[要旨] 本研究の目的は，慢性疾患を有しながら独居生活を送っている男性高齢者の老いの体験を明らかにすることである。訪問看護を利用している60代前半から70代前半の独居男性６名に対して半構造化インタビューを行い，解釈的現象学に基づいて分析した。 研究参加者は糖尿病や脳卒中などによる失明，運動麻痺などの身体障害を有していた。分析の結果，中テーマ：［生活の困難を体験したからこそ，病気を自覚し，食生活を見直す］・［変化した体に残された力を見出し，発揮して，暮らす］・［仕事に代わる時間の使い方を見つけ，仕事をしていない自分を認める］・［生活のために他者からの支援を求め，受け入れて暮らす］・［思い通りにならない生活の中で，一貫して自分らしく暮らそうとする］と，全体テーマ：【体が変化したことによって，生活の困難を体験した一方で，病気の自覚と変化した体に内在する可能性に気づくことができ，自分の力と支援の両方を支えにして，思い通りにならない生活の中で一貫して自分らしく暮らす】が明らかになった。 研究参加者らは多様な人間関係が重要であった，生きがいとも言える一貫した “自分らしい暮らし” によって老いを肯定的に捉えていたが，ときに，病気は他者との繋がりを困難にした。つまり，訪問看護師には，“自分らしい暮らし” に関連した人間関係を捉えて老いの体験を解釈し，高齢者と地域を繋ぐ存在であることが求められるだろう。
[SUMMARY] This study aimed to explore the aging experiences of older men living alone with chronic diseases. We conducted semi-structured interviews with six men aged <75 years, who live alone and use the services of visiting nurses. The data were analyzed following interpretive phenomenology. Our results showed that the participants were facing challenges, and five themes, alongside one holistic theme, were identified. The five themes were: (a) disease awareness and reviewing diet due to difficulties in performing daily life tasks, (b) identifying my body’s potential ability to face physical challenges, (c) finding ways to spend free time after retirement and allowing myself not to work, (d) accepting support from others to perform daily life activities, and (e) wishing to be myself despite the inconvenience of changes to daily living. The holistic theme was: While forced to experience difficulties in daily life due to the change in my physical status, I am aware of my disease and potential ability. Thus, I invariably live as myself in an inconvenient daily lifestyle by utilizing my strength and support from others. We observed that the participants were positively aware of their aging process with their unchanging personality, i.e., IKIGAI (a reason for being), through their diverse social relationships. However, one participant had difficulty connecting with people due to his disease. In conclusion, we suggest that visiting nurses should enrich the aging experiences of men with chronic diseases through understanding social interactions related to IKIGAI, and by serving as a link between the elderly and the community.
