    研究代表者等は、大規模なゲノムコピー数変異(CNV)解析を実施し、統合失調症の発症に強い影響を及ぼすCNVを多数同定した。CNVが、脳の発達に重要な遺伝子の機能に影響を及ぼすことで神経発達障害が起こり、発症に繋がると推測される。本研究では、この結果をもとに、MRIを用いて高解像度T1画像、拡散テンソル 画像、安静時fMRI画像を取得し、発症関連CNVが統合失調症の脳の構造や脳内ネットワークの結合性に与える影響を検討する。
    今年度は、自閉スペクトラム症の患者において15q11.2-q13.1重複という自閉スペクトラム症では最も有名な変異を同定し、MRI撮像を行った。今後は、iPS細胞の樹立を計画しMRI解析結果と合わせて評価する予定である。
    また、統合失調症患者(SCZ)と健常者(CON)をサポートベクターマシンを用いて判別する方法を検討した。名古屋大学で取得したMRIデータ(SCZ50名、CON51名)を用いて判別器を学習させ、富山大学で取得したMRIデータ(SCZ49名、CON48名)で判別器の精度を評価した。トレーニングデータの判別精度は73.3%、テストデータの精度は72.2%(感度:61.2%、特異度:83.3%、陽性適中率:78.9%、陰性適中率:67.8%)であった。判別に関与した領域は、両側内側前頭皮質、側頭極、島皮質、および左舌回であった 。本研究においてSVMによる判別方法は、異なるプロトコルおよびMRI装置を使用して撮像した独立したデータセットにおいてもほぼ同じ精度を達成した。現在、英文雑誌に投稿準備中である。
    本研究では、名古屋大学・脳とこころの研究センターに設置されているMRI装置を使用しMRIデータを集積しているが、装置の不具合により、データ収集に遅れが生じている。現在は、装置は修復されており来年度は引き続きデータ収集を継続する予定である。
    ゲノムコピー数変異を持つ患者は、精神症状が重篤であることが多く、病状によりMRIデータが取得できないこともデータ収集が遅延している理由である。
    当初3年間の研究期間を予定していたが、データ収集に遅延が生じているため、補助事業期間の延長を申請し承認を得た。来年度も引き続きMRiデータおよび臨床データの収集を継続する予定である。発症関連CNVと統合失調症の脳内ネットワークの障害との関連を検討すると共に、臨床データ(陽性・陰性症状評価尺度や知的機能の簡易評価など)と脳内ネットワークの障害との関連についても検討する。
