    本研究では、以下に示す3つの研究計画を実施する。癌性腹水TIMEモデルにより同定した遺伝子を改変したCAR-Tを作成し、ヒトおよびマウスにおいて疲弊耐性の評価を行っていく(課題①)。癌性腹水誘導性の免疫疲弊を改善できる漢方補剤のCAR-Tの機能改善に寄与する因子を同定する(課題②)。癌性腹水中に存在するリンパ球の活性/疲弊プロファイリング結果を臨床情報と統合することにより、免疫疲弊を定量的に評価できる指標を確立する(課題③)。難治性腹膜播種のTIMEの病態解明、CAR-T療法の機能改善につながる遺伝子改変や天然化合物を組み合わせることで複合的免疫治療へとつながる成果を得ることが可能となる。

    腹膜播種を伴う進行卵巣がんや再発例は治療抵抗性を獲得し難治性となる。がん免疫療法は、一部のがん種では治療成績を劇的に改善したが、腹膜播種に対する効果は限定的であり、免疫抑制的腫瘍微小環境に起因することが指摘されている。本研究において、大気圧プラズマを液体化したプラズマ活性溶液がCAR-T細胞療法の活性や持続性を向上させ、がん免疫療法の課題解決のための新規治療戦略となることを目指す。
    腹膜播種を伴った進行あるいは再発卵巣がんは難治性であり、治癒や長期生存に持ち込むことは依然として困難であることが多い。腹膜播種の治療抵抗性は腫瘍細胞と宿主細胞が形成する複雑な腫瘍免疫微小環境がその一端を担っている。そのため、一部の血液悪性腫瘍には劇的な治療効果のあるキメラ抗原受容体(chimeric antigen receptor:CAR)T細胞療法(CAR-T療法)でさえ腹膜播種に対する効果は非常に限定的である。その原因として、腫瘍内においてCAR-Tの疲弊化・増殖低下・持続性低下等が引き起こされるためと考えられ、本研究では、大気圧プラズマにより活性化したプラズマ活性溶液を用いて、腫瘍微小環境の免疫抑制状態を解除し、腫瘍内の免疫環境を改変することができるかどうかを検討し、がん免疫療法の課題解決の突破口となる新技術の開発を目的とする。
    昨年度は卵巣がん患者から採取した腹水、及び免疫抑制誘導因子の一つとして考えられているTGF-b1により活性化ヒト末梢血単核球(PBMC)の免疫機能低下を誘導する免疫抑制in vitro 評価系を構築した。今年度は、レンチウイルスベクターを用いた卵巣がんにおいて高発現しているMesothelin(MSLN)をターゲットにしたCAR-T(MSLN-CAR-T)の製造法を確立し、導入効率50~60%を達成した。また、MSLN発現がん細胞に対し、MSLN-CAR-Tが機能的であることを確認し、がん性腹水によるMSLN-CAR-Tの機能低下をin vitroにおいて明らかにした。さらに、プラズマ活性溶液はMSLN-CAR-Tに対しても免疫機能を活性化させる可能性が確認された。即ち、本効果はCAR-T療法の機能改善につながる可能性が示唆された。
    今年度は昨年度より準備を進めてきたレンチウイルスベクターを用いたMSLN-CAR-Tの安定的な製造方法の確立に成功した。Mesothelin高発現卵巣がん細胞株を接着系、浮遊系の両方で樹立し、MSLN-CAR-Tが機能的に働くことを確認した。さらに、MSLN-CAR-Tに対しても卵巣がん患者由来がん性腹水による免疫抑制誘導系が確立できた。一方で、MSLN-CAR-Tの調整はPBMCに比べてかなりの時間を要するため、実験回数が限られ、プラズマ活性溶液の条件検討に対しては十分な実験回数が確保できなかった。
    来年度以降において実施予定である、腫瘍免疫学的評価のための免疫正常な動物モデルの構築を行った。予備検討において当研究室で利用可能なマウス卵巣がん細胞株であるOVA-ID-8は、腹膜播種を呈するまでに4~5か月程度を要することが明らかとなったため、高播種株の樹立を行うこととなった。既報を参考にしたところ、Krasを高発現させることで、がん細胞播種後1か月程度で腹膜播種を呈する卵巣がんモデルを作成することに成功した。さらに、CAR-T細胞として用いるOT-1トランスジェニックマウスよりOvalbumin(OVA)-TCR-T細胞の調製法を確立し、免疫正常な動物モデルの構築が完了した。
    これまでの研究成果において、プラズマ活性溶液は免疫抑制が誘導された活性化PBMC並びにMSLN-CAR-Tに対して免疫抑制の解除或いは免疫を活性化する効果を有することが明らかとなった。そこで免疫正常な動物モデルを用い腫瘍免疫学的評価を行う。具体的にはCAR-T細胞の代わりにOT-1トランスジェニックマウスよりOvalbumin(OVA)-TCR-T細胞を調製し、OVA発現マウス卵巣がん細胞(OVA-ID-8-Kras細胞)を用いた腹膜播種モデルにおいて、OVA-TCR-T細胞の抗腫瘍効果評価系に対するプラズマ活性溶液の機能を評価する。また動物モデルを用いた実験に先立ち、OVA-ID-8-Kras細胞とOVA-TCR-Tを用いてin vitro 評価系でのプラズマ活性溶液の最適化を行う。この最適条件はMSLN-CAR-Tの免疫機能へも効果を示すかどうか確認する。最終的に、プラズマ活性溶液処理有り無しによるマウス生存への影響、並びに腹腔内腫瘍微小環境における免疫学的差異について詳細に組織学的解析を中心に実施する。

