    Purpose: Advanced Kienbock's disease often involves displaced fractures of the lunate; however, whether such fractures affect surgical outcomes remains unclear. The purpose of this study was to evaluate the results of radial osteotomy for Kienbock's disease with displaced fractures of the lunate and whether these fractures would achieve union.
    Methods: We reviewed patients who had radial osteotomy for Kienbock's disease between 1975 and 2004. We included only displaced fractures of the lunate that were apparent on lateral radiography, and 31 patients (24 men, 7 women; mean age, 35 y) were then included in this study. Postoperative state of the lunate and clinical results were compared between cases of union and nonunion.
    Results: Preoperatively, mean flexion/ extension arc was 76 degrees, and mean grip strength was 22 kg. Postoperatively, mean flexion/extension arc was 840, and mean grip strength was 30 kg. Radiography revealed fracture healing in 16 wrists. No significant differences between union and nonunion were noted in any assessed clinical items.
    Conclusions: This study showed approximately 50% union after radial osteotomy. Fracture union did not affect clinical results. Clinical results were good considering the advanced stage of Kienbock's disease. Even with displaced fractures of the lunate, radial osteotomy yields results comparable with other treatments for same-stage patients and thus represents a reasonable approach.

    Purpose: Ulnar shortening is a widely used procedure for various conditions associated with ulnar wrist pain, including triangular fibrocartilage complex (TFCC) injury; however, few reports have examined the condition of the TFCC after osteotomy. The central avascular zone of the TFCC generally is considered to have no potential to heal. This study investigated whether the avascular zone of the TFCC has any potential for repair, and whether repair of the torn disc proper correlates with clinical findings.
    Methods: Between 1987 and 2005, we performed 75 second-look arthroscopies after an ulnar-shortening osteotomy for ulnar wrist disorders. Of these, 32 wrists with a TFCC (disc proper) tear on first arthroscopy were included in this study. Data from patient charts, radiography, and video images of arthroscopy were reviewed retrospectively. Tears of the disc proper were classified as radial, central, or ulnar tears, and as either linear or round tears.
    Results: Meticulous second-look arthroscopy showed repair of tears in 50% of studied wrists. Round tears tended to repair better than linear tears. Although the final clinical score was better in repaired wrists than in nonrepaired wrists, no marked differences were noted between groups in terms of age, gender, preoperative ulnar variance, follow-up period, or surgical procedures used.
    Conclusions: The avascular zone of the TFCC possesses some potential for repair; however, factors promoting spontaneous repair of this tissue were not identified.

    Background
    Although accumulating evidence shows that mesenchymal stem cells (MSC) are a promising cell source for articular cartilage repair, the fate of transplanted MSC has not been extensively studied.
    Methods
    To monitor their persistence and differentiation, we labeled uninduced MSC with a fluorescent dye, PKH26, and transplanted them, in a poly-glycolic-acid scaffold, to full-thickness defects made in the weight-bearing area of rabbit femoral trochleae with hyaluronate sheets. The fate of the labeled cells was monitored for up to 8 weeks.
    Results
    Two weeks after transplantation, immature cartilage containing collagen type II had formed. By 8 weeks, this cartilage had thinned and immunolabeling for collagen type II gradually disappeared from the basal region, which became positive for collagen type I. Most chondrocytes within the regenerated cartilage were PKH26-positive and, therefore, derived from transplanted MSC, whereas osteoblasts within the regenerated bone were a mixture of donor- and host-derived cells. The thickness of the cartilage became thinner up to 8 weeks and then remained stable up to 42 weeks after surgery.
    Discussion
    These results showed that uninduced MSC were able to survive osteochondral defects and differentiated according to the environment, making a major contribution to initial cartilage formation and a partial contribution to bone regeneration.

    We conducted a retrospective review of 11 patients with bilateral Kienbock's disease from our series of 251 patients with Kienbock's disease. There were no significant differences in radiographic parameters, including ulnar variance and carpal bone angle, between those with unilateral and those with bilateral Kienbock's disease. None of the patients with bilateral disease had been treated with corticosteroids or had a systemic disease that predisposed to osteonecrosis. Thus, this study failed to demonstrate any risk factor for bilateral, as opposed to unilateral Kienbock's disease.

    Ulnocarpal impaction syndrome was diagnosed in six wrists of five patients with neutral or negative ulnar variance. All underwent ulnar shortening with satisfactory results. The average grip strength increased from 53% to 78% and the range of flexion-extension increased from 82% to 93%, the mean Cooney's score improved from 25 to 83. These cases show that ulnocarpal impaction syndrome can occur in wrists with zero or negative ulnar variance, and that ulnar shortening is an effective treatment for such wrists.

