    力学刺激が筋損傷からの回復を促進させることが分かっている。一方、筋損傷からの回復におけるリンパ管の役割は不明であり、その分布すら分かっていない。我々は、筋内のリンパ管が他の組織と異なる大きさや分布を呈すること、そのリンパ管の数や大きさが筋損傷後の回復早期に通常の約2倍に増加することを明らかにした。リンパ管は筋損傷時の毛細血管新生のトリガーとして機能し、筋損傷からの回復に不可欠な存在の可能性がある。そこで本研究は、力学刺激⇒リンパ管新生⇒筋損傷回復という、これまでにない新たな切り口から有効な理学療法を探る。
    本研究の目的は、筋損傷からの回復における筋内リンパ管の形態応答のメカニズムを組織学的、免疫組織学的そして生化学的に明らかにし、筋損傷におけるリンパ管の役割を解明することである。さらに、このリンパ管の役割に着目して、効果的な理学療法を探ることである。
    まず、我々の開発した、可動範囲や運動速度を自由に制御できる装置を改良し、マウス前脛骨筋に対して±5%の精度で筋損傷を作製できることを確認した。そして、損傷筋線維や炎症系細胞の浸潤が多くみられる損傷4日後にリンパ管数が急激に増加し、7日後に減少し損傷前の状態に戻る事することが明らかになった。一方、毛細血管数はそれより遅れて増加することも判明した。また、内皮細胞増殖因子であるVEGF-C/-D、VEGFR-3のmRNA発現量は、リンパ管数の増加とほぼ同じ時期に高いことが判明した。すなわち、筋損傷後のリンパ管新生のメカニズムには、癌細胞増殖や他の組織損傷後と同様のメカニズムの関与が考えられた。
    筋損傷からの回復促進効果が期待できる、伸長刺激を用いて、リンパ管や毛細血管の視点から検証した。伸長刺激は足関節を0.9 mNmと0.6 mNm で他動的に足関節底屈させることにより与えた。両トルクで休息5秒間を挟んで5秒間、75回を1セットとして、損傷24時間後に1セットのみ実施する条件と、それから毎日1セット実施する条件とで行った。その結果、損傷24時間後に0.6 mNmという比較的弱い伸長刺激を1セットのみ実施すると、損傷後の再生筋線維の指標といわれる中心核筋線維の平均面積が、コントロール群に対して大きい傾向にあることが判明した。なお、筋腹の深層部の 毛細血管密度の結果でのみ2日目に比べた7日目の有意に高値を示した。リンパ管の密度は全群間で有意差がなく、条件を絞った検証が必要であることが判明した。

    これまで,筋損傷におけるリンパ管の役割は不明で,その分布すら分かっていなかった。我々は,筋内のリンパ管が他の組織と異なる大きさや分布を呈すること,そのリンパ管の数や大きさが筋損傷後の回復早期に通常の約2倍に増加することを明らかにした。一方近年,リンパ管新生は周囲組織の毛細血管新生までも誘導し筋の壊死を抑えることが報告された。よってリンパ管は,筋損傷においても毛細血管新生のトリガーとして機能し,筋再生やエネルギー供給に寄与し,筋損傷からの回復に不可欠な存在かもしれない。本研究は,力学刺激⇒リンパ管新生⇒筋損傷回復という,これまでにない新たな切り口から有効な理学療法を探る。

    The guidelines were created with the aim of providing educational content that can be used in clinical practice, based on experimental research and anatomical and physiological evidence to support nursing techniques. The results are as follows:
    1) Several Japanese textbooks and American textbooks were translated, and new suggestions were obtained. 2) As research-based evidence, ① "Position changes and blood pressure changes" was added to the "Encouraging patients to get out of bed" section, ② "A new technique for intramuscular injections into the deltoid muscle" was added to the "Dealing with infection" section, ③ "Movement of the epiglottis when swallowing and the timing of pushing the gastric tube forward" was added to the "Meal assistance" section, and ④ "Care for lymphedema" was incorporated from a new perspective to understand "Assisting breast cancer patients." 3) Microscopic photographs were added to show actual situations at the cellular and tissue levels.

    The purpose of this study was to investigate the sleep conditions of elderly patients who had undergone orthopedic surgery of the lower extremities from the night before surgery until one week postoperatively. Elderly patients were studied using Mattress-type sleep monitoring device (SleepscanR SL-503; Tanita Co., Tokyo) and St. Mary’s Hospital Sleep Questionnaire. Sleep patterns of patients with knee/hip surgery were showed deprivation and fragment of sleep. Sleep parameters in Lumbar spine surgery group were not significantly between before and after surgery. There was no significant difference in sleep parameters between the two groups, lumbar spine surgery and knee/hip surgery.
    In the lumbar surgery group, it tended to be worse subjective sleep perception of postoperative day 6 than that before surgery.

