    反復性肩関節脱臼に対する空気を用いた関節造影CTの診断能につき調査・検討した.対象は2007年1月~2016年8月に反復性肩関節脱臼に対して空気を用いた関節造影CTおよび手術を施行した86例で,透視下に両肩の造影を行ったのちCT検査を施行し,手術時に全例関節鏡にて関節内病変を確認した.86例中,造影CTでBankart病変を容易に診断できたものが67例,容易ではなかったものが12例,診断できなかったものが 7 例で,HAGL病変や関節包断裂は8例を疑い,実際に病変があったものは5例であった. CT検査を施行し,手術時に全例関節鏡にて診断できなかった7例のうち,4例はPerthes病変,1例はALPSA病変,2例で明らかな前方関節唇病変はみられなかった.HAGL病変の診断は感度100%であったが,関節包断裂は感度60%であり,Perthes病変や関節包断裂の診断能をあげる工夫が必要であると考えた.

    <b>Objective</b>: In the pharmacological therapy of osteoarthritis of the knee, various medicines have been prescribed to control knee pain. The purpose of this study was to evaluate the efficacy of a tramadol hydrochloride-acetaminophen (TRAM) combination in patients with osteoarthritis of the knee.<br><b>Methods</b>: We prescribed TRAM to 46 patients with osteoarthritis of the knee who were unable to achieve adequate pain relief using non-steroidal anti-inflammatory drugs (NSAIDs). The patients included 15 men and 31 women, with a mean age of 79.7 years. According to the change in their knee pain intensity after the administration of TRAM, patients classified their pain as one of the following: 'disappeared,' 'decreased,' 'mildly decreased,' 'unchanged,' or 'increased.'.<br><b>Results</b>: Knee pain 'disappeared' in two patients, 'decreased' in 13, 'mildly decreased' in 12, was 'unchanged' in 18, and 'increased' in one patient. Nine patients discontinued taking TRAM due to side effects, all of which subsequently resolved.<br><b>Conclusion</b>: Because tramadol acts as a μ-opioid receptor agonist, it is believed that the TRAM combination directly decreases the pain by inhibiting its transmission. In this study, approximately 60% of all patients who could not achieve sufficient pain relief using NSAIDs could do so using TRAM. However, some patients had to discontinue the medication because of side effects. We think it is necessary to devise a suitable analgesia regimen for patients with osteoarthritis of the knee using TRAM.

    上腕骨2 part外科頚骨折に対する骨接合術における成績不良の原因について検討を行ったので報告する.対象は髄内釘を用いて手術を行った17例で,年齢は平均73歳,観察期間は平均22.8ヵ月であった.調査項目は骨癒合の有無,肩関節可動域,合併症,上腕骨頚体角,JOA scoreで,最終観察時の上腕骨頚体角と可動域の相関も調査した.骨癒合は全例に認めた.合併症は内反変形治癒を1例,上腕骨頭壊死を1例に認めた.上腕骨頚体角は平均133&deg;,JOA scoreは平均82.3点であった.上腕骨頚体角と屈曲可動域に有意な相関を認めた(p=0.008).内反変形に伴う上腕骨頚体角の低下は大結節の上方化につながり,屈曲可動域が不良となったと考えた.また患者側の要因として術前に認知症や上肢麻痺がある症例の成績は不良であった.

    鏡視下バンカート修復術後のアンカー孔の変化をCT画像にて調査した.<BR>対象は26例で,使用アンカーはPanalok-Loop <SUP>&reg;</SUP>(PL)12例49個,Bioraptor <SUP>&reg;</SUP>(BR)7例42個,JuggerKnot &trade;(JK)6例18個であった.術後1か月以内と,術後6か月以上で撮影した3D-CT画像にてアンカー孔の大きさを計測し,その変化を縮小,不変,軽度拡大,拡大(1.5倍以上)に分類し評価した.<BR>術後6~10か月ではBR群でアンカー孔の縮小や不変が多く,PL群では拡大・軽度拡大が多かったが,JK群には一定の傾向はみられなかった.またPL群では術後平均12か月になると拡大が減り縮小が増える傾向があった.肩甲骨関節窩の時計面3~5時では拡大・軽度拡大が多く,1症例あたりの使用アンカー総数が少ない症例で拡大・軽度拡大が多い傾向が見られた.今後さらに症例を増やし,詳細な検討をする必要がある.