    In the elderly, functional recovery after peripheral nerve injury is insufficient, in part because of age-related degeneration of postsynaptic structures. In this study, the nerve transfer was performed for the long-term denervation muscle of the aged mouse. By injecting the Wnt antagonist intramuscularly into the denervated muscle and suppressing the Wnt/β catenin signal pathway, we found that the reinnervation of the denervated muscle was improved. By using the Wnt antagonist for the nerve injury treatment of the elderly in which the change of the postsynaptic structure is remarkable, the possibility of preventing further degeneration of the acetylcholine receptor after the nerve injury and improving motor function recovery after the nerve repair was suggested.

    Injuries and diseases such as spinal cord injury and amyotrophic lateral sclerosis maintain normal brain function, but extensive neurogenic muscle atrophy progresses to severe, life-threatening disorders. In this research, we are developing a cybernetics technology that achieves functional recovery to a level where independent living is possible by applying three originally developed basic technologies for such intractable paralysis.
    (1) Ganglionic induction of spinal-cord-like structures near paralyzed muscles by transplantation of neural stem cells into peripheral nerve trunks. (2) A newly developed multiple controllable implantable electrical stimulator. (3) Artificial intelligence that can predict brain control signals and control them with implicit knowledge.

    The effect of agrin on the neuromuscular junction after denervation was investigated.
    We examined the effect of agrin decline on the neuromuscular junction after denervation between agrin deficient mouse and wild type mouse. Degeneration of the neuromuscular junction after denervation was found to be more severe in Agrin deficient mice. Next, agrin and saline were locally administered to the denervated muscles, and the differences in their effects were compared and studied. Deterioration of the neuromuscular junction was shown to be mild in the Agrin topical administration group.

    周術期に呼吸法とストレッチングを基本とした運動療法を実施した上肢筋骨格系障害、外傷に対して手術が行われた16歳以上の患者106例(男性59、女性47例)、平均年齢48±16歳を対象として、周術期における関連痛の有無、関連する要因について調査した。肩関節および脊椎に病歴のある患者と腕神経叢損傷患者は、対象から除外した。手術2週前、手術前日、手術翌日、術後1週時に関連痛の有無、関連痛の部位とpain VAS(Visual Analog Scale)、McGill 痛み質問表SF-MPQ、上肢機能評価表Hand10、自己評価抑うつ尺度SDS、痛み不安症状尺度PASS20、心理的ストレス反応尺度SRS18を調査した。関連痛の有無によって2群に分け、各評価項目に加えて、手術侵襲の大きさ、麻酔の種類(全身麻酔、伝達麻酔)、性別、年齢、BMI(Body Mass Index)、喫煙および飲酒の有無、職業(white、 blue-collar、non-worker)について統計学的解析を行った。関連痛あり51例(男性26、女性25例)、平均年齢47±18歳、なし55例(男性33、女性22例)、平均年齢48±14歳であった。2群間の比較において、手術侵襲の大きさは全例intermediateであり、全ての項目において有意差を認めなかった。上肢筋骨格系障害、外傷に対する周術期において48%の症例に関連痛を認めたが、疼痛強度、自己評価上肢機能、うつ状態、不安、ストレス、性別、年齢、BMI、喫煙、飲酒、職業については、関連痛との相関を見出すことはできなかった。

    We assessed approximately 40 patients who had upper limb joints pain. We analysed factors for related joint pain. Some factors (cytokines in the joints) had a significant correlations with pain and functions both pre and post operation. We will present this finding in the future.
    And we published some papers about upper limb joint pain.

    我々が以前神経縫合部に使用したPLAフィルムには薄く柔らかいと破れやすいもろさがあり、厚くなると硬くなり凹凸のある組織との接着性に問題が生ずる。また、フィルムであるために神経に対して周辺からの血行を妨げる可能性があった。エレクトロスピニング法(粘性溶液に電圧を印加することでファイバーを得る手法)で作成されたポリヒドロキシアルカノエート(PHA)ファイバーマットは引張強度で比較するとPLAフィルムよりも弱かった。しかし、応力-ひずみ曲線から材料の降伏点における歪みエネルギーの大きさを求めるとPHAファイバーマットの方が優れていた。特に膜厚100μmのPHAファイバーマットはより大きな負荷を加えても永久歪みを生じにくいと考えられた。そこでラット坐骨神経断裂部に人工神経としてこのPHAファイバーマットを使用する実験を行った。ラット坐骨神経を大腿中央で切断後、断裂部を10mm四方のPHAファイバーマットで包み込むように被覆、両端を1針ずつ10-0ナイロンで縫合した。PHAファイバーマットはPLAフィルムより約10倍の厚みがあるにもかかわらず柔らかく操作性は良好であった。手術1か月後の肉眼所見においてまだファイバーマットは残存し、神経縫合部も再断裂することなく維持されていた。縫合部のマッソントリクローム染色でファイバーマット周囲に多核巨細胞の出現を認めたが、炎症性細胞の浸潤は認めなかった。1か月以内での再断裂が無かったことは初期強度的な問題が無いことが考えられる。今後、生分解の期間とその過程で生じる現象や神経機能回復に及ぼす影響を調査していく必要がある。現在は1カ月までの標本しか得られていないが、2か月以降経過の標本を待機している段階である。転勤のために実験は共同研究者及び新任者が引き継いで行っていく。

