    本課題は腹腔内微小環境における癌関連腹膜中皮細胞に注目して、腹腔内オカルト播種の休眠の機能および維持メカニズムを追究する研究である。休眠の維持に関する解析項目に「腫瘍-CAMのNotchシグナル」や免疫回避の第一歩として「SDF-1α制御-腫瘍関連マクロファージ」との関連を取り入れた斬新かつ挑戦的な研究である。本課題で得られた知見によって腹膜全体をターゲットとした包括的治療戦略の策定や新規バイオマーカーの同定を目指す。特に腹膜は面積的に広範囲であるため、標的にしえた場合の治療及びQOL改善の効果も大きいと考えられる。さらに本研究は卵巣癌に限らず癌性腹膜炎を形成しうる他癌にも応用可能となりうる。

    本研究は、卵巣がんにおけるエクソソームを始めとした細胞外小胞(Extracellular Vesicle: EV)を対象とし、EVが有する未だ明らかにされていない腫瘍生物学的意義を明らかにし、臨床応用へ向けた基盤となる知見を得ることを目的とします。未だ臨床的問題点が多く残る難治性卵巣がんにおけるEVの機能解明は極めて重要と考えられます。本研究では、①卵巣がん悪性化に関わるEVの機能解析 ②個別化医療を実現するEVバイオマーカーの創出 ③EVによる難治進行性卵巣がんの新規治療開発の、3点を軸に研究を展開し、卵巣がん患者予後の改善に繋が成果を得ることを目指します。

    We reported how ovarian cancer (OvCa)-associated mesothelial cells (OCAMs) induce platinum-resistance in OvCa cells through direct cell-to-cell crosstalk. We evaluated mutual associations between OvCa cells and human primary MCs with in vitro co-culturing experimental models and in silico omics data analysis. Results of in vitro experiments show that mesenchymal transition is induced in OCAMs primarily by TGF-β1 stimulation. OCAMs can induce decreased platinum-sensitivity in OvCa cells via induction of the FN1/Akt signaling pathway via cell-to-cell interactions. Further, we also confirmed activation of Akt signaling in OvCa cells in contact with TGF-β1 stimulated peritoneum, using an in vivo mice model. Their results suggest that the tumor microenvironment, enhanced by direct cell-to-cell crosstalk between OvCa cells and OCAMs, induces acquisition of platinum-resistance in OvCa cells, which may serve as a novel therapeutic target for prevention of OvCa peritoneal dissemination.

  9. Analysis of tumor-infiltrating lymphocytes in malignant gynecologic tumors for development of personalized tumor immunotherapy

    In order to develop personalized immunotherapy for malignant gynecologic tumors, we aimed to characterize tumor infiltrating lymphocytes (TIL) and to clarify the mutated neoantigens of individual patients with malignant gynecologic tumors.
    Although TIL yield varied between patient samples, we could expanded the TILs from the small fragments of tumor in most cases (97.6%). A total of 14 patient derived xenografts (PDX) models were established. It could be confirmed that the reactive cells to autologous tumor cells were present in the expanded TILs. In two cases with ovarian cancer, we identified candidates of mutated neoantigens by the next generation sequencing combined with bioinformatics prediction and evaluated TILs from the same patients.

    We demonstrated that the exosome from ovarian cancer stimulated the peritoneal dissemination. By mouse model the exosome also prompted abdominal metastasis and causes morphological change in mesothelial cells. This phenomenon was caused by induction of apoptosis. We observed that the exosome increased MMP1 expression in mesothelial cells.
    Using clinical data base, we observed that MMP1 is an important prognostic marker and there are ovarian cancer patients with ascites including MMP1 mRNA.
    In conclusion, the exosome including MMP1 mRNA may be a prognostic biomarker and a treatment target for preventing abdominal metastasis.

    For the purpose of not only the development of obstetrics and gynecology in Vietnam, but also creation of evidence worldwide and building of medical research resources to Japan, we investigated obstetric management and gynecological rare tumor in Vietnam at several facilities in Vietnam. In the present study, several findings, which are related to improvement of mothers and children's prognosis,were gained. First, Pregnancy with dengue fever causes thrombocytopenia, which is at risk of postpartum hemorrhage. Second, cesarean section scar pregnancy with an increase in cesarean section rate in Vietnam, could be managed by medication for only selected patients. Third, evaluation of cardiac function by ultrasound examination for fetal growth restriction would be beneficial. Lastly, we found that there was a high secondary rate after molar pregnancy in Vietnam and preventive measures might improve it.

    We aimed to determine the role of cancer-associated mesothelial cells (CAMCs) in the promotion of tumor neovascularization and vascular permeability via enhanced VEGF production and loclal immunologic effects. We examined whether a characteristic morphological change in human peritoneal mesothelial cells (HPMCs) was observed in the presence of malignant ascites and tumor-derived TGF-β. We focused on the enhanced production of VEGF in CAMCs and its crucial role in endothelial tube formation. We found that EOC-derived TGF-β induced typical EMT-like morphological alteration in HPMCs, which was associated with CAMCs. We further discovered that CAMCs play a crucial role in the enhanced VEGF production.Furethermore we showed that IL-33 secretion was elevated in high metastatic cell lines.The novel mechanism of CAMCs as a facilitator of EOC progression and immnunological effect are displayed by microenvironmental cell-to-cell communication.

    Photodynamic therapy (PDT) is known as a minimally invasive treatment for cancer. 5-Aminolevulinic acid (ALA) is a precursor of the photosensitizing agent protoporphyrin IX (PpIX). We evaluated the efficacy of ALA-PDT and investigated relevant factors of enhancement of effect on various ovarian cancer cells in vitro and in vivo.
    We confirmed the effect of ALA-PDT using various ovarian cancer cell lines. Especially,we found that three cell lines of clear cell carcinoma, which is chemoresistance tumor,were also highly sensitive to ALA-PDT.The cell lines sensitive to ALA-PDT expressed PEPT1 (an ALA uptake transporter) adn the cell line resistant to ALA-PDT expressed ABCG2 (a PpIX export transporter). In the resistant cell line, a combination treatment with both ALA and an ABCG2 inhibitor resulted in the promotion of cytotoxic sensitivity in vitro.
