culturally sensitive care is new to the Japanese healthcare education, we need to gain knowledge about the concept from other forward-thinking countries. However, when introducing the concept of competence in culturally sensitive care from foreign countries to Japan, the knowledge and experiences of those countries should be considered.

    多文化共生社会となりつつある我が国の現状を鑑みると、多様な文化的価値観を持つ人々を理解し、その多様性を尊重したケアができる保健医療人材の育成は喫緊の課題であり、その人々の保健医療ニーズを明らかにし、文化的・社会的価値に働きかけることが、健康格差の根本的解決に向けて求められている。本研究の目的は、多様な文化的価値観を持つ人々の保健医療ニーズを明らかにし、その多様性を尊重したケアができる保健医療人材の教育プログラムを開発することで、健康格差の解消を目指すことである。
    2019年度(2年目)は、昨年度実施した第一段階(2018-2019)の日本国内における多様な文化的価値観を持つ在日外国人の保健医療ニーズを明らかにするために実施した2つの調査の結果を分析した。さらにフィンランドJANK大学への訪問により、フィンランドの保健医療人材育成やその多文化共生社会の在り方について専門家とディカッションを深められた。昨年度訪問した、米国マイアミ大学看護学部での米国におけるヒスパニック系移民を始めとした多様な背景を持つ人々のケアへの実践例や保健医療人材の教育の知見と併せて、情報の整理を行い学会での発表を通して他の研究者とのディスカッションを進めてきた。また、第二段階の看護職への調査の準備を開始した。
    昨年度の調査結果は、国際学会での発表後、そのディスカッションを基に、分析に修正を加えながら、複数の論文の執筆を進めている。

    第一段階(2年間計画)の目標である「在日外国人の保健医療ニーズ調査の実施と評価」と「看護職への調査の準備」は、部分的に達成している。前者の昨年度からの実施の遅れもあり、その結果の分析、評価の進捗がやや遅れており、現在も執筆中の論文がある。また、後者は、昨年度末から蔓延し始めたCOVID-19のパンデミックの影響もあり、対象者である看護職への負担を考慮し、プレテストを見合わせている状況である。
    2020年度は、引き続き、第1段階の調査結果をまとめ論文化する作業を継続し、投稿や学会発表等で報告をする予定である。また、COVID-19のパンデミックが落ち着きしだい医療現場が平常となるタイミングで看護職を対象とした調査のプレテスト及び、調査を実施する予定である。看護職は、看護師・保健師・助産師を対象とし、それぞれの視点からの多様な背景を持つ人々をケアする上で必要な知識・経験について明らかにする予定である。進捗の遅れを補うため、研究協力者を増員し実施することとする。
    さらに、国外の実践事例として、オーストラリア、インドネシア等へ訪問を予定している。そこでは、各国の多様な背景をもつ人々へのケア実践例や保健医療職への教育事例や研究成果を調査し、地域性に合わせた教育プログラムについて専門家と意見交換を予定している。それを踏まえ、次段階の保健医療人材の多文化適応能力向上プログラム開発への参考とする予定である。