    <b>Objective:</b> The treatment strategy for osteochondritis dissecans of the knee depends on various factors such as patient activity, open or closed growth plates, radiographically determined arthroscopic stage, and location, size, and stability of osteochondral lesions. In our hospital, we adopt conservative therapy for patients with stable osteochondral lesions and surgical therapy for patients with unstable lesions or lesions that do not heal after conservative therapy. The surgical procedure is selected depending on the following factors: open or closed growth plates, arthroscopic stage, location, size, and amount of bony osteochondral lesions. <br><b>Methods:</b> We examined the clinical outcome in 39 patients (43 knees) with osteochondritis dissecans of the knee treated at our hospital from 2003 to 2012. Thirteen patients (17 knees) were treated using conservative therapy and 26 (26 knees) using surgical therapy, respectively. Of the 26 surgical patients, 5 were treated using the arthroscopic microfracture method, 14 using internal fixation with bioabsorbable pins or bone strips from the tibia, 1 using mosaicplasty, and 6 using autologous chondrocyte implantation with platelet-rich plasma gel. The clinical outcomes in all patients were evaluated according to the Hughston criteria and Lysholm score. <br><b>Result:</b> The clinical outcomes were satisfactory in all patients (good in 20 patients and excellent in 18 patients) according to the Hughston criteria, and the average Lysholm score was high (97.2 points). <br><b>Conclusion:</b> The selection of the surgical procedure in patients with osteochondritis dissecans of the knee, which was based on the presence of open or closed growth plates and the stability of osteochondral lesions, had excellent clinical results.

    <B>Background:</B> In our hospital, the air-contrast computed tomography arthrography (CTA) is one of the preoperative examinations for patients with recurrent dislocation of the shoulder. Although the potency of CTA has improved by multidetector CT, there is some difficulty in detecting the antero-inferior glenoid labral lesions clearly. In this study, we examined features of the patients with antero-inferior glenoid labral lesions which were difficult to detect by the air-contrast CTA.<BR><B>Methods:</B> We performed air-contrast CTA in 29 patients who had no osseous fragment of glenoid and underwent arthroscopic Bankart repair between 2007.1 and 2012.5. The frequency of dislocation/subluxation, duration from the primary dislocation to arthrography, the bone-deficit size of the glenoid and the rotational position of the humeral head in CT image were evaluated in these patients.<BR><B>Results:</B> Antero-inferior glenoid labral lesions were undetectable in three patients by the air-contrast CTA and two of three were Perthes lesions. In these patients, the mean frequency of dislocation was lower than that of all other patients, and the mean duration from primary dislocation was 12.7 years. Mean bone-deficit size of glenoid was 2.9% that is smaller than that of all other patients (9.5%). The rotation of humeral head tended to be more external compared with the overall mean and we speculated that the subscapularis muscle holds the articular capsule down by external rotation.<BR><B>Conclusion:</B> Features of the patients with undetectable antero-inferior glenoid labral lesions by the air-contrast CTA were a lower frequency of dislocation, smaller bone-deficit of glenoid and more external rotation of the humeral head.

    <B>Background:</B> Subscapularis (SSC) lesions are often underdiagnosed in clinical routine. The purpose of this study is to examine the diagnostic values of clinical tests for SSC lesions.<BR><B>Methods:</B> 26 consecutive patients, including 18 men and 14 women with a mean age of 59.6 years old who underwent arthroscopic procedures for rotator cuff lesions, were clinically evaluated using the belly-press test (BPT), the bear-hug test (BHT) and the lift-off test (LOT) preoperatively. Diagnostic arthroscopy was the reference that determined the actual pathologic lesions. For the three tests, the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of each test were calculated. Diagnostic accuracy was determined using positive likelihood ratios (LR).<BR><B>Results:</B> SSC tears occurred with a prevalence of 61.5%. The BPT was found to be the most sensitive (60.0%) of all (BHT; 55.6%, LFT; 33.3%). The specificity was 76.3% for the BPT, 74.5% for the BHT and 75.0% for the LOT respectively. All of those showed a positive likelihood ratio less than 5 (BPT; 3.05, BHT; 2.21, LOT; 1.21), suggesting that they were inaccurate independently of each other. Combinations of three tests increased positive LR and PPV (all tests were positive; positive LR and PPV were 6.86 and 88.4%, both BPT and BHT were positive; positive LR and PPV were 5.32 and 85.1%), suggesting they were considered to be diagnostically useful.<BR><B>Conclusion:</B> Three clinical tests for SSC lesion were singularly inaccurate. However, the combination of those tests can lead to improved diagnostic accuracy for SSC lesions.

