    食道癌は解剖学的・生物学的特徴から早期に転移をきたす予後不良な疾患である。なかでも難治な食道癌血行性転移の治療成績を改善するには、癌進展の精緻な病態解明に基づく新規分子標的治療薬の開発が望まれる。研究代表者らはTranscriptome解析において血行性遠隔転移を有する食道癌組織で有意な発現増加を認める分子群から標的分子としてAKR1B15とGDPD2を厳選した。本研究ではこれらの食道癌転移分子機構における機能を明らかにするとともに、医薬基盤・健康・栄養研究所創薬デザイン研究センターとの共同研究で革新的な効果を有する食道癌治療アンチセンス核酸医薬、およびその治療奏効度を予測するコンパニオン診断技術の開発を目指す。

    食道癌転移経路特異的Transcriptome解析により、申請者らの仮説に一致して血行性転移群とリンパ行性転移で発現パターンが明瞭に異なるクラスターがあることが示され、血行性転移関連5分子(VWA5B2、CFAP47、NECAB2、SYCE3、RPL3L)、リンパ行性転移関連3分子(C1QL4、CCDC87、CAMK2N2)を厳選している。
    これらの食道癌転移形式別関連分子の機能解析・発現解析を行うことで、食道癌転移の分子生物学的機序の解明を通じ、食道癌予後を劇的に改善しうる分子標的治療薬と、その奏効度を事前に予測しうるコンパニオン診断法開発の基盤となるデータを得ることを目的とする。
    食道癌転移経路特異的Transcriptome解析により、血行性転移群とリンパ行性転移で発現パターンが明瞭に異なるクラスターがあることが示され、血行性転移関連分子およびリンパ行性転移関連分子を厳選している。本研究では、これら食道癌転移形式別関連分子の機能解析・発現解析を行うことで、食道癌転移の分子生物学的機序の解明を通じ、新規分子標的治療薬とその奏効度を事前に予測しうるコンパニオン診断法開発の基盤となるデータを得ることを目的としている。前年度までは、臨床的重要度の高い血行性転移に関連する分子として、NECAB2を第一候補に解析を進めてきた。前年度にはsiRNAを用いたノックダウン実験系による機能解析、多施設共同研究体制による発現解析を進めてきた。
    令和4年度の主な成果として、以下のデータを取得した。NECAB2の人工的発現抑制により、caspase3/7活性の亢進、細胞周期G0/G1期の増加を認めた。これらは細胞増殖能阻害活性の機序を説明しうるものとなる。NECAB2を発現ベクターで強制発現することにより、癌細胞増殖能が亢進した。Western blotting法による細胞内シグナルの解析では、NECAB2を抑制することにより細胞周期調節因子のcyclin D1, CDK2のリン酸化が低下することを発見した。さらに、NECAB2抑制により食道癌細胞の血管内皮への接着能が低下することを見出した。これは、NECAB2が血行性転移に特異的に作用することの機序解明として重要なデータとなる。また同時にNECAB2に匹敵しうる有望な食道癌転移関連分子を検索したところ、siRNAによる発現抑制が高度の細胞増殖抑制を引き起こす分子は同定されなかった。そのため、NECAB2についてさらに深く検討する方針とした。
    令和4年度は、複数の血行性転移関連分子、リンパ行性転移関連分子を横断的に比較解析することで、最有望標的分子候補としてNECAB2選定に至った。NECAB2が、どのような食道癌細胞機能を調節しているかを説明しうるデータ構築が順調に進むとともに、本研究の独自性の強い部分である転移形式特異的な挙動についても、血管内皮への接着能解析によりデータ取得に成功した。令和5年度は、これまでの知見をさらに深めるデータを取得しつつ、in vivo実験に重きを置いて計画を進めていく。
    ここまで順調にデータ構築してきた食道癌血行性転移関連分子NECAB2についてさらに詳細な検討を進める。
    機能解析(担当:小寺・田中):主要な細胞機能への影響は評価済みであるが、さらにアポトーシス細胞比率、食道癌のKey drugである5-FUおよびCDDP感受性への影響についてin vitro実験系で解析する。
    in vivo実験(担当:小寺・神田・清水):樹立済みのshRNAによるNECAB2安定的ノックダウン細胞株を用いる。まず、マウス皮下腫瘍モデルでの造腫瘍能を解析する。ついで、癌細胞尾静注によるマウス肺転移モデルにおける転移形成能についてモニタリングする。肺転移モデル作成方法はすでに確立している。
    シグナル解析:前年度までの成果で、NECAB2は特に細胞周期関連因子に強く干渉し、癌細胞機能を調節することが示唆されている。ここを切り口に、より強固なデータを取得する。