    Utilizing a lentiviral vector that incorporates a CD3ζ-CD28 co-stimulatory domain, we consistently achieved a high gene transduction efficiency of approximately 50% into both peripheral blood mononuclear cells (PBMCs) and isolated CD3-positive T-cells. We performed assays to measure cytotoxic activity against cancer cells, effector cytokine production such as IFN-γ, and antigen-specific proliferation using CFSE analysis. Through these systems, we made the critical observation that cancerous ascites, often present in patients, significantly suppress T-cell function. To overcome this immunosuppression, we modified a specific target (Gene X) within the CAR-T cells. This genetic enhancement successfully restored their anti-tumor functions. The superior efficacy of these modified CAR-T cells was validated in vivo, where they demonstrated a significant tumor-reducing effect in a murine subcutaneous tumor model, highlighting their enhanced therapeutic potential for clinical application.

Various cytotoxic antitumor drugs and molecular targets have been used as maintenance therapy, but unfortunately, none can completely eradicate the "dormant cancer cell clusters (seeds)." This study aims to elucidate, through interdisciplinary research, the mechanisms by which cancer-associated mesothelial cells (CAM) assist in the dormancy and evolution of tumor cells and help them evade attacks from existing antitumor drugs. By exploring the maintenance mechanisms of intra-abdominal occult seeding by CAM, we attempted to develop a comprehensive treatment strategy targeting the entire peritoneum and identify novel biomarkers.

    In outcomes of this study, we investigated impact on the malignancy of ovarian cancer cells due to adipose-derived mesenchymal stem cells (ADSC) sourced from the omentum, through our unique ceiling culture method. We successfully demonstrated that proliferation of ovarian cancer cells, the enhanced motility of cancer cells, and the increased ability to form tumors in vivo, were all induced. We reported these series of results in a research paper. Moreover, we have previously identified that ovarian cancer-associated peritoneal mesothelial cells (OCAM) deeply contribute to progression of advanced ovarian cancer in the peritoneal microenvironment. We demonstrated that calcitriol (vitamin D) is a promising candidate substance to inhibit the process of normal peritoneal mesothelial cells transforming into OCAM, and reported these findings in a research paper as well.

    1It has been demonstrated that immune cells whose character was changed at the peritoneal metastasis microenvironment could contribute to cancer progression in gynecologic malignancy. In this study, we investigated the activation of anti-tumor immune response in tumor microenvironment treated with plasma-activated solution, liquified gaseous of an atmospheric pressure plasma, having an effect of anti-tumor and cell activation. The results showed 1) increased M1-type macrophage, having an anti-tumor activity, infiltration in the peritoneal metastasis, 2) cancellation of the immunosuppression of T cells induced by patient-derived cancerous ascites, and 3) activation of T cell immunosuppressed by TGF-β1.

    We evaluated the significance of DDIT4 in gynecologic malignancies, which is upregulated upon p53 activation. Suppression of DDIT4 expression significantly reduced cell growth, cisplatin resistance, migration and invasive capacity. Immunostaining of clinical specimens revealed a significant correlation between DDIT4 expression and clinicopathological factors correlated with malignant prognosis, such as lymphatic invasion and lymph node metastasis. We identified a correlation with the NFκB pathway involved in malignant characteristics. Intraperitoneal administration of DDIT4-suppressed cell lines suppressed metastatic potential in nude mice. Using LA-ICP-MS, we found that tissue platinum distribution is a useful biomarker to identify platinum resistance in ovarian hyper-atypical serous carcinoma with p53 mutation.

    The aim of this study was to elucidate new disease mechanisms by focusing on cell-cell interactions in the ovarian cancer tumor-microenvironment (TME), and to discover novel therapeutic targets for ovarian cancer patients. Throughout the three-year research period, we have elucidated the malignant mechanisms of refractory ovarian cancer from extremely multiple aspects. The research included metabolomic analysis, small RNA sequencing, therapeutic effects of vitamins, and the relationship with adipose tissue. All of the results are the whole new findings, which have been reported for the first time. The function of mesothelial cells in TME is still under investigation. Although this research grant is in its final year, we will continue to elucidate the functions of various cells in the ovarian cancer TME and leading to develop them for clinical applications such as diagnosis and treatment.

    To evaluate the effects of exogenous mutant p53 protein on endogenous miRNAs expression, two different mutant p53 protein, R248W and R248Q, were expressed in ovarian cancer cell lines haboring p53-null. We identified eight miRNAs that showed 1.5-fold or more increase in expression and two miRNAs that had two-third or more expression reduction in two forced expression strains of mutant p53.
    As a part of elucidation of the mechanism of peritoneal metastasis, we conducted comprehensive analysis of cancer-related miRNAs using FFPE between patients with mature teratomas and malignant transformations, and identified multiple miRNAs. We also elucidated and reported a novel mechanism by which PAI-1, which is abundantly present in cancerous ascites fluid, promotes changes in peritoneal mesothelial cells to promote the tumor-promoting environment.
