    本研究の目的は、「分子状水素水を用いた間質性肺炎(IP)に対する新たな治療法の開発」である。本研究ではマウスによる肺線維症モデルおよびIPの一因であるARDSモデルを用いて、分子状水素水のIPに対する治療効果とそのメカニズムを系統的に分析する。分子状水素の活性酸素(ROS)に対する防御機能や、Nrf2を介したヘムオキシダーゼ(HO)-1、制御性T細胞(Treg)の誘導機能により、IPやARDSが制御されることが期待される。
    現在、分子状水素を用いた様々な臓器保護の研究が行われている。肺に関しても理解が得られつつあり、今後の展望に期待ができる。我々は、マウス気管移植後OAD(obliterative airway disease)モデルにおける分子状水素の効果に関する研究を行ってきた。これを基盤に、分子状水素水の間質性肺炎(IP)に対する効果およびそのメカニズムを検討し、将来の臨床試験への道標とすることが主な目標である。肺はその構造と機能のため、常に活性酸素(ROS)による酸化的障害を受けやすいとされている。 これを防ぐためには抗酸化物質による酸化-抗酸化(レドックス)制御が重要である。ROS は、脂質、蛋白、核酸を修飾あるいは変性させ、またミトコンドリア膜を傷害し直接アポトーシスを誘導する。さらに、細胞外や細胞内におけるシグナル伝達物質として、細胞 増殖・分化・アポトーシスを制御する機能を持つ。IPにおいては、上皮細胞損傷の一因として、マクロファージや好中球、 線維芽細胞由来のROSが増加し、抗酸化物質が低下していることが挙げられている。 転写因子Nrf2は、さまざまな環境因子による酸化ストレスに対する防御機構の代表である。 Nrf2が誘導する 酸化ストレス防御遺伝子群の中では、ヘムオキシゲナーゼ(HO)-1の働きが主要視されている。さらに、HO-1は制御性T細胞(Treg)機能の亢進による免疫制御に関わっていることが知られており、我々はマウスの気管移植片にて分子状水素水によるTregのマスター転写因子であるFoxP3の増強効果を発見した。FoxP3は、Tregの分化・機能発現・分化状態の維持すべてにおいて必須の役割を担う。
    我々はこの期間、マウスモデルにおいて、分子状水素水投与とNrF2、HO-1、FoxP3および、IL-2、IL-6などのサイトカインとの関連の研究を概ね計画どおりに順調に進めてきた。分子状水素水が Nrf2 を介して HO-1 を誘導することにより、 FoxP3 とそれに引き続く Treg の増強により、抗炎症・抗酸化作用を発揮するのではと期待している。
    ブレオマイシンを用いたマウスモデルにおいて、分子状水素水の病理組織学的ならびに分子生物学的な効果をさらに検証していく。現在までに判明しているNrf2/HO-1 pathwayが、このモデルにおいても仮説通りに機能しているかを検証する。現時点では研究計画の変更や研究を遂行する上での課題を認めていない。仮説が否定されるような結果となった場合、なぜその結果になったか慎重に検討し、実験手法を再設定する。
