    放射性標識CuATSMは固形がんに集積し、PET診断や標的RI治療への利用が期待され、臨床研究において、CuATSM集積と予後に相関が報告されているが、CuATSMの集積が予後不良につながる理由は十分解明されていない。本研究では、これまでにCuATSMの細胞集積と治療抵抗性獲得を結ぶ可能性が示された細胞質NAD+/NADHや遺伝子発現調節因子CtBPの他に、より広範に働く機序として、NADの調節を受けるタンパク質やNADの生合成・分解に係る酵素の関与について、培養細胞や動物を用いてCuATSMの集積が予後不良につながる機序の解明を進め、CuATSM高集積がんの効果的な治療法の提案につなげる。
    放射性のCuで標識されたCuATSMは、低酸素や代謝の変化による細胞内の過還元状態を反映して固形がんに集積すると考えられ、PET診断や標的アイソトープ治療への利用が期待されている。一方で、CuATSMの集積は、従来の一般的な「低酸素腫瘍」の概念が当てはまらない特徴を示している。
    これまで申請者らはCuATSMの集積が細胞質の遊離NAD+/NADH比を反映することを明らかにしてきたが、細胞質の遊離NAD+/NADH比は、細胞のグルコース、アミノ酸、核酸などの代謝に関与している。これらを踏まえ、本研究では、がん細胞の代謝変化やNAD+依存タンパク質の関与に焦点を当て「CuATSMの集積とがんの難治性を結ぶ、広範ながんに共通する機序は何か」との問いに対する答えを追求するとともに、CuATSMの集積が高いがんに対するNAD+依存タンパク質や代謝をターゲットとする薬剤の効果を明らかにしたいと考えている。
    2022年度は、前年度に引き続き、主要なNAD+の生合成酵素であるNAMPTの阻害剤である FK866を細胞の培養液に加えて、細胞全体のNAD+およびNADHの濃度、およびNAD+/NADH比への影響と、[64Cu]CuATSMの細胞集積への影響の比較を検討した。FK866の濃度、作用時間などの変化により、NAD+およびNADHの濃度は複雑な変化を示し、また、細胞の種類によってもFK866に対する反応のパターンは異なっており、[64Cu]CuATSMの取込みとNAD+およびNADHの濃度、あるいはNAD+とNADHの比との明瞭な関係は示すことはできなかった。
    一方、NADの影響を受けて活性が変化するとされる細胞周期の調整やDNA修復と、腫瘍内の[64Cu]CuATSMの集積の高低には相関があることを示唆する結果を得ることができた。
    共同実験先の福井大学高エネルギー医学研究センターにおいて、[64Cu]CuATSMをもちいる臨床検査(腫瘍診断)の実施頻度がさがっており、これに伴って[64Cu]を使用する実験のスケジュール調整が難しくなっている。
    また、使用しているNAMPT阻害剤の効果が、細胞種や添加時間によって予想外に大きく変化したため、安定した実験条件の設定に時間を要している。
    2023年度では、FK866に加えて、細胞内のNAD+およびNADHの濃度に変化をもたらすと報告されているNADの前駆体となるkynurenineやNADを消費するPARPの阻害剤による細胞内NAD+およびNADHの濃度調節を試み、細胞全体のNAD+およびNADHの濃度、およびNAD+/NADH比と[64Cu]CuATSMの細胞取り込みとの関係を明らかにするとともに、[64Cu]CuATSMの集積により強く影響すると考えられる細胞質でのNAD+/NADH比との関係についても検討する。
    また、NADによって活性に影響が表れ、DNA修復に関わるとされているPARP-1の活性の変化と腫瘍内の[64Cu]CuATSMの集積の関係を調べ、[64Cu]CuATSMの集積が指示する腫瘍の難治性にDNA修復の亢進がかかわっている可能性についても検討する。

    High Cu-64-ATSM accumulation in tumor was shown to predict poor prognosis. Cancer stem cell-like cells were reported to be more abundant in the area of high Cu-ATSM accumulation. Increased cellular accumulation of Cu-ATSM has also been reported to associate with increased NADH/NAD+ ratio, which was shown to increase the C-terminal binding protein (CtBP) activity involved in the acquisition of cancer cell stemness. In this study, we focused on CtBP, and pursued the possible mechanism of treatment resistance which can relate to the accumulation of Cu-ATSM.
    In MCF7 cells conditioned to 5% O2, both CtBP and CD133, a stem cell marker, expression and Cu-ATSM accumulation significantly increased, which suggest that Cu-ATSM may capture the changes in cancer cell characteristics mediated by CtBP dependent on high cellular NADH/NAD+ ratio. High levels of intratumoral Cu-ATSM accumulation was observed where CtBP expression tends to be high, which supported the in vitro results.

    An analytical method of radiocarbon based on cavity ring-down spectroscopy was developed for metabolites analysis. Suppression of baseline fluctuations by modulating the optical path length and separation/concentration of sample gas (carbon dioxide) by adopting adsorption columns were developed to improve the sensitivity of C-14 quantitative analysis. Using those techniques, C-14 analysis in samples fractionated by high-performance liquid chromatography was demonstrated. In addition, a La-GPS scintillator-based imaging detector was developed for in vitro metabolite distribution measurements. Imaging of bremsstrahlung caused by β-particles emitted from C-14 as well as tritium was demonstrated by using the detector. In vivo imaging of a mouse to detect the X-rays from a 14C solution orally administered was successfully demonstrated.

    Cu-ATSM labeled with positron emitting radioisotopes is supposed to accumulate into hypoxic tissues and expected to play a critical role in detecting hypoxic cancer and help to predict prognosis and select therapy strategy. The relation between cancer hypoxia and Cu-ATSM accumulation is, however, still ambiguous, hindering the interpretation of Cu-ATSM accumulation and its utilization. In this study, we established cell lines stably expressing newly developed Peredox protein which can report cytoplasmic NADH/NAD+ ratio as green/red fluorescence ratio, and demonstrated that cellular accumulation of Cu-ATSM depends on the cytoplasmic NADH/NAD+ ratio which reflects the redox status of cancer cell cytoplasm and the presence of hypoxia.