    本研究では、細胞接着因子を標的としたアンチセンス核酸医薬による新たな胃癌分子標的治療薬およびコンパニオン診断技術開発を目指して実験を進めている。令和3年度は、以下の進捗を得ている。
    1) アンチセンス核酸スクリーニング:アクセス効率を最大化するmRNAの二次構造予測システムを用いて候補配列をデザインし、29配列の中からノックダウン効率とin vitroでの胃癌細胞機能阻害効果から2つの有望配列を取得した。これら配列については、標的分子の発現レベルが低い癌細胞では抑制活性を示さず、オンターゲット活性であることも確認できている。
    2) メカニズム解明:同分子阻害の作用機序を調査している。2つの有望アンチセンス核酸配列を用いた標的細胞接着因子ノックダウンにより、胃癌細胞接着能および浸潤能が低下することを明らかにした。標的細胞接着因子無発現癌細胞株であるKATOIIIに対して標的分子の強制発現を行うと、胃癌細胞の増殖能が増強した。細胞内シグナル伝達系のリン酸化状態を網羅的に調査可能なPTM Scanを用いてアンチセンス核酸による標的細胞接着因子ノックダウン前後の細胞内シグナルの変化についてデータを得た。
    3) in vitro活性評価:選抜した有望アンチセンス核酸配列の濃度依存性なin vitro細胞増殖能阻害活性について、0.25nMから800nMの濃度域で検証した。このデータをもとに、翌年度以降のin vivo解析の投与量を設定していく。
    令和3年度は、序盤での有望配列スクリーニングが速やかに完了し、取得した2配列を用いた基礎データの構築が順調に進んだ。in vivo毒性テストの結果をみるまでは最終配列決定には至らないが、これら2配列を主軸に研究計画を進めていく。また、シグナル解析の第一歩としての網羅的リン酸化解析のデータを取得できたため、翌年度以降に再現性確認を含めて詳細に検討していく。標的細胞接着因子の阻害が生体に致命的な影響を与えないかを調べるためのノックアウトマウスの合成にも着手した。
    研究計画は順調に推移しており、当初の計画に沿って令和4年度も研究を進めていく。メカニズム解明(担当:研究代表者 小寺、研究分担者 神田、清水)として、癌細胞の転移能や薬剤抵抗性に重要である癌幹細胞性について調査する。アンチセンス核酸による標的分子ノックダウン前後でのスフェロイド形成能について調べる。また、胃癌を含むさまざまな固形癌におけるkey drugである5-FUおよびシスプラチン感受性への影響についても解析する。令和3年度にPTM Scanで抽出された候補シグナル系に着目してwestern blotting法で詳細に検討する。ゲノム編集技術を用いたノックアウトマウスを樹立する。
    ついで、in vitro活性評価(担当:研究分担者 神田)も拡大する。選抜配列を用いて、癌種横断的(食道癌、膵癌、大腸癌、肝癌、乳癌)に細胞株に対する細胞増殖阻害能をin vitroで調べる。in vivo活性評価(担当:研究代表者 小寺、研究分担者 清水)として、有望2配列のマウス皮下腫瘍モデルに対するアンチセンス核酸腹腔内投与での造腫瘍能抑制効果を評価する。

