<p>The purpose of this study was to identify any challenges involved in the nursing practice being carried out by ward nurses in small and medium-sized hospitals in their discharge support work, in addition to clarifying the details of the work. </p><p>Semi-structured interviews were conducted with 13 nurses working at small or medium-sized hospitals. Thereafter the data was qualitatively analyzed using an inductive approach. </p><p>The content of the practice was: Working to improve motivation by leveraging the strengths of patients; Consideration for maintaining a good relationship between patients/family and the medical staff; Involvement that emphasizes the safety, maintenance, and feasibility of medical treatment behavior after discharge; Encourage patients/family to face reality and life after discharge; Finding compromises with patients/family regarding the life after discharge with functional deterioration; Coordinating points of agreement with patients/family while giving consideration to the circumstances of the hospital organization; Exercise leadership across multiple occupations by utilizing the expertise of nurses; and Team management to maintain and strengthen the support system for interdisciplinary approach. </p><p>The nurses provided support of patient/family decision making and team management for inter-professionals. To promote discharge support by nurses working autonomously, it is important that a nursing manager evaluate and approve the ward nurses’ discharge support practice.</p>

Background: Breast cancer-related lymphedema (BCRL) is a chronic swelling of the arm due to breast cancer treatment. Lymphedema is diagnosed and staged on the basis of limb circumference measurements and the patient's subjective symptoms, which have poor reproducibility and objectivity: these cannot detect any fluid accumulation in the tissue. Ultrasonography is a feasible noninvasive technique that can be used to evaluate tissue structure in real time. This study aimed to assess the ability of texture features for discriminating the presence of accumulated fluid within the subcutaneous tissue of BCRL using ultrasound (US) imaging. Methods and Results: This study included 20 women who were treated for unilateral breast cancer and who subsequently developed BCRL (International Society of Lymphology stage II). Subcutaneous tissue was scanned through an US system (Sonosite Edge II; Sonosite, Inc., FUJIFILM) using a 6- to 15-MHz linear transducer to assess the ability of texture features for discriminating the presence of accumulated fluid within the subcutaneous tissue of BCRL. Fluid accumulation was observed using a 3-Tesla MR system under double-echo steady-state conditions. There was a significant difference among the three groups (with hyperintense area, without hyperintense area, and unaffected side) in 11 of 14 textural features (p < 0.05). Post hoc analysis (Mann-Whitney U test; Bonferroni correction p < 0.0167) revealed significant differences in seven textural features within the hyperintense area. Conclusions: This study revealed that seven texture features quantified by US imaging data can provide information regarding fluid accumulation in the subcutaneous tissue of lymphedema.

Background:
Manual lymph drainage (MLD) is one of the common treatments for breast cancer-related lymphedema (BCRL). Although the primary goal of MLD is to drain the excessive fluid accumulated in the affected upper limb and trunk to an area of the body that drains usually, the use of MLD is decided based on swelling and subjective symptoms, without assessing whether there is fluid accumulated in the affected region. The purpose of this study was to examine truncal fluid distribution in a sample of BCRL patients and investigate any correlation between such fluid distribution and swelling or subjective symptoms.
Methods and Results:
An observational study was conducted with 13 women who had unilateral, upper extremity BCRL. Fluid distribution was evaluated by using two magnetic resonance imaging (MRI) sequences: half-Fourier acquisition single-shot turbo spin echo and three-dimensional double-echo steady-state. The presence of swelling was determined by lymphedema therapists, and subjective symptoms were measured by using a visual analog scale. On MRI, no participants had any free water signals in the trunk. However, seven had swelling and all 13 had some kind of subjective symptoms on the affected side of their trunk.
Conclusions:
These results suggest that swelling and subjective symptoms do not correlate with the presence of truncal fluid. For such cases, a different approach than MLD may be needed to address truncal swelling and related subjective symptoms. Checking for the presence of fluid in the truncal region may help MLD be used more appropriately.

