Problem consideredThis bibliometric analysis aimed to identify publication trends, international collaboration, influential articles, keyword bursts, and thematic structures of research on sleep quality among workers.MethodsArticles published from 2005 to 2024 were retrieved from Scopus and the Web of Science. Analyses included descriptive statistics, international network analysis, keyword burst detection, and thematic mapping.ResultsA total of 1428 articles were analyzed. Publications increased steadily, peaking at 233 in 2022, then declined slightly to 218 in 2024. China (16.3%) and the United States (12.7%) were the most productive countries, while European countries showed higher proportions of internationally coauthored papers. Single-country publications accounted for 78.4% and multicountry publications for 21.6%. Keyword bursts indicated an increased focus on mental health, stress, and occupational issues during the COVID-19 pandemic. Thematic mapping identified two clusters: (1) basic themes comprising COVID-19, mental health, stress, and anxiety; and (2) niche themes covering sleep, shift work, and occupational health.ConclusionsResearch on workers’ sleep quality has increased significantly, reflecting its growing recognition as an essential component of occupational health. To advance this field, future efforts should integrate fragmented research areas and strengthen international collaboration to build a more comprehensive, globally comparable body of evidence.

Objective: The positive deviance approach is a community-based strategy that promotes behavioral and social change to improve health outcomes and demonstrates effectiveness even in resource-limited settings. Although the number of scientific studies on positive deviance has increased, comprehensive quantitative assessments of its overall trends remain lacking. This study conducted a bibliometric analysis to analyze global trends and key themes in positive deviance research within public health over the past 50 years (1976–2024). Methods: We used data from Scopus and Web of Science, examining publications including the term “positive devian*” alongside “public health” or “community health.” The analysis covered articles and reviews published from 1976 to 2024. Additionally, we conducted descriptive analysis, network analysis, and thematic mapping. Results: We identified 319 publications in 219 journals in the studied period. Contributions were made from 45 countries, including 25 low- or middle-income countries (55.6 %). Notably, 106 papers (33.2 %) and 30 countries (66.7 %) involved international collaboration, with the United States leading in both productivity and collaboration. Thematic mapping indicated that high-income countries prioritized motor themes (well-developed, relevant thematic areas) such as “obesity” and “resilience” and emerging themes like “quality improvement” and “primary care.” Conversely, lower-income countries focused on motor themes like “stunting” and “wasting” and emerging themes like “qualitative” research and “malaria.” Conclusions: There is a rising global interest in positive deviance, as evidenced by increasing publication volume, participation from lower-income countries, and strengthened international collaborations. Focusing on underexplored themes could help shape the direction of future positive deviance research in public health.

Background: Ensuring an adequate density of public health nurses (PHNs) is vital for effectively addressing community health needs. This study aimed to (1) map the geographic distribution of PHN density across all 47 prefectures in Japan from 2019 to 2024, (2) investigate regional factors associated with PHN density at the prefectural level, and (3) analyze these associations at different quantile levels of PHN density. Methods: An ecological study was conducted using available public data from 47 prefectures in Japan, focusing on variables related to PHN density such as school density, educational capacity, percentage of graduates selecting PHN careers, percentage of graduates remaining within the prefecture, recruitment rate, and turnover rate. Choropleth maps were created to visually represent geographic distributions. Ordinary least squares (OLS) regression and quantile regression (QR) models were used to analyze the identified regional factors associated with PHN density. Results: The mean prefectural-level PHN density was 378.0 per million population, with a 3.6-fold difference (range, 172.7–618.4). The results of OLS regression indicated positive associations between PHN density and educational capacity (regression coefficient B = 0.71, standardized regression coefficient β = 0.40, p = 0.016), the percentage of graduates selecting their first career as a PHN (B = 3.29, β = 0.34, p = 0.013), and those selecting their first PHN career within the prefecture (B = 2.15, β = 0.24, p = 0.032). The QR analysis showed that these associations varied across different quantiles. Education capacity was significant in lower (30th percentile) and higher quantiles (90th percentile). The proportion of graduates who started their career as PHNs and those who remained in the prefecture were significant across several quantiles. Conclusions: Fostering pathways that encourage graduates to pursue public health nursing as their first career choice and remain in their prefectures is essential to enhance prefectural-level PHN density. A balanced approach that addresses both the quantity and quality of PHNs is necessary to advance public health nursing in Japan.
