    吸入麻酔薬が一部のがん細胞の増殖を促すとする報告が増えるなか、その安全使用のための正確な基礎データの集積が各国で望まれている。また近年では血管新生を促すがん細胞は遺伝子の異常修飾を利用し一部のケモカイン遺伝子発現を抑え、生体内の新規の血管を誘導することが知られてきた。そこで本研究では吸入麻酔薬暴露後に血管新生を生じたマウス皮下腫瘍がこのようなケモカイン発現不均衡を生じたかどうかを検証をする。
    現在複数の麻酔薬が癌の外科手術に適用を受けているが、そのうち吸入麻酔薬では心筋保護作用などの未だ十分に解明できていない細胞への影響がある。しかしこの保護作用をがん細胞が利用しているという懸念を払拭するために、厳密な研究が望まれている。そこで本研究では周術期に必要な吸入麻酔薬のうち、その覚醒の速さから汎用されているセボフルランを取り上げた。本研究ではセボフルラン暴露後の大腸がん細胞への影響を明らかにする基礎データの集積を目的としている。これまでに我々は吸入麻酔薬セボフルランを暴露した一部の大腸がん細胞株が、ケモカイン遺伝子の発現不均衡を示しながら増殖を高めることを観察した。このことから本研究計画ではまず、この予備実験で観察された増殖能亢進を複数の大腸がん細胞株でも観察できるかどうかを検討し、さらに異種移植による血管新生脳の有無を検討する。そのうえでセボフルラン暴露後に増殖を高めた大腸がん細胞株を絞り込み、血管新生に関わる遺伝子の発現を高めるかどうかを検討し、ケモカイン遺伝子の発現不均衡の有無を同定していく計画である。
    今年度までに本研究で得られた実績では12種類のヒト大腸がん細胞株についてセボフルラン暴露後の増殖能を検討した点である。その結果6種類の細胞株について単層培養における増殖亢進が観察された。さらにこれらの3種類の細胞株については異種移植下でも腫瘍形成の亢進が確認でき、一部では血管増殖遺伝子の発現増を同定した。
    新型コロナウィルスによる休校措置により、研究分担者の出勤が適わなくなり計画の遅延が生じた。実験助手を雇いその遅延に対処したがまだ十分ではない。
    採取した腫瘍組織の免疫染色を行い、また血管新生に関わるケモカインの発現量を定量していく。推進方策としては新型コロナウィルスによる大学休業によりテレワークで行える解析などできる限り進める。

    術後の覚醒の早さから外科手術に広く用いられている吸入麻酔薬であるが、一部の培養がん細胞株の増殖に影響があるという報告もあり、この作用機序を明らかにすることは喫緊の課題である。一方、原がん遺伝子RASは大腸がん患者の45-50%に変異を生じ、抗がん剤への治療抵抗性を示すことからバイオマーカーとして治療前検査が行われている。そこで本研究は麻酔薬暴露後の増殖原因の一つにKRAS遺伝子を仮定しその発現量などを比較検証する。本研究が達成されれば、広く用いられている吸入麻酔薬の安全利用に資する重要な基礎データを提供でき、抗がん剤事前検査で用いるバイオマーカーKRASを麻酔薬選択のために新たに活用する可能性を広げる。
    一部の麻酔薬が低酸素下での細胞死を抑制する効果があることが動物や培養細胞を用いた研究などが報告されているが、がん細胞に対して麻酔薬がどのように働くのかまだ明らかになっていない。一方で担癌患者は近年その外科的切除手術数を伸ばしているが、原がん遺伝子RASに変異をもつ大腸がん患者への抗がん剤への治療抵抗性は、遺伝子治療を対象とする重要な治療因子となっている。そこで本研究ではこの原がん遺伝子RASに着目し、麻酔薬後に生じる増殖能の変化と、変異の有無との関連を明らかにすることを目的とした。これまでに研究代表者は広く汎用されている吸入麻酔薬の一つセボフルランに暴露した由来の異なるヒトがん細胞株らが、その増殖能を高めたことを報告した。特に一部の結腸がん細胞株はこの原がん遺伝子KRAS(KRAS proto-oncogene)の発現を高めて、免疫不全ヌードマウスの皮下で腫瘍を大きくしたことに着目した。そこで原がん遺伝子の変異の有無に着目し、今年度までに12種類のヒト大腸がん細胞株についてセボフルラン暴露後の増殖能を検討した。その結果野生型RASをもつ6種類の細胞株について単層培養における増殖亢進とさらに、3種類の結腸がん細胞株については異種移植下でも腫瘍形成の亢進が確認できた。また細胞ライセートによる細胞分裂因子タンパク質の増加、血管新生遺伝子の発現増などを同定した。今後はKRAS遺伝子の発現量を確認し、マイクロアレイによる検討を進める。
    新型コロナウィルスによる休校措置により、研究分担者の出勤が適わなくなり計画の遅延が少なからず生じたが、初年度の研究進捗に変更をきたすほどではない。

    Volatile anesthesias have been reported protect myocardial tissue against reperfusion injury on myocardial ischaemia and kidney disturbance. Surgical treated cancer cells potentially influenced similar protection in the perioperative period. We recently showed one of a volatile anesthesia sevoflurane induced the growth on some of cancer cell lines as context independent manner. In this studied we revealed that sevoflurane induced Wnt/β-catenin canonical pathway accompanied with MAPK activation by established Wnt knock out clones derived from human colorectal cancer cell line. These clones also revealed that one of Wnts gene is necessary for anchorage independent growth and colonization growth post its exposure of clinical relevant dosed sevoflurane. Above all we clarified that sevoflurane onwards the Wnt/β-catenin pathway and maintain its growth to one of human colon cancer cell line.

    From the growing in vitro evidences suggests that sevoflurane potentially plays a role in the progression of tumors. In this study we revealed that sevoflurane potentially induced one of colorectal cancer cell line through MAPK and Wnt/β-catenin canonical pathway activation. Whereas two of our established Wnt knock out clones undo this proliferation and reduced the number of colonies after 2 weeks in regular culture. These suggested that Wnt genes that comprise a family of secreted lipid-modified signaling glycoprotein were one of key regulator after sevoflurane induced cell growth. To furthew explore we hypothesized that a potential of methylation would occur onto Wnt suppressor genes upon sevoflurane exposure, supporting the our found lower expressed those of suppressor genes. In the contrast our study could not revealed that sevoflurane potentially methylated DNA and lead to reduce the expression such as JMJD Wnt suppressor gene.
