    Community ambulation comprises a continuum of tasks- standing up, transferring, and stepping- during which the interaction between a patient and therapist can be viewed as a form of human-human interaction (HHI). This interaction also involves external factors such as handrails, orthoses, and selective cognitive functions.
    In this study, we characterized HHI differences between (i) an assisted approach, which prioritizes patient safety, and (ii) an independence- guided approach, which aims to elicit the patient’s residual functional abilities. Trunk acceleration of both therapist and patient served as the principal quantitative index. We found a significant association between patients’ subjective insight and the phase difference/coherence of trunk acceleration between the therapist and the patient. These findings provide new insight into goal-oriented, evidence based physiotherapy and rehabilitation for complex, continuous movements.

    Gait initiation is a process in which gravity is efficiently converted into propulsive force through complex, coordinated control. During this transition from standing to steady walking, the regulation of the center of gravity and foot pressure is crucial for maintaining posture and generating forward propulsion. Falls are more likely to occur when weight transfer; known as anticipatory postural adjustment (APA); is insufficient at the initiation of gait, making it a critical factor in stability and safety.
    In this study, we analyzed trunk acceleration using information science-based methods to investigate the fine control mechanisms that occur dynamically and briefly during the non-steady phase of gait initiation. The results clearly revealed subtle imbalances in acceleration control at the moment of the first step, particularly in the forward and lateral directions. These findings characterize the specific deficits in balance control at gait.
