    免疫チェックポイント阻害薬(ICI)は従来の抗がん薬とは一線を画す作用機序により画期的な治療効果を有し、現在適応症も拡大の一途をたどっているが、過剰な細胞障害性T細胞 は様々な臓器に重篤な副作用(免疫関連有害事象:irAE)を引き起こし、正常な心筋をも攻撃しがん患者の生命予後に大きく影響を来すものとして心血管irAEが挙げられる。心血管irAEに関する発症予測因子や免疫学的な関連メカニズムの解明が望まれるが、近年、がん病巣拡大の指標として注目されている可溶性免疫チェックポイント分子(sPD-L1)が心血管irAE発症時より上昇をするため、臨床上有用なバイオマーカと成り得るかを検証する。
    当院ではがん化学療法関連心血管合併症のチャートレビュー(承認番号 2019-0176)研究を行ってきており、2021年 Prognostic impact of immune-related adverse events on patients with and without cardiovascular disease: a retrospective review をCardiooncologyに投稿済である。(2021 Jul 6;7(1):26. doi: 10.1186/s40959-021-00112-z)。2020年9月からは化学療法部と循環器内科連携でICI投与患者全例に対して、左心機能の僅かな変化の検出にも有用とされる心エコーでのgloballongitudinal strain (GLS)評価や定期的な血液検査を多科共同で行う体制を構築し、稼働を開始しており、2020年4月27日以降 登録患者数は499人となっている。心筋炎を合併した症例に関しては心筋生検を行い、HE染色に加えて、CD3, 4, 8, 20, 68, PD-1, PD-L1 染色を行い、病態と免疫染色との関連に関して更なる解析を行ってきた。また他臓器におけるirAE と心血管合併症との関連やその後の予後に関してや心エコー上のGLS データとバイオマーカとの関連に関しても着実にデータ収集ができている。
    2022年度の研究計画通り、概ね順調に進行をしている。
    2022年度以降、複数の劇症型心筋炎症例を経験している。これらの血液検体からヒト蛋白マイクロアレイを用いて、自己抗体の同定を目指す。

    We prospectively enrolled CS patients with abnormal FDG accumulation in the heart (n=56) from October 2016, and calculated total lesion glycolysis (TLG) before and after treatment to evaluate the response to treatment. Patients in the Poor Responder group were randomly divided into two groups: PSL re-increase group and MTX intervention group as intensification therapy. After six months of PSL treatment, about 85% of patients showed a reduction in TLG of 70% or more. However, in the treatment-resistant group, neither re-PSL nor MTX had a significant effect on TLG reduction.

    We examined the relationship between serum FGF-23 levels and biomarkers, hemodynamics, and prognosis in 42 patients with dilated cardiomyopathy. FGF-23 levels in peripheral vein was negatively correlated with estimated glomerular filtration rate and positively with high-sensitivity troponin T levels. In survival analysis, the cardiac event rate was significantly higher in the High FGF-23 group than in the Low FGF-23 group. In multivariate analysis, the high FGF-23 group was an independent predictor of cardiac events. In multiple regression analysis, the estimated glomerular filtration rate and brain natriuretic peptide levels were significant determinants of FGF-23 levels. Collagen volume fraction in endomyocardial biopsy samples was not significantly correlated with peripheral FGF-23 levels, but was weakly correlated with the transcardiac gradient of FGF-23 levels defined as the difference in concentration between the coronary sinus and the base of the aorta.

    Hypertrophic cardiomyopathy (HCM) is characterized by gradual thickening of the cardiac muscle and impairing myocardial blood flow (MBF). We assessed MBF undergoing NH3-PET and in order to perform transmural layer analysis, the reconstructed images were transferred to a workstation using PMOD software. We enrolled 17 control patients and 16 HCM patients. Even though there was no significant difference between control patients and HCM patients in subendocardial MBF at rest, hyperemic subendocardial MBF in HCM was significantly lower than normal patients (1.159±0.144 vs 1.422±0.202, p=0.0002). Furthermore, HOCM was significantly lower than apical HCM patients (1.32±0.18 vs 1.92±0.49). These results might relate the major cause of sudden cardiac death due to latent myocardial damage in HOCM patients.
