    The reduced migration/invasion of extravillous trophoblasts (EVTs) is a key feature of the genesis of preeclampsia. We and others previously reported that transcriptional factors activator protein-2 (AP-2) alpha and AP-2gamma act as suppressors of tumor invasion. The present study examined the expressions of AP-2alpha and AP-2gamma in preeclamptic placenta vs. control placenta and investigated their effect on the function of EVTs. The expressions of AP-2alpha and AP-2gamma were elevated in the preeclamptic placentas in comparison with the gestational age-matched control placentas. Their expressions also increased in EVTs of the preeclamptic placentas. Thereafter, we transfected AP-2alpha or AP-2gamma into human EVT cell line, HTR-8/SVneo. The overexpression of AP-2alpha or AP-2gamma decreased the migratory and invasive abilities in HTR-8/SVneo cells. This was followed by the reduction of protease activated receptor-1 and matrix metalloproteinases and a significant induction of plasminogen activator inhibitor-1 and the tissue inhibitor of metalloproteinase-1. AP-2alpha and AP-2gamma were weakly expressed in the cultured EVTs and HTR-8/SVneo cells, whereas they were induced by TNF-alpha, which increases in preeclamptic placenta and impairs trophoblast invasion. In the presence of TNF-alpha, the invasion of the HTR-8/SVneo cells was partially restored by a blocking of AP-2 induction using small interfering RNA of AP-2. The present data suggest that AP-2 may suppress trophoblast migration and invasion, thus leading to a shallow placentation in preeclampsia.

    妊娠高血圧症候群は、未だに母児ともに予後不良な疾患である。児は子宮内胎児発育遅延を合併し、新生児医
    療の進歩によりその生存率は改善したものの、脳障害などの後遺障害が問題となる。他方、母体では、HELLP
    症候群や脳血管障害といった重篤な合併症が問題となっている。脳血管障害は、我が国の妊産婦死亡原因の約
    25%を占めることが明らかとなり、その管理に関心が高まっている。病態は絨毛外絨毛細胞(EVT)の母体脱落
    膜への浸潤不全による胎盤低形成にあるとされるものの、未だ不明な点が多いために、有効な予防法や治療法
    が確立されていない。我々は、癌研究領域でのendoglinが細胞浸潤へ関与するという報告およびに妊娠高血圧
    症候群胎盤ではendoglin発現が増加するという報告に着目し、endoglinが胎盤低形成に関与しているという
    仮説をたてた。Endoglinが低酸素やTGF-といった胎盤低形成因子によって発現増加するという報告や、ラッ
    ト動物実験でその注入によりHELLP症候群モデルを作成したという報告もあり、本研究では、成因を解明する
    と同時に、重篤な合併症の発症予測・予防法を確立し、妊娠高血圧症候群の予後の向上を目指したい。

    不妊症・不育症には自己免疫疾患が合併することがあるが、その詳細は不明である。我々は患者血清中に存在する自己抗体とその標的タンパクProstate, ovary, testis, ankyrin domain family member F (POTEF)を同定した。POTEFは卵巣、胎盤に特異性高く発現し、自己免疫機序によるPOTEFの障害が卵巣機能不全、反復流産の病態であるという仮説をたてた。
    卵巣、胎盤におけるPOTEFの機能解析を行うとともに、抗POTE抗体をマウスに投与し、自己免疫機序による病態の再現が可能かどうか検証する。抗POTEF抗体のアッセイ系を用い臨床的検討もあわせて行う。

    Preterm birth occurs by inflammation, and the fetus is affected by exposure to inflammation. In the present study, it was shown that exposure to inflammation in utero caused postnatal autism spectrum like findings, accompanied with several alterations in the brain tissues, amyglada, including microglial activation. Furthermore, some of the molecular mechanisms have been elucidated. In addition, as a preventive strategy against , the results suggesting that antenatal molecular hydrogen administration may prevent the development of postnatal neuropsychiatric disorders in preterm infants. Pregnant women with other inflammation related diseases will be also indicated as candidates for this preventive strategy, although further studies are needed.

    For the purpose of not only the development of obstetrics and gynecology in Vietnam, but also creation of evidence worldwide and building of medical research resources to Japan, we investigated obstetric management and gynecological rare tumor in Vietnam at several facilities in Vietnam. In the present study, several findings, which are related to improvement of mothers and children's prognosis,were gained. First, Pregnancy with dengue fever causes thrombocytopenia, which is at risk of postpartum hemorrhage. Second, cesarean section scar pregnancy with an increase in cesarean section rate in Vietnam, could be managed by medication for only selected patients. Third, evaluation of cardiac function by ultrasound examination for fetal growth restriction would be beneficial. Lastly, we found that there was a high secondary rate after molar pregnancy in Vietnam and preventive measures might improve it.

    妊娠高血圧症候群の病態へのスフィンゴシン1リン酸(S1P)の関与を証明し、S1P受容体をターゲットとした新しい治療法を提示することを目的としている。現在では、妊娠高血圧症候群は絨毛外栄養膜細胞(extravillous trophoblast, EVT)の母体脱落膜への浸潤不全によって発症するという説が有力であり、本研究では、S1PのEVT浸潤機能に対する直接作用についてその分子機構まで解明する。また、S1Pの主な供給源は血球系であるとされているが、本研究では、脱落膜のスフィンゴシンキナーゼ(SK)により合成されたS1PがEVTをパラクラインに刺激しその浸潤を調節しているという仮説を証明する。S1PによるEVT浸潤調節の役割を証明した後、妊娠高血圧症候群症例におけるこれらの物質の発現を正常妊娠症例と比較検討する。さらに、新規免疫抑制薬FTY720をはじめとするS1P受容体作動薬の投与がEVT浸潤調節機構に及ぼす影響についてもその分子機構まで証明し、その治療的応用の可能性を提示していく。
