    正常な妊娠維持には適切な免疫寛容、Th17およびTregの適切なバランスが重要である。早産を含めたさまざまな疾病発症に関わるとされる腸内細菌叢はこれらTh17/Tregの分化誘導にも大きな影響を与える。また、腸内細菌叢は生体内における唯一の分子状水素産生源でもある。つまり、Th17/Treg、腸内細菌叢、分子状水素は互いに密接な関係にある。一方で、生理的に産生される分子状水素の生体内での役割、特にヒト妊婦における役割は未だ解明されていない。本研究の目的は分子状水素がヒトの妊娠免疫寛容にもたらす影響の検討およびヒト妊婦への分子状水素投与を実践に移すことである。
    早産および胎児期に感染/炎症に曝された児は、脳性麻痺や慢性肺疾患など長期予後に大きな影響を及ぼすことが知られており、世界的な問題である。早産の予防や発症予測する試みは社会的要請度の高い課題である。
    我々は兼ねてより早産や胎児脳障害に関する病態および炎症性サイトカインの関わりなどを研究してきた。これら知見をもとに、近年では分子状水素が抗酸化作用や抗炎症作用を有すること、分子量が小さいため拡散により胎盤や脳血液関門を容易に通過できる特性を持つことに注目し、妊娠予後の改善を図る新規管理法としての、生体内における生理的な分子状水素産生量が妊娠予後に与える影響、ならびに、経母体的分子状水素投与の有用性について研究している。
    胎児は母体にとって半異物であり、正常な妊娠維持には適切な免疫寛容が必須である。免疫寛容の一端として、CD4陽性T細胞のsubtypeであるTh17およびTregの適切なバランスが重要とされる。早産を含めたさまざまな疾病発症に関わるとされる腸内細菌叢はこれらTh17/Tregの分化誘導にも大きな影響を与える。また、腸内細菌叢は生体内における唯一の分子状水素産生源でもある。つまり、Th17/Treg、腸内細菌叢、分子状水素は互いに密接な関係にある。
    一方で、生理的に産生される分子状水素の生体内での役割、特にヒト妊婦における役割は未だ解明されていない。本研究の目的は分子状水素がヒトの妊娠免疫寛容にもたらす影響の検討およびヒト妊婦への分子状水素投与を実践に移すことである。
    令和3年度に予定していた実験ならびに仮説の検証は順調に遂行し得た。
    具体的には;
    1)実験動物を用いた解析:Tcell活性化物質であるantiCD3投与の早産モデルマウスにおいて、飽和水素水の強制飲水および自由飲水の併用が早産予防効果を有するとの結果が得られた。一方で、antiCD3投与と同時に分子状水素を投与する治療効果の検証においては有意な差を示せなかった。
    2)ヒト検体・情報を用いた解析:満期帝王切開症例を対象に無菌的にヒト羊膜・絨毛脱落膜を回収し組織培養。リコンビナントIL-26添加よりTNF-α、IL-6等のサイトカイン発現の有意な上昇を確認できた;健常人の末梢血単核球細胞を用いた実験においては予定していたTh17、Tregに加えてTh1細胞もフローサイトメトリーにて分離回収。antiCD3存在下に抗原提示細胞と共培養。分子状水素投与が各細胞の分化誘導やeffector化を抑制する働きを有するとの結果を得た。なお、当初の計画どおりにTh17はCD3+,CD4+,CCR6+,CXCR3-分画Th1はCD3+,CD4+,IFNγ+分画を持って、TregはCD3+,CD4+,Foxp3+分画を持って定義し、Th17のeffector化はIL26+orIL22+を、Tregのeffector化はFoxp3+hi&CD45RA-を持って定義した。
    当初の仮説・推察のとおり、分子状水素がTh17の分化ならびにeffector化を修飾し、早産予防効果を発揮する可能性ことが分かった。次年度の研究では、免疫細胞や早産発生減少がどのような作用機序の元に起きるのかその解明に着手する。また、ヒト妊婦糞便/膣分泌物の回収ならびに16srRNA解析を実施し、ヒト腸内/膣内細菌叢と生体内H2濃度、Th17/Tregの関連を検討する。

    We first demonstrated the relationship of maternal hydrogen levels to immune cells and clinical PTB. Our findings suggested that there is a population of pregnant women with low hydrogen levels, and these women are divided into a high-risk group for PTB. In addition, we have shown that Th17, which is a negative factor for maternal immune tolerance, and Th17-producing inflammatory cytokine IL-26 could be suppressed by molecular hydrogen. Due to the ease and simplicity, measuring maternal hydrogen levels could be a potential clinical tool in the management of PTB. Moreover, supplementation of molecular hydrogen may be beneficial in the prevention of PTB.

    It has been demonstrated that, in our maternal immunse activation (MIA) model mice, maternal administration of H2 attenuates oxidative damage and neuroinflammation in the fetal brain. In the present study, behavioral abilitiy were evaluated at postnatal 3 or 4 weeks. The number of neurons and oligodendrocytes was also analyzed at postnatal 5 weeks by immunohistochemical analysis. Offspring of the LPS-exposed dams showed deficits in short-term memory and social interaction, following neuronal and oligodendrocytic loss in the amygdala and cortex. Maternal H2 administration markedly attenuated these LPS-induced abnormalities. Moreover, the number of activated astrocytes was increased in LPS-exposed offspring, but decreased in the offspring of H2-administered dams. Overall, these findings indicate that maternal H2 administration exerts neuroprotective effects and ameliorates MIA-induced neurodevelopmental deficits of offspring later in life.
