    統合失調症の病態に神経ネットワーク形成障害等の関与が提唱されているが、未だ明確ではない。今回、統合失調症死後脳の生前の臨床情報のデータベース化から、特定の表現型を抽出し、関連する脳組織病理所見を明らかにすることにより、その臨床表現型と関連した神経ネットワークの変化(GABA系、カテコラミン系、オリゴデンドロサイト/ミエリン等)を神経病理学的アプローチにより明確化する。また、ゲノム変異を同定することで遺伝的背景が明確な統合失調症死後脳でも比較検討し、組織表現型とその分子生物学的な背景との関連性を明確にする。
    前年に引き続いて、例数を増やすために関連施設において死後脳の収集・蓄積を行うとともに、生前の臨床診断、臨床経過(発症年齢、罹病期間、臨床症状、薬剤、併存身体疾患等)、病前の社会適応、家族歴、神経画像等の臨床情報の収集を行った。これらの臨床情報及びCPCによって確認した神経病理学的診断情報を漸次データベース化した。
    前年度までの検討で、有意な神経変性疾患を認めない認知症症状を示す統合失調症症例群を、この疾患の表現型のひとつと考え、本年度は、引き続き、その中の3例に関し、さらに詳細に臨床情報と神経変性疾患の病理所見との比較・検討を行った。神経原線維変化、老人斑、嗜銀顆粒、レビー小体等の病変はごく軽度認められるものの、既知の神経変性疾患の病理のみで、これらの症例の認知症症状を説明することは困難であった。これらの症例では、統合失調症自体の病態による脳の脆弱性に、ごく軽度の加齢性変化が加わり、重度の認知機能低下を引き起こしている可能性が推量された。
    また、稀なゲノム変異を有する統合失調症では、22q11.2欠失を有する統合失調症において、大脳皮質におけるmyelin oligodendrocyte glycoprotein(MOG)の発現の変化(皮質の層構造における発現の変化、ミエリンの厚みの変化)、白質におけるNogo-A陽性オリゴデンドロサイトの密度減少を病理所見として確認していたが、これらの神経病理学的所見と臨床経過(生前の神経画像所見や知的能力、薬歴や薬剤への治療反応性を含む)との関連を検討した。
    2020年度も引き続き例数を増やすために死後脳の収集・蓄積を行うことは目標の一つであったが、2020年は関連施設において、15例程度の死後脳の収集・蓄積を行うことができた。さらに、これらの症例に関して、順次、生前の臨床診断、臨床経過、病前の社会適応、家族歴、神経画像等の臨床情報の収集及び一般神経病理所見の検討を行い、詳細な臨床情報と神経病理学的診断情報のデータベース化をさらに推し進めた。
    有意な神経変性疾患を認めない認知症症状を示す統合失調症症例群については、その中の3例に関しさらに詳細な臨床神経病理学的評価を行い、統合失調症の脳病態と加齢性変化との関係について、一定の推量をすることができ、論文として投稿、受理された。
    また、稀なゲノム変異を有する統合失調症においては、前年のGLO1フレームシフト変異を有する統合失調症に加え、22q11.2欠失を有する統合失調症において、それまでに見出した神経病理学的所見と臨床経過との関連を検討し、これらを論文として投稿、受理された。
    これらのことから、概ね順調に進展していると考えられる。
    見出した特定の表現型の死後脳組織を免疫組織学的な手法を用いて観察を行う。有意な神経変性疾患を認めない認知症症状を示す統合失調症症例群はその症例数は少なく、一定の臨床表現型を抽出したとはいえ、生物学的な背景病態の異質性はやはり想定される。
    脳画像上、大脳皮質の萎縮を認めるが、神経病理学的には既存の変性疾患の所見を認めず、X染色体に大規模な重複を持つ統合失調症死後脳の皮質萎縮の組織病理学的背景を先ず明らかにすることも、引き続き考慮する。

    Expression of Myelin Oligodendrocyte Glycoprotein (MOG) and thickness of MOG-positive fiber-like structures were decreased in the superior temporal gyrus and the CA3 of hippocampus of the long-term schizophrenic brains. These findings suggest that oligodendrocyte- myelin abnormalities in these regions may be related to the pathophysiology of schizophrenia.
    In also schizophrenic brains with rare variants (22q11.2 deletion, GLO1 frameshift mutation), morphological alternation of proteins associated with oligodendrocytes-myelin (MOG, Nogo-A) were observed. These findings contribute to understanding of relationship between pathological phenotype and molecular biological backgrounds.
