    SpAでは、腱靱帯付着部で炎症に続いて生じる骨新生が、関節可動域を制限し身体機能を大きく損なうことがある。抗サイトカイン療法を含む既存の治療は付着部骨新生を抑制する効果に乏しく、新たな治療法の開発が待たれる。
    細胞内エネルギー代謝は、細胞の分化や増殖を調整する重要な因子であり様々な刺激により変動する。本研究の目的は、SpAモデルマウスを用いて、付着部骨新生における細胞内エネルギー代謝の機能を解明することである。炎症により付着部に誘導される幹細胞を対象とした実験により、内因性幹細胞の分化をコントロールし付着部骨新生を抑制する方法の開発を目指す。

    腱は修復能に乏しく損傷すると元の状態まで回復することは困難であり, 加齢とともに軟骨化生や異所性骨化などの変性を生じやすくなる. 過去の研究結果をもとに, 本研究では 「ヒアルロン酸は腱損傷部に誘導される間葉系幹細胞においてヒアルロン酸レセプターCD44を介してMAPKのリン酸化を抑制し, 腱損傷後の軟骨化生を抑制するとともに腱の修復を促進する」 という仮説を立てた. 損傷腱に対するヒアルロン酸の変性予防効果と細胞内情報伝達機構を検討し以下の結果を得た.
    ① CD-1マウスのアキレス腱損傷部から単離した損傷腱由来間葉系幹細胞をmicro mass cultureにより軟骨細胞へ分化誘導し, ヒアルロン酸で処理した. アルシアンブルー染色のintegrated densityはヒアルロン酸の濃度に依存して減少した. また, 軟骨分化マーカーであるaggrecan, Sox9, II型コラーゲンの遺伝子発現量はヒアルロン酸の濃度に依存して減少した. 一方で腱分化マーカーであるMohawk, I型コラーゲンの遺伝子発現量はヒアルロン酸の濃度に依存して増加した. これらのヒアルロン酸の作用は抗CD44抗体により減弱せず, 抗CD44抗体単独でも軟骨分化マーカーの遺伝子発現量は減少した.
    ② ヒアルロン酸合成阻害剤4-メチルウンベリフェロン (4-MU) を用いて同様の実験を行った. その結果, aggrecanの遺伝子発現量はヒアルロン酸の濃度に依存して増加したが, Mohawkの遺伝子発現量に有意な変化は認められなかった. この結果は, 損傷腱由来間葉系幹細胞におけるヒアルロン酸の軟骨分化抑制作用を裏付けるものである.
    ③ In vivo実験として, アキレス腱損傷モデルマウスにヒアルロン酸の局所投与および4-MUの腹腔内投与を行っている.
    本研究では, アキレス腱損傷モデルマウスおよびそのマウスから単離した初代培養細胞を用いて実験を行っている.
    理由① 新型コロナウイルス感染拡大の影響により実験動物舎が一時使用不能となったため.
    理由② in vivoにおいて, ヒアルロン酸の粘稠度が高くマウスのアキレス腱損傷部にヒアルロン酸を局所投与する手技の確立に時間を要したため.
    2020年度までの研究により, ヒアルロン酸が損傷腱由来間葉系幹細胞の軟骨分化を抑制することが示された. さらに, CD44自体が損傷腱由来間葉系幹細胞の軟骨分化に重要な役割を果たしている可能性が示唆された. 今後は以下の実験を計画している.
    ① 現在in vivo実験で薬剤を投与しているモデルマウスの組織学的及び放射線学的評価
    ② MAPK経路を含めた損傷腱由来間葉系幹細胞におけるヒアルロン酸の細胞内情報伝達機構の検討
