    The purpose of this study was to evaluate epileptogenic networks in focal epilepsy using novel modalities such as rsfMRI, MEG, and EEG-fMRI, create the connectome map, and justify with stereotactic electroencephalography (SEEG). Multilayer analysis with rsfMRI revealed epileptogenic hubs near the focus with excitatory connection to the contralateral hubs and inhibitory connections to the default mode network, and this may reflect pathological mechanism in focal epilepsy. Modified DS(distributed source) analysis in MEG and subsecond analysis in EEG-fMRI improved detection of epileptic focus and its networks. Novel metrics for demonstrating connectivity to canonical networks were created, and used as a connectome map. SEEG analyzed with coherence showed relationship between epileptic focus and its networks.

    MRgFUS (MR-guided focused ultrasound) has become possible in Japan as a non-invasive coagulation therapy for the treatment of involuntary movements such as essential tremor. In this study, the improvement rate of the upper limbs was about 65% one year after treatment, and no serious complications were observed. In addition, network analysis by resting fMRI revealed that the more severe the tremor, the lower the connectivity of the network, and the higher brain function mainly in the frontal lobe is also impaired.

    We examined 7 patients with West syndrome (6 patients with cryptogenic West syndrome and 1 with tuberous sclerosis complex) by EEG-fMRI at the onset of West syndrome before ACTH therapy. The temporal information of periodic hypsarrhythmia was used to perform event-related analysis for detecting hypsarrhythmia-related blood oxygenation level-dependent (BOLD) changes on fMRI. As a result, positive BOLD changes were seen in brainstem and multiple cerebral cortices in 6 of 7 patients. After ACTH therapy epileptic spasms disappeared in all patients. However spasms relapsed in 2 patients and focal seizures were seen in 2. At 12 months of age, 3 patients had focal epileptiform discharges. EEG-fMRI at 12 months of age in these 3 patients revealed bilateral frontal positive BOLD in 2 patients and left hippocampal positive BOLD in 1. Positive BIOLD signal in the brainstem disappeared at 12 months of age in all 3 patients.

    There have been remarkable advancements in the neurosurgical operation theaters, such as operative microscopes, endoscopes, fluorescence imaging and blood flow imaging. Moreover, intraoperative brain mapping has become common to localize and evaluate brain functions. In our study, we have developed a new image integration technology which allow us to fuse among various optical imaging modalities and radiological images, such as images of MRI and CT (Multimodal Optico-Radiological Image Integration; MORI). We have confirmed usefulness of the technology in several clinical settings, demonstrating that it enable us to identify both functional brain areas obtained by intraoperative brain mapping, and remnant tumor infiltrating bran areas shown by intraoperative fluorescence imaging on anatomical images. The technology could enhance future development of a new and comprehensive surgical information platform.
