    一側性難聴者は、騒音下や多人数での会話において聞き取りが不自由であるにも関わらず、本邦では医療、教育、社会的支援に目が向けられてこなかった。令和2年度の日本医療研究開発機構(AMED)岩崎班の研究において、一側性難聴者の障害の現状とその対応方法に関する研究が行われた。その結果、雑音下語音検査では、検査結果に個人差が多く、医学的介入(補聴器、人工内耳等)の適応評価のための検査条件をさらに詳細に検討する必要があることがわかった。本研究の目的は、雑音下語音検査の検査条件を確立することであり、多機関共同研究とすることでより多くのデータを収集・分析し、雑音下語音検査の標準化を目指す。

    メニエール病は、めまいや難聴などの症状を生じ生活の質を著しく低下させる疾患である。発作予防に対してさまざまな段階的治療法が推奨されているが、その有効性には限界がある。低侵襲で有効な治療法として、100 Hzの軽微な音刺激を連続して与えるという新たな治療法を提示し、症状や内耳機能改善効果を解析しその有効性を検証する。
    めまいや聴力低下を生じるメニエール病は保存的治療で対応が可能である症例も多いが、難治性のめまいを伴う平衡障害は患者のQOLに大きな影響を及ぼし、今までそのような症例では外科的治療を含む侵襲的治療が選択されてきた。本研究では、治療抵抗性のメニエール病に対して新たに低侵襲で有効な治療法の開発を目標とし、100 Hzの軽微な音刺激が平衡感覚を改善する可能性についてメニエール病症例の低下した前庭耳石機能改善を目指した研究を開始した。
    難治性めまいで来院しMRIにて内リンパ水腫が確認されたメニエール病確実例で同意の得られた症例を対象に、試作した音発生装置を用いて100Hz 75dBの刺激音を両耳に5分間与えた。刺激前後で、蝸牛機能評価として耳音響放射、平衡機能として重心動揺検査および前庭誘発筋電位検査を行った。刺激後に耳音響放射の変化はなく、蝸牛障害の副作用は生じないことを確認した。一方重心動揺検査では、刺激後に総軌跡長および外周面積のロンベルグ率が有意に低下した。また、刺激後に患側の低下した前庭誘発筋電位の振幅が有意に増大した。一方250Hz 75dBの刺激音を5分間与えたコントロール群では、ロンベルグ率の改善は認められず、かえって患側の前庭誘発筋電位の振幅は有意に減少した。
    100Hz 75dBの刺激による重心動揺の改善は耳石を介した作用であり、聴覚系を介した作用と推定している。この結果を得て、音刺激時間を10分間に延長し、さらなる評価を行った。特に違和感を感じた症例はなく、遂行可能であった。
    本研究でリクルートしたメニエール病患者は、保存的な治療でもコントロールが不良な不可逆期または進行期の症例であった。初診からMRI検査による内リンパ水腫確認および刺激検査まで数ヶ月を要したため、音刺激検査の際にはめまいや聴力改善のあった症例も存在したが、そのような症例も含めて100Hz 75dBの刺激音の有効性が確認されている。当初から使用している音発生装置は試作品のためサイズが大きい。今後の利便性を考慮し、ポータブルタイプの音発生装置を企業の協力を得るべく、特定臨床研究承認を得て共同研究契約もR4年度内に締結した。R5年度途中から、ヘッドホンタイプの装用機器にて音刺激研究を継続している。
    ヘッドホンタイプの音刺激機器を用いてエビデンスを確立すべく、無作為抽出にて音刺激研究を継続し、症例の集積を行っていく。単施設での症例組み込みでは限界があるため、多施設共同研究の特定臨床研究へと発展させる予定である。

    In a comparison of bilateral and unilateral implantation in sequential bilateral cochlear implant surgery cases more than 6 months after implantation, SRT was better in bilateral implantation, but the first side tended to be slightly better. However, in the interactive spatial listening test, there was no difference in the direction of the noise. However, in the interactive spatial listening test, binaural hearing aid users showed better discrimination of the minimum angle. Additive and squelch effects due to binaural use were observed, but limited effects were suggested for SRM. This is a new finding that the effect of noise on hearing is important, and that an environment in which the effect of noise on the good hearing ear is minimal is better.

    The results of ABR in R702C-Atoth1 mice revealed that some mice caused hearing loss and others did not. Three out of 46 mice showed 70 dB or more in all frequency ranges. Immunostaining was performed using NMMHC-IIA (MYH9). A negative control was prepared. The expression site of the NMMHC-IIA in the inner ear was compared in three individuals in one wild-type gene expression and two expressing the deafness in the R702C-Atoth1 mice. As shown in FIGS. 2 and 3, the expression was almost the same as that of the wild type, and there was no difference between individuals. No morphological changes were observed on light microscopy and immunostaining. Since only a very small amount of the inner ear tissue sample can be obtained, the amount was insufficient to confirm that the MYH R702C gene was contained in the inner ear in a tissue-specific manner. In the future, we will focus on the morphological change of sensory hair, which is a finer structure, and study it repeatedly.