    In contrast to cardiac muscle or smooth muscles, re-innervation is a prerequisite not only for functional recovery but also for survival of striated muscles. However, severe neuronal damage at higher levels such as complete spinal cord injury has little chance of recovery. Should the damaged nerve tissue be reconstructed by means of regenerative medicine technologies, optimal functional recovery would be less likely to occur due to extremely long recovering time of neural network. In this experimental study, we tried to develop a technology that allows swift re-innervation and functional recovery of striated muscles even in such situations by implanting motoneurons near paralyzed striated muscles. We first developed an ES-cell derived motoneuron integrated microelectrode array, however, due to difficulty of long time maintaining of cells on chips in vivo, we abandoned the technology. We thendeveloped a novel technology named motoneuron integrated striated muscles (MISM). Development of MISM technology consists of 4 parts, i.e. induction of motoneurons that can stably form functional neuromuscular junctions from pluripotent stem cells, transplantation of induced motoneurons into Wallerian degenerated peripheral nerve trunk, development of implantable FES system that , and motioncontrol by tacit learning program based functional electrical stimulation.

    In work-related upper limb musculoskeletal disorders such as carpal tunnel syndrome and stenosing tenosynovitis, symptom duration correlated negativelywith pain severity and positively with disability. In pathological finding of tenosinovium, mast cell triptase which is an agonist of PAR2 was mainly express around vessels. Mast cell triptase correlated positively with cytokines such as TNF-α and IL-6. Weconsider that these factors were involved in pain severity. The expression of triptase decreased and the expression of VEGF increased over the course of the disease. Wethink that mast cell triptase plays an important role in the inflammation of the tenosynovium and pain severity.

    The purpose of this research is to achieve artificial limbs such that those devices have a similar level maneuverability and controllability as human. The research took three steps. First, we established a method for picking up the meaning from action potentials of motor neurons. Second, we proposed a method for feeding back sensory signals to brains by electrical stimulations. Third, we integrated these two methods to achieve a high performance of artificial limbs.
    The experimental and simulation results show that the electrical stimulation for artificial sensory feedback can be conducted non-invasively, and the positive effects appear in the limb movements ; moreover, the stimulation is very useful for rehabilitation and assisting human movements. We applied the technique of electrical stimulation to design of assistive robotic device for upper limbs and to planning of rehabilitations. We also conducted the research for measuring sensory information by a newly developed tactile sensor, and introduced the sensor into artificial hands. It is shown with the experiments that the introduction of the feedback by the tactile sensor is quite useful for maneuvering the artificial hand.

    GFP導入マウスより通常の方法にて体外受精を施行し、受精卵を作成した。作成した受精卵に凝集法を用いて幹細胞(幹細胞については現在市販されているマウスのES細胞を培養し使用)を導入することでmosaicismを作成した。子宮内に移植して妊娠させ、出産させた個体について、導入した細胞の確認・分布について蛍光顕微鏡を用いて確認。出生した個体は精巣・卵巣を含む全身に導入した細胞の確認が出来ていた。ついでGFP導入マウスのES細胞を樹立し、これを用いて通常のマウスの受精卵とのmosaicismの作成を試みた。集合胚は作成されるものの、マウス子宮内へ移植した後の妊娠の継続がされなかった。また、中胚葉性幹細胞(MSC)についてはマウス大腿骨より分離・培養した細胞を用いてmosaicismの作成を試みた。しかし、これについても集合胚は得られるものの妊娠の継続が不能であった。両者ともに集合胚は確認されており、mosaicismの作成・子宮内への移植は同様の方法で行っており、細胞レベル等何らかの障害が発生し妊娠の継続がえられなかったと考えられた。
    先天異常モデルマウスについてはC57BL6 Hm+系統(合指・合趾症モデルのマウス)のマウスを導入した。受精卵を採取し、この系統のマウスに対してのmosaicismを作成することにより先天異常の発生を防ぐことを試みた。このマウスを繁殖させ、仔の四肢に幹細胞(C57BL6/EGFP系統:ES細胞・MSC)を注入し、細胞の生着および組織への分化を確認している。今後はこの結果を踏まえて論文などにまとめていく予定となっている。