    本研究では、細胞接着因子を標的としたアンチセンス核酸医薬による新たな胃癌分子標的治療薬およびコンパニオン診断技術開発を目指して実験を進めている。令和4年度は、以下の進捗を得ている。 メカニズム解明のために、ターゲットバリデーションを進めた。標的分子の機能解析としてアンチセンス核酸によるノックダウン前後でのスフェロイド形成能を比較したところ、ノックダウンにより顕著にスフェロイド形成は阻害された。また、幹細胞性のsurrogateマーカーであるALDH活性はノックダウンにより低下した。どのような細胞内シグナルに干渉することで活性を示しているかを調べるためにPTM Scanで得られたリン酸化状態のデータを解析して候補シグナル系を抽出した。標的分子の生態・恒常性維持における役割を調べるため、WT/ヘテロ/ホモノックアウトマウスを作成した。次年度に胎生死の有無、生態、成長、行動、代謝(血液検査を含む)および生殖を経時的に観察していく。in vitro活性評価も進め、癌種横断的(胃癌、大腸癌、肺癌、乳癌、食道癌、膵癌)な細胞株に対するアンチセンス核酸の細胞増殖阻害活性データを取得した。in vivo活性評価に先だって、in vitro細胞毒性評価とin vivo毒性評価(肝毒性に着目)を行い、配列選抜を進めた。コンパニオン診断開発に向けて、胃癌症例の組織検体を対象に免疫染色法を実施し、標的蛋白の組織中発現解析の条件設定を完了した。
    令和4年度は、有望配列スクリーニングを経て、取得した配列を用いた基礎データの構築が順調に進んだ。in vivo活性テストに向けて毒性評価も進んでいる。メカニズムを明瞭化するための機能解析、シグナル解析も順調に進捗している。標的因子の阻害が生体に致命的な影響を与えないかを調べるためのノックアウトマウス合成も完了した。コンパニオン診断技術開発のための免疫染色法至適条件設定も完了した。
    メカニズム解明(担当:小寺・神田・清水):アンチセンス核酸によるノックダウン前後での5-FUおよびシスプラチン感受性の変化について解析する 。PTM Scanで抽出された候補シグナル系に着目してwestern blotting法で詳細に検討する。作成済みのWT/ヘテロ/ホモノックアウトマウスを用いて、胎生死の有無、生態、成長、行動、代謝(血液検査を含む)および生殖を経時的に観察し、標的分子喪失の生体への影響を調べ、安全性データの一部とする。標的分子は中枢神経系組織中に比較的発現が多いとの報告もあるため、名古屋大学神経内科教室の協力のもと、KOマウスの認知運動障害の有無をローターロッド試験にて評価する。
    毒性評価とオフターゲット探索(担当:笠原・小比賀):アンチセンス核酸の有害事象として肝毒性が報告されており、投与量をエスカレーションした場合の安全性を担保するためマウスのAST等を指標に各濃度域での肝毒性や、休薬によってどの程度回復するかについて調査する。
    in vivo活性評価(担当:神田):マウス皮下腫瘍モデルでの腫瘍形成抑制活性評価を実施する。ついで、胃癌腹膜播種モデルもしくは同所移植モデルに対するCEM法でのアンチセンス核酸腹腔内投与(週1回、3濃度設定)により、造腫瘍能/転移形成/生存期間を評価する。
    コンパニオン診断(担当:田中):免疫染色の至適条件は設定済みである。主に手術標本組織を対象に免疫染色法を実施し、組織中標的蛋白発現の臨床的意義を調べる。

  5. 高齢消化器癌手術症例における術後自立度低下に関するリスク評価と介入の有効性の検討

    本研究は,消化器癌における様々な術式の自立度への影響と複数の診療科を含む多職種横断的な介入の効果を包括的に把握し,高齢者に対する癌治療のエビデンス構築の一助となるデータを構築することを目的とする。まず,術前後の自立度の変化について調査票と血清バイオマーカー値の両面から評価する。次に,自立度が低下する患者については,老年科専門医・リハビリテーション・栄養サポートチームによる介入で機能維持を図り,ひいては,事前に自立度が低下する群を予測し,予防的介入が可能となることを見込む。
    これまでの研究で、術後に自立度が低下する群が存在する可能性が示唆されていた。このため本研究では、消化器癌における様々な術式の自立度への影響と複数の診療科を含む多職種横断的な介入の効果を包括的に把握し、高齢者に対する癌治療のエビデンス構築の一助となるデータを構築することを目的としている。
    本年度は、術前、退院時、術後6ヶ月に施行している高齢者機能評価(自立度)の調査を進め、約100例の症例を集積し、解析を開始している。
    さらに75歳以上の胃癌を有する高齢者に対して胃切除術を行った患者を対象として、老年科専門医による評価および術後早期の理学療法士によるリハビリテーションの導入症例の集積を進め、解析可能な症例数を得た。以上により、介入による有効性を評価するが、介入の有効性、あるいは抽出したリスク症例に対する介入の有効性に関しては、現時点で介入効果が不明であるため、研究分担者である統計家と密に相談しながら、評価を行う。
    また、自立度が低下した症例群と維持された症例群の2群において、自立度のバイオマーカーの探索を開始した。自立度が維持された症例と低下した両群の症例の術前血清を用いて、網羅的なたんぱく質の同定および比較定量を行った。本年度はこの結果を得た。自立度が維持される候補タンパクとして7種、低下に関与する候補タンパクとして10種類を抽出した。次年度は術前検体を用いて血清中の候補タンパクの発現を定量化し、さらに手術による自立度の変化との関連を調査することを予定している。
    胃切除術を行う高齢者を対象として高齢者機能評価を施行した症例およびリハビリテーション介入を施行した症例の集積をほぼ完了した。
    症例の集積は終了したため、今後は術前・術後因子のリスク因子の抽出を行い、介入の影響も検討する。また、自立度に影響を及ぼす可能性がある候補タンパクの抽出は終了している。今後は、術前検体を用いて血清中の候補タンパクの発現を定量化し、手術による自立度の変化との関連を調査し、自立度に関わるバイオマーカーの探索を行う。いずれも最終年度に終了する見込みである。