    <B>Background:</B> The purpose of this study is to evaluate clinical results of arthroscopic Bankart repair (ASBR) with a minimum of one-year follow-up and to perform statistical examination to find some factors that affected postoperative outcomes.<BR><B>Methods:</B> We retrospectively investigated 47 patients (49 shoulders), who underwent ASBR using bioabsorbable suture anchors, for traumatic anterior shoulder instability with a minimum of one-year follow-up. 1 case who had recurrence after ASBR was excluded. The pre and postoperative Japanese Shoulder Society shoulder instability score (JSS-SIS) and Rowe score, and the range of motion of external rotation in external rotation at side (ERS) and external rotation at abduction (ERA) were mainly evaluated statistically.<BR><B>Results:</B> The ANOVA analysis showed that the higher the age at ASBR, or the longer the period between primary dislocation and ASBR, significantly lowered the postoperative ERA. Multiple linear regression analysis revealed that an age older than 30 years old at ASBR significantly lowered the postoperative ERA and Rowe score, and the longer the period during primary dislocation and ASBR significantly lowered the postoperative ERS and JSS-SIS.<BR><B>Conclusion:</B> This study suggested that the ASBR performed earlier would show better outcomes. Further investigation will be required.

    Medical students in the second to fifth years evaluated all classroom lectures and laboratories in 1998. To clarify the faculty's opinions on student evaluations, in 2000 a questionnaire survey was given to 420 members of the teaching staff, of whom 137 (33%) responded. One hundred three (81.1%) instructors immediately checked the questionnaires after the classroom lecture or laboratory session, of whom 100 reported that the questionnaires were helpful for preparing subsequent lectures. One hundred nine faculty members replied that student evaluations of faculty teaching were meaningful, and 109 supported similar surveys in the future. Although conducting surveys of many students requires considerable time and cost, an effective feedback system plays an important role for both faculty and students.

    <B>Background:</B> To investigate the effect of the glenoid bone-deficit for shoulder stability, we examined the correlation between the load-and-shift (LAS) test and the glenoid bone-deficit in patients with recurrent dislocation of shoulder (RDS).<BR><B>Methods:</B> 24 patients who underwent surgery for RDS with glenoid bone-deficit between 2007 and 2010 were studied. Under general anesthesia, the LAS test at 45&deg;, 90&deg; and 120&deg; shoulder abduction (LAS45, LAS90, LAS120) was performed by an examiner and each LAS-point (0-3) according to Hawkins' classification(0-3)was recorded. Using CT-image, we calculated the glenoid width ratio and divided the morphology of bone-deficit into "avulsion type (A-group)" and "compression type (C-group)".<BR><B>Results:</B> Mean LAS-point was high in the order of LAS90(2.58), LAS45(1.29) and LAS120(1.00). As well as bone-deficit size, mean LAS-point in A-group was significantly higher than that in C-group at LAS90, but was lower at LAS120. Mean glenoid width ratio was 77.1% (A-group), 85.6% (C-group) and 81.0% (whole group). Significant correlation between LAS-point and glenoid width ratio was seen at LAS90 in C-group and whole group.<BR><B>Discussion:</B> Although the bone-deficit in C-group was smaller than in A-group, the point of LAS120 in C-group was higher than A-group. These results suggest that it was because the humeral head and glenoid contact on different areas, and the ligament restriction of the anterior translation of humeral head is also different at different glenohumeral abduction angles. About the correlation between LAS-point and glenoid width ratio, we speculate that it was because LAS-point does not increase more than three points even if the bone-deficit is larger.

    Our aim is that effective use of regional collaboration clinical pathway for hepatocellular carcinoma with high recurrence rate and clearly defined high-risk group improves intermediate and final outcomes study. If we can derive the same outcome as a specialist even if we are not a specialist by using the regional collaboration clinical pathway, we believe that we can contribute to the quality of regional medical care. It was revealed that it is equivalent to a specialist even if it is not a specialist. We have improved the quality of medical care by using the clinical pathway method, as a goal of the optimal preventive and therapeutic measures against difficult-to-treat diseases such as cancer.