This study determined the level of implementation and the importance placed on various nursing activities at clinics in rural areas of Japan in order to explore the achievement and perceived importance of implementing certain nursing roles and activities at such clinics. To identify these items, a questionnaire was administered to 40 nurses working in rural clinics. The results showed that activities related to "Basic Nursing Practice" and "Community Understanding" were recognized as important and were performed by almost all nurses. Some activities related to "Administration and Operation" and "Cooperation with Local Government" were recognized as important, but were not implemented, thereby hampering the continuum of care across the health system. These activities, which are related to collaboration with hospitals and local governments that support the clinics, included adjustment of staff inside and outside the facilities to guarantee the use of paid holidays, as well as collaboration with acute care, remote medical systems, and local governments during emergencies and for disaster preparation. Additional support for nurses in collaboration between clinics in rural areas, hospitals, and regional administrations that support the clinics remains a challenge to be addressed.

<p>The purpose of the present study was to evaluate the distribution patterns of fluid in the upper limbs of patients with breast-cancer-related lymphedema by using magnetic resonance images (MRI).</p><p>Seven women (mean age ± SD: 55.9 ± 13.6 years) with unilateral breast cancer who subsequently developed lymphedema were recruited for this study. The status of lymphedema was evaluated using four MRI sequences, i.e., T1-weighted images, T2-weighted images without fat suppression, T2-weighted images with fat suppression (STIR), and conversion into fluid-attenuated inversion recovery (FLAIR) images. For the quantitative analysis of fluid, areas with hyper-intense signals on the STIR images were calculated for every transverse image slice.</p><p>On STIR images, hyper-intense signals indicating the presence of free water were observed in the forearms, particularly in the ulnar half, of all patients. FLAIR images showed no hyperintense signals in this region. In the brachial region, five patients had such signals in the posterior arm(Regio brachii posterior). In the axillary region, no patients displayed hyper-intense signals. In the hands and fingers, the hyper-intense signals on STIR images were detected over the dorsal surface. Quantitative analysis indicated that a large amount of water accumulated in the ulnar half of the forearm.</p><p>On the basis of these facts, we suggest that the ulnar half of the forearm should be an important target in lymph drainage therapy.</p>

A collateral pathway established after lymphadenectomy could play an important role in long-term lymphedema treatment. The present study investigated alterations of lymph dynamics 1 year after lymphadenectomy using indocyanine green fluorescent lymphography to determine if a collateral pathway may be used for long-term lymphedema treatment. Wistar rats were anaesthetized and lymph nodes were excised at the inguinal and popliteal fossae. The treated hind limb was evaluated by fluorescent lymphography between 10 weeks and 6 months and between 6 months and 1 year postoperatively. Fluorescent lymphography demonstrated a lymphatic pathway to the ipsilateral axillary fossa in all rats 1 year after lymphadenectomy. Some capillary branches in the paths leading to the ipsilateral axillary fossa were dilated and tortuous. In addition, areas in which a fluorescent signal was not visible were increased in the thigh. In conclusion, the collateral pathway did not appear to be only for temporary use to compensate for drainage from the edematous limb but appears more stable as a component of a compensating lymphatic system. These new dilated vessels, although functional at this point, may still be susceptible to disturbance by further alteration to the lymph vessels.

Purpose: Clinical perceptorship is an effective method in the development of professional nurses. However, being a new nurse preceptor is challenging: limitation in their skills and knowledge as a mentor, busyness and pressure. This study explored the process by which experienced nurses grow as nurse educators through their experience being perceptors, to provide effective support to the preceptors.
Methods: Semi-structured interviews were conducted with 12 nurses from University A Hospital with 4 to 8 years of experience and analyzed using the modified grounded theory approach.
Result: In the process of growing up as a nurse educator, nurses challenge to the preceptor role, but struggle to build relationships with preceptee and recognize their
limitation in their skills as a mentor at first. From the introspection that it is necessary to
overcome the lack of competence, the preceptors tried to understand new graduate nurses, search for teaching methods, practiced teaching methods that match with the
characteristics of each preceptee. Then, the preceptors felt their own growth as a preceptor and sense of accomplishment which made them to realize the significance of human resource development. In addition, the atmosphere and the guidance policy of the workplace influenced the process of overcoming their negative experiences such as lack of competence.
Conclusion: The growing process of preceptor as nurse educator were realizing lack of
their ability as mentors, introspection of negative experiences, understanding the characteristics of new nurses, practicing effective guidance for each preceptee, feeling
sense of accomplishment, then realize the significance of human resource development. The support of their working place was effective in their growth.
