    BACKGROUND:
Recent advance of genetic testing has contributed to the diagnosis of hereditary pheochromocytoma and paraganglioma (PPGL). The clinical characteristics of hereditary PPGL are varying among the types of mutational genes. It is still difficult to specify the pathognomonic symptoms in the case of rare genetic mutations. Here, we report the case of synchronous bilateral pheochromocytomas and paraganglioma with novel MYC associated factor X (MAX) gene mutation.

    CASE PRESENTATION:
A 24-year-old female had hyperhidrosis and hypertension. Her urine test showed high normetanephrine and vanillylmandelic acid. Enhanced computed tomography revealed three enhanced masses in right adrenal gland, left adrenal gland, and left renal hilus. She was diagnosed with PPGL. Because 123I-metaiodobenzylguanidine scintigraphy indicated the accumulations in the left adrenal gland mass and the left renal hilus mass and not in the right adrenal gland mass, we performed laparoscopic left adrenalectomy and extirpation of the left renal hilus mass to preserve the right adrenocortical function. However, her symptoms recurred shortly after the operation presumably due to unveiling of the activity of the right pheochromocytoma. Following right adrenalectomy as the second operation, the catecholamine levels declined to normal range. Her genetic testing indicated the novel germline mutation in MAX gene (c.70_73 del AAAC/p.Lys24fs*40).

    CONCLUSIONS:
MAX germline mutation is recently identified as a rare cause of hereditary PPGL. The deletion mutation in MAX gene in this patient has never reported before. In the case of bilateral pheochromocytomas, the surgical indication should be decided considering each patient's genetic background. Due to the possibility for other types of malignant tumors, close follow-up is essential for MAX mutation carriers.

Oncolytic viruses are a promising method of cancer therapy, even for advanced malignancies. HF10, a spontaneously mutated herpes simplex type 1, is a potent oncolytic agent. The interaction of oncolytic herpes viruses with the tumor microenvironment has not been well characterized. We injected HF10 into tumors of patients with recurrent breast carcinoma, and sought to determine its effects on the tumor microenvironment. Six patients with recurrent breast cancer were recruited to the study. Tumors were divided into two groups: saline-injected (control) and HF10-injected (treatment). We investigated several parameters including neovascularization (CD31) and tumor lymphocyte infiltration (CD8, CD4), determined by immunohistochemistry, and apoptosis, determined by terminal deoxynucleotidyl transferase dUTP nick end labeling assay. Median apoptotic cell count was lower in the treatment group (P=0.016). Angiogenesis was significantly higher in treatment group (P=0.032). Count of CD8-positive lymphocytes infiltrating the tumors was higher in the treatment group (P=0.008). We were unable to determine CD4-positive lymphocyte infiltration. An effective oncolytic viral agent must replicate efficiently in tumor cells, leading to higher viral counts, in order to aid viral penetration. HF10 seems to meet this criterion; furthermore, it induces potent antitumor immunity. The increase in angiogenesis may be due to either viral replication or the inflammatory response.

In a mouse model of spontaneous pancreatic neuroendocrine tumors that frequently cause liver metastasis and peritoneal dissemination, we investigated whether tumor immunosuppression contributes to disease progression and can suppress disease progression by activating tumor immunity. An attempt was made to examine the activation of immune cells in the primary and metastatic lesions of pancreatic neuroendocrine tumors in this model mouse by immunohistochemical staining, but the staining conditions were not determined and could not be examined. Regarding the activation of tumor immunity by drugs, which was planned to continue, the evaluation method was not determined and no results could be obtained.

We attempted to establish a care program for breast cancer-related lymphedema in diabetic patients. We examined from various viewpoints based on the complete decongestive therapy (CDT).
MR images of the forearm revealed that fluid accumulates in ulnar side more than radial side. Therefore, it is recommended that the drainage should focus on this part. After 120 minutes of meal, it was found that blood pressure stabilize. Hence CDT should be carried out at 120 minutes after a meal. Because the evaluation using a tape measure or subjective symptoms was not sufficient, in order to understand the situation of edema, magnetic resonance imaging or ultrasonic diagnostic imaging is necessary. In self-care, it is important to visually grasp the effects of care in order to encourage patients. For this purpose, we considered to use a 3D scanner.

We explored treatment / prevention methods by clarifying pathologically and molecular biologically the biological properties of tumor developed in glucagon gene knockout mouse in which pancreatic tumors arose spontaneously. We reported that the pancreatic tumor developed in this mouse closely resembles histological and biological attitudes to human pancreatic neuroendocrine tumor (pNET). Expression of cellular growth factors and angiogenic factors (AKT, mTOR, VEGF, etc.) in pancreatic islet cells at each stage of the process of malignant transformation were examined by immunostaining of excised tissue specimens. Everolimus, an mTOR inhibitor which has clinical indication for pNET, was continued to be administered to these mice shortly after birth, and it was confirmed that pancreatic tumor development was suppressed.