    In the present study, trace biogas samples from pathological and physiological edema areas in mice and healthy subjects were analyzed by gas chromatography method without setting feature targets (Experiment 1; study using a rat hindlimb lymphedema model, Experiment 2; study using a mouse tail lymphedema model, Experiment 3; Search for trace biogas components specific to physiological edema).As a result, a maximum of over 1000 features were found.
    Further study is needed for finding a promising features and candidate substances.

    This study aimed to establish early detection and quantitative evaluation methods for upper limb and truncal lymphedema after breast cancer surgery. For upper limb lymphedema, ultrasonography, MRI, and fractal analysis were used to assess subcutaneous tissue structure. A correlation was found between fluid retention and structural complexity. Notably, some cases showed symptoms despite no fluid on MRI, suggesting fractal analysis may detect early changes undetected by conventional methods.For truncal lymphedema, a multifaceted method combining 3D scanning, MRI, and expert assessment was developed. The 3D scanner measured thickness differences accurately. All patients reported symptoms, even without MRI-confirmed fluid, highlighting the need for more sensitive assessments.These findings support improved early diagnosis and clinical application.

    This study aimed to clarify whether the skin surface structure, which remains poorly understood in patients with lymphedema, could be used as an indicator for evaluating the condition. Using existing magnified images, we analyzed skin furrows and ridges in patients with upper limb lymphedema. The analysis showed that the proportion of the skin ridge area tended to increase on the affected side, suggesting this may be a characteristic feature of lymphedema.
    We also examined changes in skin surface structure before and after the onset of physiological edema in the lower limbs of healthy women. Results indicated that the ridge ratio in the peroneal region was higher than in other areas. These findings support the potential usefulness of magnified skin surface images for evaluating edema. Our study suggests that such imaging could provide a simple, noninvasive method to support the assessment and monitoring of lymphedema in clinical settings.

    The purpose of this present research was to develop nursing care to prevent fiberization after lymphedema.
    A lymphedema model rat was made, and then interstitial pressure was measured by Pressure measurement catheter(Miller). Moreover, alterations of interstitial pressure was measured when manual lymph drainage or bandage was applied on it.
    To drain lymph from distal to proximal in the lymphedema leg, a strongish pressure was needed. However, we couldn't find how such strong pressure causes lymph vessels and interstitial tissues for an extended period.

    We attempted to establish a care program for breast cancer-related lymphedema in diabetic patients. We examined from various viewpoints based on the complete decongestive therapy (CDT).
    MR images of the forearm revealed that fluid accumulates in ulnar side more than radial side. Therefore, it is recommended that the drainage should focus on this part. After 120 minutes of meal, it was found that blood pressure stabilize. Hence CDT should be carried out at 120 minutes after a meal. Because the evaluation using a tape measure or subjective symptoms was not sufficient, in order to understand the situation of edema, magnetic resonance imaging or ultrasonic diagnostic imaging is necessary. In self-care, it is important to visually grasp the effects of care in order to encourage patients. For this purpose, we considered to use a 3D scanner.

    The purpose of the present study was to elucidate the sleep conditions of patients who applied J-PAD guideline after surgery through in an ICU and general wards. Sleep parameter from 21 patients who underwent cardiac surgery, craniocervical cancer reconstructive surgery, thermotherapy of digestive cancer were analyzed. In term of sleep duration, duration of awakening, and sleep efficiency, sleep quality of patients in the ICU seemed better than those in the surgical ward after discharge from ICU. Sleep on 3-4 th night in the surgical ward was fragmented. The quality of sleep decreased. Analgesics/sedative management based on J-PAD could improve sleep quality of patients who underwent surgery in the ICU, but those in the surgical ward was disturbed when the analgesia/sedative management was discontinued.

    Multi-layer bandaging (MLB) are very effective in the management oflymphedema. However, MLB is considered to add stress to patients such as pressure and restricted movement on extremity, hence, MLB during night may disrupt their sleep.This study examined whether MLB disrupt the sleep.This study recruited 14 healthy female volunteers and six female patients with stage II lymphedema as a result of breast cancer surgery. Both of the healthy volunteer group and the patient group were required to wear a wrist actigraph for 2 weeks. In the healthy volunteer group, MLB increased only WASO on the first day but WASO recovered on the seventh day. In the patient group, MLB shorten SLAT on the seventh day, meaning to fall asleep easily.
    These results suggest that wearing MLB may not reduce the quality of sleep during the night in patients with breast cancer-related lymphedema.