    We aimed to develop novel therapeutic agents using antisense nucleic acid drugs for metastatic or recurrent gastrointestinal cancers. Through a transcriptome analysis, CSRNP3, CPLX1, and RNFT2 were selected as candidate molecules controlling cancer metastasis. The antisense nucleic acids designed with the secondary structure prediction system showed an inhibitory effects of cell proliferative potential in a variety type of cancers. Antisense nucleic acid against CSRNP3 inhibited tumor progression in mouse peritoneal metastasis models. Moreover, knockdown of CSRNP3 enhanced the antitumor effect of cisplatin in mouse subcutaneous tumor models. Tissue expression levels were correlated with survival and metastatic recurrence rates in patients with gastric cancer.

    As a pivotal clinical trial for development of treatment strategy for clinical stage III gastric cancer, a multicenter, randomized phase III clinical trial to confirm the superiority of addition of preoperative adjuvant chemotherapy with S-1 + oxaliplatin (SOX) over standard treatment is ongoing. If the efficacy of chemotherapy can be predicted before decision making of treatment, physicians can select the most appropriate treatment method for each patient, which will have great significance not only in terms of treatment effect but also in terms of medical economics. In this study, we performed a multi-omics analysis and identified candidate markers for the efficacy of preoperative SOX therapy in plasma samples (protein and microRNA). Moreover, we confirmed the feasibility of the Cancer Panel using endoacopic biopsy FFPE samples. The results of this study will greatly enhance the quality of a future biomarker study.

    Since patients with esophageal cancer have a poor prognosis, identification of novel esophageal cancer-related molecules is desirable. In this study, we focused on NPTXR, a receptor that is frequently upregulated in esophageal squamous cell carcinoma. shRNA-mediated functional analysis of NPTXR revealed that NPTXR regulates the proliferative potential, invasive potential, and 5-FU resistance of esophageal cancer cells. It was demonstrated that NPTXR mediated malignant phenotypes of esophageal cancer cells primarily through Akt-mTOR signaling. Anti-NPTXR antibody inhibited the growth of esophageal squamous cell carcinoma cell lines in vitro. NPTXR expression levels in esophageal cancer tissues was clearly distinguishable by immunohistochemical staining and correlated with postoperative recurrences and metastasis.

  10. 介入による消化器癌周術期高齢者機能の改善と機能低下に影響するバイオマーカー探索

    一昨年の解析結果をまとめると、胃癌に対する胃切除を施行した症例において、自立度、錯乱、認知機能は術前の平均スコアと比較して、退院時の平均スコアのみでなく術後6ヶ月もスコアは低下していなかった。さらに退院時に自立度が低下した症例を対象として低下の予測因子を調査したところ、年齢、性別、胃全摘術、せん妄は関連を認めなかった。しかし、術後合併症の有無(p=0.0326)が自立度低下の予測因子としてあげられた。術前と比較した退院時の認知機能低下は40例中7例(18例)に認められた。
    本研究においてMMSEやCDR-Jといった認知機能の評価は専門医である老年科医師により行われている。現実問題として、これらの評価を周術期に外科医が行うことは困難である上に、専門医による認知機能の評価がどの病院でも常時可能であるとは言えない。我々は認知機能低下症例におけるバイオマーカーを含めた予測因子の探索を行った。
    まずは術前と比較して退院時にMMSEスコアが低下した症例を対象に、臨床病理学的な予測因子の有無を調査したところ、術後せん妄を認めた症例(p=0.1890)、術後在院日数が本研究の平均値である19日を超える症例(p=0.0751)、Clavien-Dindo分類II以上の術後合併症を発症した症例(p=0.0498)であった。以上より、術後合併症を発生した症例はMMSEスコアが有意に低下していたことがわかった。術前IL-6・アディポネクチン・レジスチンの値と術後MMSE低下に明らかな関連は認めなかった。術後CDR-Jスコアの悪化と上記の各因子との間に関連は認めなかった。
    以上より、認知機能の低下と関連するバイオマーカーは見いだせていないが、術後合併症の発生は、自立度のみでなく認知機能の低下に影響する可能性があると考えられた。
    症例集積に時間を要した。しかし、昨年度で高齢者機能評価の測定とバイオマーカー測定は概ね終了した。今後は成果報告を行う。
    成果報告を行うために1年延長した。
    今後は学会、英語論文での成果報告を進めたい。
    さらに、サルコペニアやサルコペニア症例に対する手術の影響などより詳細な解析を進める。バイオマーカー探索も続ける予定である。

  11. Molecule target for treatment and companion diagnostic tool of peritoneal dissemination in gastric cancer

    Peritoneal metastasis is a frequent cause of death in patients with gastric cancer. The pathophysiology and thus targets for treatment are unknown. Synaptotagmin XIII (SYT13) was expressed in patients with peritoneal recurrence of gastric cancer. Inhibition of SYT13 expression in gastric cancer cell lines was associated with decreased invasion and migration ability of the cells. In vivo experiments using a mouse xenograft model of gastric cancer showed that intraperitoneal administration of SYT13-specific small interfering RNA or antibodies significantly inhibited the growth of peritoneal nodules and prolonged survival. SYT13 expression in primary gastric cancer tissues or ascites fluids had diagnostic potential both in the detection and prediction of peritoneal metastasis of gastric cancer. SYT13 may also be a promising target for the treatment of peritoneal metastasis of gastric cancer.

  12. The diagnostic performance and mechanisms of the release into the circulation of novel serum biomarkers for gastric cancer

    Development of high-performance serum biomarkers will likely improve treatment outcomes of patients with gastric cancer (GC). We conducted a prospective multicenter observational study aimed at validating the diagnostic performance of these potential markers. We analyzed serum levels before and after surgery of the three potential biomarkers in patients with GC and healthy volunteers. Quantification of serum and GC tissue levels was performed using an ELISA. Area under the curve values that discriminated patients with GC from healthy controls were-0.7058, 0.6188, and 0.5031 for ANOS1, DPYSL3, and MAGED2, respectively. Serum ANOS1 levels were significantly elevated in patients with stage I GC compared with those of healthy controls and decreased after resection of primary GC lesions. The combination of serum ANOS1 and DPYSL3 levels increased the AUC value that discriminated patients with GC from healthy controls.

    In this study we create an outcome-based training course for endoscopic surgery.
    Competences required for each operative procedure were analyzed and subdivided, and the unit was defined as “module”. At first the tasks of conventional dry laboratory were applied to these modules, and the new training systems were constructed. Using the method of instructional design, the structure of modules was modified and a module was rebuilt while performing surgical seminars. A course for laparoscopic cholecystectomy that most surgeon initially learn was designed as a model of procedure course. New box trainer related to many modules was developed as a module improvement.

    Regarding the training of endoscopic surgery, we usually use animal laboratory training as intermediate task between simulation training of basic technique and on-the-job training during actual operation, while there are many problems in the animal laboratory training as well known.
    In this study, we aimed to create new tasks to replace animal trainings. We developed the perivascular dissection program and the intestinal anastomosis program as the integrated tasks combining basic procedures. We created various phantoms of intestinal tract and box trainers. We also used VR module for the perivascular dissection program. Evaluation of both program was based on a checklist of a supervising doctor and a trainee and the results were fed back to the trainee in the briefing. We actually perform these programs in our simulation center, improving the systems.
